PURPOSE OF REVIEW: A decade has passed since vestibular migraine (VM) was formally established as a clinical entity. During this time, VM has emerged amongst the most common cause of episodic vertigo. Like all forms of migraine, VM symptoms are most prominent during individual attacks, however many patients may also develop persistent symptoms that are less prominent and can still interfere with daily activities.
RECENT FINDINGS: Vestibular inputs are strongly multimodal, and because of extensive convergence with other sensory information, they do not result in a distinct conscious sensation.

Dizziness is very common, but it is never normal. Dizziness can make performing daily activities, work, and walking difficult. Inner ear balance problems can make people dizzy when they turn their head, which can cause problems during walking and make people more likely to fall. Most of the time dizziness is not from a life-threatening disease. Often, dizziness is related to a problem of the vestibular (or inner ear balance) system. Vestibular disorders can be caused by infections in the ear, problems with the immune system, medications that harm the inner ear, and rarely from diabetes or stro

OBJECTIVES: Traumatic brain injury occurs frequently worldwide. Half of traumatic brain injuries are related to falls or motor vehicle accidents. The term "concussion" is often used to describe a minor form of traumatic brain injury. These often involve decelerative events to the head (e.g., flexion/extension injury) and can also cause damage to the vestibular system of the inner ear.
MATERIALS AND METHODS: The European Society for Clinical Evaluation of Balance Disorders meets yearly and has proposed an investigation and analysis of the vestibular consequences of traumatic brain injury. This

PURPOSE OF REVIEW: To present evidence of a functional interrelation between the vestibular and the anxiety systems based on a complex reciprocally organized network. The review focuses on the differential effects of various vestibular disorders, on psychiatric comorbidity, and on anxiety related to vertigo.
RECENT FINDINGS: Episodic vertigo syndromes such as vestibular migraine, vestibular paroxysmia, and Menière's disease are associated with a significant increase of psychiatric comorbidity, in particular anxiety/phobic disorders and depression. Chronic unilateral and bilateral vestibulopat

OBJECTIVES: To evaluate the relationship between the loss of the cochleovestibular functions in Ménière's disease (MD).
METHODS: Forty-three patients with definite MD underwent pure-tone audiometry (PTA) and caloric test. Canal paresis (CP%), dPTA (interaural difference), and average PTA results were contrasted. IBM SPSS V24 was used for statistical analysis.
RESULTS: According to PTA, most patients were in stage C, and caloric weakness was found in 29 patients. Linear (R2 = 0.06) and nonlinear correlation tests (rho = 0.245, P = .113) between canal paresis (CP%) and dPTA showed no correlati

A sudden unilateral loss or impairment of vestibular function causes vertigo, dizziness, and impaired postural function. In most occasions, everyday activities supported or not by vestibular rehabilitation programs will promote a compensation and the symptoms subside. As the compensatory process requires sensory input, matching performed motor activity, both motor learning of exercises and matching to sensory input are required. If there is a simultaneous cerebellar lesion caused by the tumor or the surgery of the posterior cranial fossa, there may be a risk of a combined vestibulocerebellar l

The surgical treatment of patients suffering from incapacitating peripheral vertigo is still controversial. In the ENT Department of the Civil Hospital of Venice 43 patients with disabling menieric vertigo spells underwent a selective vestibular nerve section employing either a retrosigmoid (25 cases) or a retrolabyrinthine approach (18 cases) between 1991 and 1993. The two approaches are compared with regard to surgical technique, pre- and postoperative complications and i.e. functional results, i.e. vertigo control and hearing preservation. Functional results, classified according to the gui

OBJECTIVE: To report a case of idiopathic perilymphatic fistulas diagnosed and successfully treated during life and confirmed histopathologically post mortem.
METHODS: Perilymphatic fistulas were diagnosed clinically and repaired surgically. Light microscopic histopathologic studies were made of both temporal bones after death.
RESULTS: Vestibular symptoms had improved postoperatively. Postmortem histopathologic examination of the temporal bones demonstrated patencies of the labyrinth capsule that had been predicted clinically during life, the qualities of the membranous labyrinth in both th

A desirable animal model of acute hydrolabyrinth was made by injecting artificial endolymph into the cochlear duct of guinea pigs. In 12 animals with intact Reissner membranes, the endolymphatic potential kept normal after injection of artificial endolymph, but their ECochG showed a rise of SP amplitude, a decrease in AP amplitude, an increase of SP/AP ratio, and a delay of N1 latency. In 7 animals with ruptured membranes, the EP significantly decreased. ECochG showed a decrease in hearing in all test frequencies, a distortion of SP-AP wave or even a loss of AP. Based on the results of our exp

Ménière's disease is generally accepted to be a consequence of distention of the endolymphatic sac of the inner ear. Although the exact etiology is unclear, there is a body of research suggesting that Ménière's disease is of psychosomatic origin. While we do not intend to review the literature exhaustively, we briefly review several frequently cited studies. Even though this literature is so severely flawed that no solid conclusions may be drawn from it, certain questions about the psychological aspect of this disorder continue to surface. Our limited access to patients with vertigo has not al

An inner ear etiology for drop attacks is easily overlooked and seldom mentioned in the literature. Cardiovascular pathology is what generally is looked for, e.g. transitory ischemic attacks. Twenty-nine patients with frequent and severe Meniere attacks were treated with gentamicin intratympanally, when they had reached a stage of incapacitation due to vertigo and dread of further attacks and all medical treatments had failed. All but one patient were suffering from more or less frequent drop attacks. The attacks of vertigo were successfully cured in all patients except 2, who had recurrence o

Through history taking and physical examination, the primary care physician should be able to distinguish between the four most common causes of recurrent vertigo. Particular caution is advised when dealing with the first attack of vertigo, because more sinister possibilities may be present, although most of the disorders that cause vertigo are benign. Categories of treatment that can be used include medical, rehabilitative or exercise-related, surgical, dietary, and unconventional (eg, acupuncture, manipulation).

Three pairs of temporal bones with otologic syphilis were studied histopathologically to detect obliteration of the endolymphatic duct and the associated hydrops. All cases showed infiltration and obliteration of the endolymphatic duct by round cells, giant cells, and fibrous tissue. These findings may be related to failure of medical treatment and sac surgery to arrest or alleviate symptoms of advanced syphilis.

Vertigo is not a disease, but a symptom. The management of vertigo requires more than the treatment of the symptom alone, as it concerns the whole physical and psychological condition of the patient. In addition to causal treatment, symptomatic treatment is needed in many cases. This specific treatment consists of vestibular exercises and drug therapy. The efficacy and use of the different drugs available are discussed.

Emphasis on treatment in Meniere's disease has recently shifted from a concentration on the vertiginous component alone to include conservation or improvement of hearing as well. This presentation tabulates the current otological literature's reports of both the medical and surgical options available in the treatment of Meniere's disease. The authors conclude that these more recent reports deemphasize the importance of the vertiginous component, while emphasizing the importance of the hearing component in evaluation of the efficacy of any therapeutic modality. Furthermore, these reports indica
