Early onset behavioral variant frontotemporal dementia due to the C9ORF72 hexanucleotide repeat expansion: psychiatric clinical presentations.

A hexanucleotide repeat expansion in the first intron of C9ORF72 has been shown to be responsible for a high number of familial cases of amyotrophic lateral sclerosis or frontotemporal lobar degeneration with or without concomitant motor neuron disease phenotype and TDP-43 based pathology. Here, we report on three cases carrying the hexanucleotide repeat expansion with an atypical presentation consisting in the development of psychiatric symptoms. Patient #1, a 53 year old man with positive family history for dementia, presented with mood deflection, characterized by apathy, social withdraw, a

Potential health effects of odor from animal operations, wastewater treatment, and recycling of byproducts.

Complaints of health symptoms from ambient odors have become more frequent in communities with confined animal facilities, wastewater treatment plants, and biosolids recycling operations. The most frequently reported health complaints include eye, nose, and throat irritation, headache, nausea, diarrhea, hoarseness, sore throat, cough, chest tightness, nasal congestion, palpitations, shortness of breath, stress, drowsiness, and alterations in mood. Typically, these symptoms occur at the time of exposure and remit after a short period of time. However, for sensitive individuals such as asthmatic

The premenstrual phase and reactions to aversive events: a study of hormonal influences on emotionality.

Fifty-eight normal cycle, healthy women were confronted with an aversive, anger-provoking situation in the laboratory. Eighteen women were tested in their follicular phase. A further 40 women were tested in the premenstrual phase, half of whom reported suffering from complaints of premenstrual emotional lability and irritation, the other half reported being without premenstrual problems. Apart from a strong effect of emotion induction on cardiovascular arousal and anger-related moods in the follicular and premenstrual groups, a premenstrual phase effect was also demonstrated, with premenstrual

Daily academic and sport-related strain and mental-health-related states among university students in sport-related majors: a 14-day ecological momentary assessment network study.

University students enrolled in sport-related majors may simultaneously manage academic requirements and regular training or competition, yet the short-term organization of their physical, academic/performance-related, affective, and cognitive states remains insufficiently understood. This study used a 14-day ecological momentary assessment (EMA) design with three smartphone assessments per day and multilevel vector autoregressive modeling. The original recruitment pool included 72 students from one university in Chongqing, China. Five participants were excluded for repeated responses outside

Delineating bipolar II mixed states in the Ravenna-San Diego collaborative study: the relative prevalence and diagnostic significance of hypomanic features during major depressive episodes.

BACKGROUND: Depressive mixed state (DMX), defined by hypomanic features during a major depressive episode (MDE) is under-researched. Accordingly, study aims were to find DMX prevalence in unipolar major depressive disorder (MDD) and bipolar II depressive phase, to delineate the most common hypomanic signs and symptoms during DMX, and to assess their sensitivity and specificity for the diagnosis of DMX and bipolar II. METHODS: 161 unipolar and bipolar II MDE psychotropic drug- and substance-free consecutive outpatients were interviewed during an MDE with the Structured Clinical Interview for D

A pilot study of brexpiprazole for bipolar depression.

BACKGROUND: Past studies suggest that brexpiprazole is an effective adjunctive treatment for major depressive disorder and schizophrenia; however, no studies have examined brexpiprazole for bipolar depression. In this study, we examined the effects of brexpiprazole on mood, cognition, and quality of life in outpatients with bipolar depression. METHODS: Twenty-one adults with bipolar disorder (most recent episode depressed) and scoring at least a 25 on the Montgomery-Åsberg Depression Rating Scale (MADRS) were recruited. Brexpiprazole was titrated up to 4 mg/day over the 8-week period. Depress

Loxapine add-on for adolescents and adults with autism spectrum disorders and irritability.

OBJECTIVES: Our clinical experience with low dose loxapine (5-15 mg/day) suggests promising efficacy and safety for irritability in autism spectrum disorders (ASD). We studied low dose loxapine prospectively in adolescents and adults with ASD and irritability. Additionally, we measured loxapine and metabolite concentrations, and brain-derived neurotrophic factor (BDNF) as a biomarker of neuromodulation. METHODS: We performed a 12 week open trial of add-on loxapine in subjects, ages 13-65 years, diagnosed with ASD, and Aberrant Behavior Checklist-Irritability (ABC-I) subscale scores >14. Loxap

Managing Menopausal Symptoms: Common Questions and Answers.

Menopausal symptoms are widespread and significantly impact quality of life. Common symptoms of menopause are vasomotor (i.e., hot flashes and night sweats) and genitourinary (e.g., vulvovaginal irritation and dryness, dyspareunia, urinary problems), although women may also experience changes in sexual function, mood, and sleep. Estrogen-containing hormone therapy is effective treatment for vasomotor symptoms. Nonhormonal medications for vasomotor symptoms include selective serotonin reuptake inhibitors, serotonin-norepinephrine reuptake inhibitors, and gabapentin. Selective serotonin reuptake

Evidence-Based Assessment of DSM-5 Disruptive, Impulse Control, and Conduct Disorders.

The assessment of oppositional defiant disorder, conduct disorder, antisocial personality disorder, and intermittent explosive disorder-the Disruptive, Impulse Control and Conduct Disorders-can be affected by biases in clinical judgment, including overestimating concerns about distinguishing symptoms from normative behavior and stigma associated with diagnosing antisocial behavior. Recent nosological changes call for special attention during assessment to symptom dimensions of limited prosocial emotions and chronic irritability. The present review summarizes best practices for evidence-based a

Three case studies of nonparaneoplastic limbic encephalitis (NPLE) in young adult males: Onset, course, and recovery following rehabilitation services.

Limbic encephalitis (LE) is a rare neurological disorder characterized by inflammation of the brain caused by autoimmunity or infection. LE has been a difficult to define and diagnose disorder due to the insidious and nonspecific (e.g., irritability, low mood, short-term memory complaints) presentation of early symptoms, as well as inconsistent findings on neuroimaging, lumbar puncture serum analysis, and electroencephalogram. Seizures, memory problems, and psychiatric disturbance are among the earliest and most prominent clinical features. This manuscript describes three adolescent males who

Aripiprazole in the treatment of challenging behaviour in adults with autism spectrum disorder.

BACKGROUND: Autism spectrum disorders (ASD) are associated with repetitive behaviours and often also with hyperactivity, aggression, self-injurious behaviour, irritability and lability of mood. There is emerging evidence that aripiprazole, an antipsychotic with partial agonist dopaminergic effect, may be effective in the treatment of these challenging behaviours. Nevertheless, there is little evidence for their efficacy in adults with ASD. OBJECTIVES: The aim of this article is to present preliminary data on the use of aripiprazole in the treatment of challenging behaviour in the setting of A

Management of treatment-refractory panic disorder.

Treatment resistance remains a relatively common problem in panic disorder (PD) despite the success of the selective serotonin reuptake inhibitors (SSRIs) and cognitive behavioral therapy (CBT) as first-line agents. Factors contributing to medication treatment resistance include inadequacy of trial duration, improper dosage, poor tolerability, noncompliance, and medical and psychiatric comorbidity. Poor tolerability to the SSRIs can frequently be addressed by judicious lowering of the initial dose, with a gradual upward titration. For patients who have not responded to one or more adequate tri

Network analysis of anxiety, depressive symptoms, and suicidal tendencies among nursing students with recent suicidal ideation.

BACKGROUND: Suicide prevention is increasingly gaining attention, especially for those who have suicidal ideation. Nursing students face unique stressors from academic and clinical demands, leading to high rates of anxiety, depressive symptoms, and suicidal tendencies. While previous research has focused on prevalence and single-variable associations, the complex interrelationships among these mental health issues remain understudied. This study aims to use network analysis to explore the connections between anxiety, depressive symptoms, and suicidal tendencies in nursing students with recent
