Recent molecular studies have identified subgroups of acute lymphoblastic leukemia (ALL) driven by genomic aberrations rather than structural chromosome rearrangements or aneuploidy. Recent years have brought the advent of immunotherapy to cancer treatment regimens, and several effective agents have been developed for B-ALL. With increasing knowledge of molecular drivers of pediatric ALL and the identification of potential targets widely expressed on leukemic blasts, targeted agents and immunotherapies have been developed for pediatric patients with R/R-ALL.

The nonprofit organization Blood Cancer United acquired the remaining supply of luveltamab tazevibulin and will continue the compassionate use program for children with a rare form of AML. The move points to the challenges of funding pediatric cancer drug development.

Acute myeloid leukemia (AML) cells exhibit aberrant metabolism defined by a shift away from oxidative phosphorylation and towards anaerobic glycolysis, favouring cell growth. Glycolytic enzymes are altered for this change to occur, including pyruvate kinase (PK), where the tetrameric and high activity M1 isoform (PKM1) is replaced with the predominantly dimeric and low activity M2 isoform (PKM2). Dimeric PKM2 produces less pyruvate and acts as a protein kinase in the nucleus, demonstrating divergent roles in both cell metabolism and as a transcriptional co-activator. In this study, the role of…

Gene rearrangements of the human MLL gene (also known as KMT2A) cause leukemia with poor prognosis by generating MLL fusion oncoproteins. MLL is an epigenetic regulator that has an evolutionarily conserved role maintaining expression of homeotic (HOX) genes during embryonic development. HOX genes and other MLL target genes such as MEIS1 block differentiation and promote proliferation in hematopoietic cells. MLL gene rearrangements generate a constitutively active version of the wild-type protein, which causes overexpression of HOX genes and other target genes, leading to leukemic transformatio…

Leukemia cutis (LC) refers to cutaneous infiltration by leukemic cells, most often by acute myeloid leukemia (AML) in adults and by lymphoblastic leukemia in children. Although uncommon, LC is a clinically significant manifestation of extramedullary disease and may precede, coincide with, or follow bone marrow involvement. Recognition of AML-associated LC is essential, as it can be diagnostically challenging due to histopathologic overlap with inflammatory dermatoses and other cutaneous hematolymphoid neoplasms. Recent cohort-based studies have systematically characterized AML involving the sk…

Relapsed or refractory acute myeloid leukemia (AML) continues to have a poor prognosis, highlighting the urgent need for novel therapeutic approaches. Immunotherapy, with its distinct mechanisms of action from chemotherapy or molecularly targeted therapies, offers a promising option for treatment-resistant cases. To date, numerous chimeric antigen receptor (CAR) T cell products targeting surface antigens such as CD33, CD123, and CLL-1 have been developed and evaluated in clinical trials. T cell receptor (TCR)-T therapies targeting intracellular antigens such as WT1 are also in development, wit…
