Treatment of acute and chronic anterior cruciate ligament-posterior cruciate ligament-lateral side knee injuries.

Knee dislocations are rare but devastating injuries. The ACL-PCL-lateral side injury combination is representative of the challenges these injuries present. Early management is focused on vascular integrity. When possible, acute repair and reconstruction within 3 weeks from injury is preferred. Chronically deficient knees generally will require lateral side reconstruction rather than repair and may require limb realignment. Addressing all injured structures is imperative to afford the best chance at a reasonable outcome. Good results with surgery are possible, but some degree of persistent dis…

Examination under Anesthesia in Patients with Recurrent Patellar Dislocation: Prognostic Study.

Surgical management of patellar instability is a matter of wide debate. While some authors believe that isolated medial patellofemoral ligament (MPFL) reconstruction is sufficient, others advocate procedures to treat the underlying pathology, namely, patella alta and trochlear dysplasia. Radiological studies have been traditionally used as a determinant of these pathologies and to determine the need for an additional procedure. The value of examination under anesthesia (EUA) in formulating a treatment plan remains unknown. Our aim is to assess the prognostic value of EUA in assessing patients

Medial Patellofemoral Ligament Reconstruction Using Suture Tape for Patellofemoral Joint Instability.

OBJECTIVE: To describe a surgical technique using suture tape for reconstruction of the medial patellofemoral ligament (MPFL). This technique restores the stability of the reconstructed ligament and has excellent postoperative outcomes. METHOD: This is a retrospective analysis. From January 2016 to June 2018, 17 patients underwent MPFL reconstruction using high-strength suture (FiberTape; Arthrex) augmentation, with at least 12 months of follow up. There were 11 female and 6 male patients. The mean age at the time of MPFL reconstruction was 22.1 years (range 13-34 years). Clinical outcomes in

Age-Dependent Susceptibility to Mechanical Unloading-Induced Femoral Morphological Changes and Patellar Instability in a Rat Model.

Abnormal bone morphology and joint malalignment are key risk factors for several musculoskeletal disorders, including patellar instability and lateral patellar dislocation (LPD). Insufficient weight loading can affect bone formation in immature joints; however, its impact on mature joints remains unclear. The aim of this study was to compare the effects of hindlimb suspension (HS) on femur morphology and knee alignment associated with patellar instability and LPD in rats with different levels of musculoskeletal maturity. Immature (4-week-old) and mature (12-week-old) female rats underwent 8 we

Lateral Collateral Ligament and Proximal Tibiofibular Joint Reconstruction for Tibiofibular Instability.

Instability of the proximal tibiofibular joint (PTFJ) can be post-traumatic or due to accumulative injuries and may also be underdiagnosed pathology that can present with symptoms of lateral and/or medial knee pain. It can be associated with subtle instability and subluxation or frank dislocation of both the PTFJ and the native knee joint. Previously described techniques have been either nonanatomic, require secondary hardware removal, disrupt native anatomy, or fail to account for the inherent stabilizing effect of the lateral collateral ligament, which is likely additionally injured or lax i

[Ultrasound diagnosis in knee and foot trauma].

Subsequent to clinical and radiologic examination, ultrasonography is an important diagnostic method in knee and foot trauma, especially to assess pathologic conditions of the periarticular soft tissue. The main indications for ultrasonographic assessment of the knee region are fluid conglomerations, injuries of tendons, ligaments, and muscles, and vascular diagnosis. The indications for ultrasonography in the foot region are diagnosis of foreign bodies, dislocation of the peroneal tendons, lesions to the flexor and extensor tendons, and osseous capsular and ligamentous avulsions. Changes of t

Acute traumatic proximal tibiofibular dislocation: Treatment of three cases.

INTRODUCTION: Acute traumatic dislocation of the proximal fibula occurs in an anterolateral, posteromedial, or superior direction. The dislocation is seen both isolated and in combination with other injuries of the leg. A dislocation is an uncommon injury. We have recently treated three patients with this injury which we believe will illustrate some treatment aspects. CASE REPORTS: Case 1: A 25-year-old man fell in a football match. He had pain in his leg especially proximally. There was a prominent fibular head on inspection. X-rays showed an anterolateral dislocation in the proximal tibiofi

Symptomatic snapping knee from biceps femoris tendon subluxation: an unusual case of lateral pain in a marathon runner.

Snapping biceps femoris syndrome is an uncommon cause of lateral knee pain and may be difficult to diagnose, resulting in unsuccessful surgical intervention. In this report, we present an unusual case of a 37-year-old male marathon runner with unilateral snapping knee secondary to dislocation of the long head of the biceps femoris over the fibular head during knee flexion. The pain was great enough to interfere with his ability to practice sport. Possible causes of symptomatic snapping knee include multiple intra-articular or extra-articular pathology. Biceps femoris snapping over the fibular

Knee Injuries in Baseball.

While knee injuries account for just 7% of injuries in professional and collegiate baseball, they are the fourth most common cause of missed professional games. The 3 most common knee-related issues in baseball are patellar tendinopathy, meniscus tears, and medial collateral ligament sprains. While injuries such as anterior cruciate ligament tears and patellar dislocations are less common, they are notable due to their potentially significant effect on time missed. Treatment considerations of all these pathologies are multifactorial and should include skeletal maturity, level of play, timing o

Physical diagnosis of the multiple-ligament-injured knee.

Knee dislocations and related knee trauma represent dramatic orthopedic injuries. The severity and spectrum of injury may not be immediately evident to the clinician. A high index of suspicion for associated neurovascular involvement must be maintained throughout the initial care of the patient. In anticipation of ligament repair and reconstruction, a preoperative MR imaging study supplemented by an examination under anesthesia allows for an accurate assessment of the pattern and severity of ligament injury. With this knowledge the surgeon is able to prepare properly for surgery and create app…

Habitual dislocation of the patella in flexion.

Thirty-five patients with habitual dislocation of the patella in flexion were reviewed; eight were bilaterally affected. Each had undergone quadricepsplasty with an average follow-up of 6 years 9 months. Bands or contractures, most commonly in vastus lateralis, the iliotibial tract and rectus femoris were seen in each. Redislocation was seen in 12 knees. At review, 79% of the knees were normal. Quadriceps lengthening is an essential part of treatment and must be performed proximally. Causes for failure include reformation of contractures and failure to correct the initial abnormality fully.

Multiligament-injured knee: what the general orthopedic surgeon should know.

Multiligament knee injuries (MLKIs) are rare but severe injuries involving bicruciate or collateral ligament disruption, frequently associated with knee dislocation, fractures, and neurovascular compromise. Vascular injury occurs in a mean of approximately 18% of cases and may be present despite palpable pulses; an ankle-brachial index (ABI) < 0.9 demonstrates high sensitivity for arterial injury. Peroneal nerve injury occurs in approximately 10-40% of cases. Early recognition and structured evaluation are critical. Serial vascular examinations, selective CT angiography, and careful neurologic…

Clinical outcomes after medial patellofemoral ligament reconstruction: an analysis of changes in the patellofemoral joint alignment.

PURPOSE: Medial patellofemoral ligament (MPFL) reconstruction using the hamstring tendon is widely performed to treat recurrent patellar dislocation. MPFL reconstruction includes a post-operative process of necrosis and reperfusion of the hamstring tendon graft. We hypothesise that the patella gradually shifts laterally because of this process, ultimately affecting the patellofemoral joint alignment. This study aimed to analyse the chronological changes in the patellofemoral joint alignment and the outcomes of MPFL reconstruction. METHODS: In this retrospective case-series study, the Knee Soc

Revamping Stability: Tibial Tuberosity Transfer and Medial Patello-Femoral Ligament Reconstruction for Patella Alta and Recurrent Dislocation.

This case report details the clinical evaluation, imaging findings, and surgical management of a 17-year-old female with a two-year history of persistent knee pain and recurrent patellar dislocations. Despite the absence of traumatic injury, the patient exhibited significant anatomical abnormalities, including a laterally dislocated patella, shallow trochlear groove, increased tibial tuberosity to trochlear groove (TT-TG) distance, and patella alta by calculating Insall-Salvati ratio. The Insall-Salvati ratio is a radiographic measurement used to assess the position of the patella within the k

A modified tibial tubercle osteotomy for patellar maltracking: results at two years.

An abnormal lateral position of the tibial tuberosity causes distal malalignment of the extensor mechanism of the knee and can lead to lateral tracking of the patella causing anterior knee pain or objective patellar instability, characterised by recurrent dislocation. Computer tomography is used for a precise pre-operative assessment of the tibial tubercle-trochlear groove distance. A distance of more than 15 mm is considered to be pathological and an indication for surgery in symptomatic patients. In a prospective study we performed a subtle transfer of the tibial tuberosity according to the

Primary patellar dislocations without surgical stabilization or recurrence: how well are these patients really doing?

PURPOSE: While a significant research has gone into identifying patients at highest risk of recurrence following primary patellar dislocation, there has been little work exploring the outcomes of patients who do not have a recurrent patellar dislocation. We hypothesize that patients without recurrent dislocation episodes will exhibit significantly higher KOOSs than those who suffer recurrent dislocations, but lower scores than published age-matched normative data. METHODS: A retrospective review of patients with nonoperatively treated primary lateral patellar dislocations was carried out, and

Habitual patellar dislocation 40 years after failed conservative treatment: A case report.

Patellar dislocation is often caused by congenital bone morphological abnormalities or joint laxity and is rarely traumatic. We report a case of long-term habitual patellar dislocation due to trauma 40 years ago without abnormal bone morphology, which required a combined procedure, including the Elmslie-Trillat (ET) procedure, massive lateral release, and medial patellofemoral ligament (MPFL) reconstruction, resulting in favorable outcomes. A 52-year-old male, with no specific underlying disease, dislocated his right patella due to falls when he was 13 years old and underwent conservative trea
