Left Main Restenosis Due to a Retained Balloon Shaft: A First Report.

BACKGROUND: Retained intracoronary devices are rare percutaneous coronary intervention complications with thrombotic and mechanical consequences. CASE SUMMARY: A 77-year-old man undergoing preoperative coronary evaluation had left main bifurcation in-stent restenosis. Optical coherence tomography (OCT) showed well-apposed stents, fibrotic restenosis, and marked recoil despite repeated balloon dilatation; excimer laser coronary atherectomy was planned. Reassessment of the OCT pullback revealed a retained balloon catheter shaft extending from the left circumflex artery to the left main ostium,

Central corneal distortion after small-incision lenticule extraction.

A 33-year-old woman with a prior history of small-incision lenticule extraction (SMILE) presented with gradual deterioration of vision in her right eye since the surgery. She had undergone bilateral SMILE for myopic correction (-7.00 diopters [D] in the right eye and -6.00 D in the left eye) 3 weeks prior. SMILE was performed with a Zeiss VisuMax femtosecond laser system, with a cap thickness of 120 µm, a cap diameter of 7.50 mm, and a lenticule diameter of 6.50 mm. At the completion of the surgery, loose epithelium was noted at the SMILE incision bilaterally, and bandage contact lenses were p

Dry eye after refractive surgery.

Photorefractive keratectomy and laser in situ keratomileusis can induce or exacerbate dry eye after surgery. This manifests as an increase in degree and frequency of symptoms, corneal findings, such as superficial punctate keratopathy, and abnormal results of dry eye tests, such as the Schirmer test and tear break-up time. The cause mainly involves decreased corneal sensation, resulting in decreased feedback to the lacrimal gland and reduced tear production. Other causes may include increased evaporation, inflammation, or toxicity of medications. Dry eye may result infrequently in impaired wou

Refractive Surgery for Special-Needs Children with High Myopia.

Refractive surgery is useful in children with high isoametropic or anisometropic myopia who are aversive to spectacles or contact lenses. The majority have a neurodevelopmental disorder and the equivalent of legal blindness before surgery. Correction of myopia < 6 Diopters (D) is achieved by photorefractive keratectomy (PRK). Those with myopia > 6 D benefit from phakic IOL implantation, clear lens extraction (CLE), or refractive lens exchange (RLE). The rate of adverse events is low.

Association of khellin and 308-nm excimer lamp in the treatment of severe alopecia areata in a child.

Alopecia areata (AA) is an autoimmune inflammatory disorder of hair, characterized by non-scarring hair loss. A 308-nm excimer lamp (EL) has been reported as one effective modality in the treatment of AA. Khellin is a furanochromone photosensitizer whose chemical structure is close to psoralens and has previously proven its efficacy in vitiligo in association with ultraviolet A. We evaluated the efficacy and safety of a combination of topical khellin and 308-nm EL in the treatment of a refractory ophiasic AA, of 1-year evolution, in a 5-year-old boy. Treatment consisted in topical application
