Cleft Lip Repair, Nasoalveolar Molding, and Primary Cleft Rhinoplasty.

Cleft lip and palate are the fourth most common congenital birth defect. Management requires multidisciplinary care owing to the complexity of these clefts on midface growth, dentition, Eustachian tube function, and lip and nasal cosmesis. Repair requires planning, but can be performed systematically to reduce variability of outcomes. The use of primary rhinoplasty at the time of cleft lip repair can improve nose symmetry and reduce nasal deformity. Use of nasoalveolar molding ranging from lip taping to the use of preoperative infant orthopedics has played an important role in improving functi…

Persistent lip enlargement: An unusual presentation of lupus erythematosus.

Macrocheilia is a challenging problem with a variety of underlying causes that are both local and systemic, and granulomatous causes underlie the majority of cases. In this study, we report on a 31-year old man who presented with a chronic lower lip enlargement and a nodular submental erythematous lesion. He was otherwise clinically healthy. Laboratory test results were within the normal limit except for a positive anti-double stranded DNA test result. A diagnosis of cutaneous lupus erythematosus was made on the basis of histopathology and direct immunofluorescence. The lesions resolved dramat…

Excision of Mucocele Using Diode Laser in Lower Lip.

Mucoceles are nonneoplastic cystic lesions of major and minor salivary glands which result from the accumulation of mucus. These lesions are most commonly seen in children. Though usually these lesions can be treated by local surgical excision, in our case, to avoid intraoperative surgical complications like bleeding and edema and to enable better healing, excision was done using a diode laser in the wavelength of 940 nm.

Trauma-Induced Giant Pyogenic Granuloma in the Upper Lip.

Pyogenic granuloma (PG) is a reactive local benign vascular lesion, where connective tissue fibrovascular proliferation occurs. The most common etiology of PG is chronic, low-level irritation. PG affects females mainly. The purpose of this paper is to report a giant pyogenic granuloma caused by an acute trauma in the upper lip of an 11-year-old boy. The initial clinical diagnosis suggested PG, which was confirmed after an excisional biopsy and a microscopic exam. Oral lesions of large proportions in children can cause functional, esthetic, and behavioral issues, and should be promptly investig…

Outcomes of cleft lip repair for internationally adopted children.

BACKGROUND: Large numbers of international children with cleft lip-cleft palate are adopted in the United States; many underwent their first operation before arrival. METHODS: The authors reviewed records of internationally adopted children with cleft lip-cleft palate treated by one surgeon over 25 years. This study focused on anatomical types, frequency/methods of repair, correction of unrepaired deformities, and secondary procedures in this country. RESULTS: Of 105 internationally adopted children with cleft lip-cleft palate, 91 percent were Asian; 75 percent had labial or labiopalatal clo…

[Labial recurrent swelling revealing median congenital upper-lip fistula].

Midline upper-lip fistulas are an extremely rare variant of congenital facial malformations. Less than 30 cases have been reported in the literature since 1970. We report the case of a 2 and a half-year-old girl presenting with a median congenital blind fistula of the upper lip, without any relation with the oral cavity. A recurrent swelling of the upper lip was the main symptom. Complete surgical excision of the cyst or of the fistulous tract must be obtained to avoid recurrence.

[EFFECTIVENESS OF CROSS-LIP FLAP WITH SIMUTANEOUS VASCULAR ANASTOMOSIS].

OBJECTIVE: To investigate the feasibility and effectiveness of a modified cross-lip flap with lip artery anatomosis so as to shorten inward time. METHODS: Between August 2010 and January 2014, 11 patients underwent cross-lip flap surgery with lip artery anatomosis to repair upper lip defects. There were 9 males and 2 females with an average age of 62 years (range, 13-70 years). The defects causes were tumors removal in 6 cases, animal bites in 2 cases, and secondary deformity after cleft lip repair surgery in 3 cases . The width and height of defect were 1.5-4.0 cm and 1.8-3.5 cm respectively…

Lip repositioning: An alternative cosmetic treatment for gummy smile.

Excessive gingival display, commonly referred to as 'gummy smile' is a major hurdle in overall personality of an individual. Gummy smile, secondary to altered passive eruption and tooth mal-positioning, can be predictably treated with Surgery and orthodontic therapy. In patients with jaw deformities, orthognathic surgery can be performed. However, this requires hospitalization and entails significant discomfort. Lip repositioning is a simple surgical procedure to treat 'gummy smile'. The procedure restricts the muscle pull of the elevator lip muscles thereby reducing the gingival display while…

Botulinum toxin to improve results in cleft lip repair.

BACKGROUND: Upper lip wounds that lie perpendicular to the relaxed skin tension lines are subjected to repetitive dynamic tension caused by the orbicularis oris muscle and are susceptible to unsatisfactory scarring. METHODS: In this double-blind, randomized, vehicle-controlled, prospective trial, 60 consecutive patients with unilateral cleft lip undergoing primary cheiloplasties between August of 2011 and June of 2012 were randomized to receive botulinum toxin type A or vehicle injections into the subjacent orbicularis oris muscle immediately after wound closure. Scars were assessed after 6 m…

Evidence-based medicine: Unilateral cleft lip and nose repair.

LEARNING OBJECTIVES: After reading this article, the participant should be able to: 1. Describe the anatomical malformations found in unilateral cleft lip deformity. 2. Discuss current methods of measuring the deformity and subsequent outcomes. 3. Discuss preoperative assessments, workup, and the use of early interventions before definitive cheiloplasty (e.g., preoperative orthopedics, lip adhesion). 4. Discuss the different techniques used for cheiloplasty and nasal repair. 5. Discuss the use of postoperative splints, taping, or molding. 6. Discuss the outcomes and evidence of cleft lip repai…

Lip abscess associated with isotretinoin treatment of acne vulgaris.

IMPORTANCE: Isotretinoin is frequently prescribed for the treatment of acne vulgaris. Among the numerous documented adverse effects, most common are xerostomia and cheilitis. Lip abscesses as a consequence of cheilitis present dramatically and may pose a diagnostic challenge. OBSERVATIONS: We present a case of a 15-year-old boy with a severe lip abscess requiring incision and drainage and hospital admission for intravenous antibiotic treatment of methicillin-resistant Staphylococcus aureus. We discuss the pathophysiologic characteristics of isotretinoin therapy and the likely causative role t…

Lower lip pits: van der woude or kabuki syndrome?

Kabuki syndrome (KS) is a multiple congenital anomaly/mental retardation syndrome with characteristic facial features. Despite more than 350 documented cases and recent correlation of MLL2 mutations as a genetic cause, its full clinical spectrum is still being defined. This report describes two patients who were initially diagnosed with Van der Woude syndrome (VWS) based on the presence of lower lip pits. However, this finding can occur with KS, albeit infrequently. For patients with lower lip pits, a thorough evaluation should be made to distinguish between VWS and KS, as there are difference…

Nasoalveolar molding in complete cleft lip nasal deformity patients.

OBJECTIVES: The aim of this study was to compare nostril height, width, collumelar length, interalar distance and nostril symetry between unilateral complete cleft lip/palate patients undergoing nasoalveolar moulding (NAM) and incomplete cleft lip patients/palate with no need of presurgical moulding before and after cheiloplasty with primary nasal correction. Our study group included 18 patients from whom 9 were NAM cleft lip/palate nasal deformity cases and 9 incomplete cleft lip/palate cases. All measurements were obtained by two independent investigators and averaged, standard deviations an…
