Prevalence rates for constipation and faecal and urinary incontinence.

OBJECTIVE: To evaluate the prevalence rates for constipation and faecal and urinary incontinence in children attending primary care clinics in the United States. METHODS: Retrospective review of case records of all children, 4-17 years of age, seen for at least one health maintenance visit during a 6 month period and followed from birth or within the first 6 months of age in our clinics. We reviewed all charts for constipation, faecal incontinence and urinary incontinence. RESULTS: We included 482 children in the study, after excluding 39 children with chronic diseases. The prevalence rate f

Reducing adolescents' aggressive and hostile behaviors: randomized trial effects of a brief family intervention 4 years past baseline.

OBJECTIVE: To examine the long-term effects of a brief family intervention on aggressive and hostile behaviors of adolescents in the general population. DESIGN: Randomized trial including 22 public schools assigned to the Iowa Strengthening Families Program or a control condition. Analyses supported sample representativeness and failed to show differential attrition effects 4 years after baseline. INTERVENTION: Seven-session intervention for parents and their sixth-grade children. MEASURES: The multi-informant, multimethod measures included independent observer ratings of adolescent aggress…

Contested claims in carpal tunnel surgery: outcome study of worker's compensation factors.

UNLABELLED: We retrospectively analyzed the importance of factors relating to worker's compensation for 273 wrists in 211 consecutive patients who underwent primary carpal tunnel release. Patients were divided into three groups: non-work related, worker's compensation-uncontested, and worker's compensation-contested. Contested claims were those in which the worker's compensation carrier denied authorization for surgery, and in which such authorization was given following intervention by a plaintiff's attorney. RESULTS: there were no statistically significant differences in postoperative retur

Use of adaptive behavior measures and overrepresentation of black students in programs for students with mild mental retardation.

The use of adaptive behavior information for classification and placement of black and white students with mild mental retardation was compared and overrepresentation of black students in special classes was analyzed. Adaptive behavior measures were not used to the same extent as general intelligence tests for either group, despite court decisions and recommendations by authoritative sources. Direct comparisons on general intellectual functioning and several indices of adaptive behavior yielded few differences, suggesting that an equal treatment conception of fairness was achieved despite subs

Associations in emergence age among permenent teeth.

Associations in the timing of emergence among the permanent teeth of Boston children were obtained from a mixed longitudinal growth study of 414 Caucasian twin pairs examined annually. Correlations were estimated by the method of maximum likelihood for corresponding left-right teeth, upper-lower teeth, and all paired combinations from central incisor to second molar in a jaw quadrant of each sex. Strong positive correlations in emergence timing prevailed throughout the dentition. Principal component analyses on correlation matrices of jaw quadrant relations for boys and girls in the maxilla an

Is a familial definition of depression both feasible and valid?

On the basis of familial constellations it is possible to separate out most of the unipolar depressive patients that are seen in the hospital. The groups may be called depression spectrum disease, pure depressive disease, and sporadic depressive disease. The sporadic depressive disease group is older at onset and older at index of admission than the other two groups. The depression spectrum disease and pure depressive disease groups are separated by virtue of the fact that there are more episodes of illness in the latter.

Clinician Perspectives on Barriers and Facilitators to Implementing e-Health Technology in Substance Use Disorder (SUD) Treatment Facilities.

BACKGROUND: Substance use disorders (SUDs) in the United States cause many preventable deaths each year. Finding effective ways to manage SUDs is vital to improving outcomes for individuals seeking treatment. This has increased interest in using e-health technologies in behavioral healthcare settings. This research is part of a larger study evaluating the efficacy of the NIATx coaching intervention for implementing RISE Iowa, an e-health patient recovery app, in SUD treatment organizations and seeks to examine clinician perspectives of the barriers and facilitators to its implementation. METH…

Effects of cocaine and HIV on decision-making abilities.

Our study aimed to understand the impact of cocaine dependence on high-risk decision-making abilities in individuals with the human immunodeficiency virus (HIV) and individuals with cocaine dependence. We recruited 99 participants (27 HIV/Cocaine, 20 HIV Only, 26 Cocaine Only, and 26 Healthy Controls). The Iowa Gambling Task (IGT) was applied to assess decision-making abilities. Independent and interactive effects of HIV status and cocaine dependence were examined using 2 × 2 factorial ANCOVA with premorbid IQ (WRAT-4: WR) as the covariate. We found cocaine dependence had a significant adverse…

Enduring Changes in Decision Making in Patients with Full Remission from Anorexia Nervosa.

BACKGROUND: Deficits in neuropsychological functioning have consistently been identified in patients with anorexia nervosa (AN). However, little is known on how decision making in AN patients evolves in response to treatment or whether impairments are reversible. METHOD: AN patients (n = 42) completed the Iowa Gambling Task (IGT) upon admission to a 3-month day-hospital treatment programme and at a 1-year follow-up. Patient IGT performance was compared to age-matched controls (n = 46). RESULTS: AN patients displayed poorer performance on the IGT at admission compared to controls (p < .001). …

Variations in the GRIN2B gene are associated with risky decision-making.

The dopaminergic system is known to modulate decision-making. As N-methyl-D-aspartate (NMDA) receptors strongly influence dopaminergic function, it is conceivable that the glutamatergic system is also involved in decision-making. We examined whether polymorphisms in the N-methyl-d-aspartate receptor 2B subunit gene (GRIN2B) influence decision-making using the Iowa Gambling Task (IGT). In total, 245 (n = 245, 127 female) healthy German students were included in the analysis. Two synonymous SNPs in exon 13, rs1806191 (H1178H) and rs1806201 (T888T) showed the strongest association with aspects of…

New questions and insights into nitrate/nitrite and human health effects: a retrospective cohort study of private well users' immunological and wellness status.

The retrospective cohort study described in this article examined 150 Iowa private well users' (aged 1-60 years) drinking water below the 10 parts per million nitrate-nitrogen (nitrate-N) maximum contaminant level and analyzed health history data and blood samples including hemoglobin fractions and immunological parameters. Positive associations existed (bivariate fit) between higher nitrate exposure and body mass index, lower recreational activity, perceived poorer health, and perceptions of susceptibility to illness. A directly proportionate relationship was seen between methemoglobin level …

The impact of mild cognitive impairment on decision making in two gambling tasks.

OBJECTIVES: This study aimed to investigate whether patients with mild cognitive impairment (MCI) present difficulties in making decisions under ambiguity and under risk. METHODS: Performance of MCI patients in the Iowa Gambling Task (IGT) and in the Probability-Associated Gambling Task-Revised (PAG-R) was compared with performance of healthy aging peers. RESULTS: In the IGT, controls made increasingly frequent advantageous selections over time; MCI patients selected randomly from advantageous and disadvantageous decks, with no significant change in performance over time. In the PAG-R, contr…

Is decision making really impaired in eating disorders?

OBJECTIVE: Decision making has been reported to be reduced in eating disorders. However, studies are sparse and have been carried out in various selected populations. In the current study we arranged to confirm previous observations and to assess the relationship between decision making and dimensions relevant to eating disorders. METHOD: Patients suffering from anorexia nervosa (n = 49), bulimia nervosa (n = 38), and healthy controls (n = 83) were assessed using the Iowa Gambling Task (IGT). All patients were euthymic and free of psychotropic medication. Self-questionnaires (Eating Disorder …

Age-related quantitative and qualitative changes in decision making ability.

The "frontal aging hypothesis" predicts that brain senescence affects predominantly the prefrontal regions. Preliminary evidence has recently been gathered in favour of an age-related change in a typically frontal process, i.e. decision making, using the Iowa Gambling Task (IGT), but overall findings have been conflicting. Following the traditional scoring method, coupled with a qualitative analysis, in the present study we compared IGT performance of 40 young (mean age: 27.9+/-4.7) and 40 old (mean age: 65.4+/-8.6) healthy adults and of 18 patients affected by frontal lobe dementia of mild se…
