INTRODUCTION: Information about biologically active adrenomedullin (mature AM), a potential new biomarker for sepsis and septic shock, is limited. Here, we investigated the value of mature AM for diagnosis and outcome prediction in sepsis.
MATERIAL AND METHODS: Patients admitted to the intensive care unit (ICU) were retrospectively cate-gorised into non-sepsis or sepsis groups, according to the Sepsis-3 definitions. Plasma levels of mature and total (the sum of the levels of intermediate and mature forms) AM were measured, and their usefulness was compared with that of other sepsis biomarkers

One hundred twenty mice were fed control and magnesium-(Mg) deficient diets containing 960 and 50 mg Mg/kg, respectively. After 12 days, mice were inoculated with several strains of Plasmodium (P). Parasitemias and survivals were monitored for 20 days after infection. The Mg-deficient diet protected mice against the nonlethal parasite P. chabaudi as shown by decreased parasitemia. All control mice infected with P. vinckei died from the infection. Mg-deficient mice had a much lower parasitemia and recovered from the infection. In mice infected with the lethal strain P. berghei, the parasitemia …

The relationship between experimental magnesium deficiency and blood pressure is complex and still the subject of much debate. The effect of Mg deficiency and blood pressure in Wistar rats receiving a Mg deficient diet (0.080 g/kg) for 40 weeks was examined. Deficient rats, when compared to controls, showed an initial transitory phase of hypotension, followed by normalization of blood pressure and then hypertension beginning after 15 weeks on the deficient diet. During the whole experimental period, heart rate was significantly increased in deficient rats as compared to controls. The fact that…

Magnesium deficit may be considered as a cardiovascular risk because of its aetiopathogenic role in the genesis of atherogenous dyslipidaemias and the so-called "idiopathic" mitral valve prolapse. It does not, however, constitute a major antihypertensive factor, though it may sometimes be an accessory co-factor. Plasma magnesium is generally normal in untreated hypertensive patients and normotension is the rule during magnesium deficit. An inverse relationship between magnesium and renin in the plasma of hypertensives has not been confirmed. In practice, plasma magnesium seems to be related to…

We report a case of magnesium deficiency associated with acetaminophen abuse. Metabolic studies showed that the patient's fractional excretion of magnesium was 12% when her serum magnesium level was 1.0 mg/dL. Renal biopsy demonstrated severe tubulointerstitial disease. The significant wasting of magnesium at a time when serum magnesium levels were depressed suggests renal magnesium wasting syndrome. This defect was temporally related to abuse of acetaminophen.

We studied the cardiotoxicity in magnesium (Mg)-deficient male rats fed 50 micrograms/g Cd for 45 consecutive days. Cd at a low concentration (0.28 ppm) in the heart induced cardiotoxic effects manifested by a decrease of the heart rate and weight and histopathological changes in the presence of Mg deficiency through Cd supplementation to a normal diet did not induce any cardiotoxic effect. Cardiac output (CO) did not increase in response to the decrease in the total peripheral resistance (TPR) in the Mg-deficient rats fed Cd, suggesting that supplementation of Cd to the Mg-deficient diet may …

The present study aimed at testing the hypothesis of decreased erythrocyte magnesium content and magnesium deficiency in essential hypertension. Atomic absorption was used to measure the erythrocyte content of total magnesium in 50-year-old otherwise healthy white males with essential hypertension (n = 12, blood pressure (mean +/- SE) 155 +/- 4/109 +/- 2 mmHg) that had never been treated and in normotensive control subjects (n = 12, blood pressure 128 +/- 2/88 +/- 1 mmHg) matched for age, sex, race, height, weight and smoking habits. The erythrocyte magnesium content was significantly increase…

Magnesium (Mg) deficiency can contribute to cardiac dysrhythmias and may predispose to ischemic heart disease. Most diuretic agents cause loss of Mg, but serum levels may be normal despite cellular depletion. We studied the clinical characteristics of hypertensive patients treated for at least 6 months with either hydrochlorothiazide (HCTZ) or a single nondiuretic drug. To evaluate Mg status in our patients, we measured the percentage retention of a parenterally administered Mg load as an accurate indicator of functionally available total body Mg. Serum lipid, blood chemistry, and serum Mg val…

A role of enzyme mediated metabolic processes is discussed. Unfavourable effect of magnesium deficiency on the functioning of various organs may lead to extensive and irreversible morphological changes of focal character. Basing on the results of several experiments and own experience, the author discusses an effect of low-magnesium diet on histological, histochemical, and microscopic lesions to the myocardium, skeletal musculature, liver, and pancreas. Magnesium deficiency predisposes to myocardial necrosis which simulates electrolyte-steroid-cardiomyopathy by necrosis (ESCN). Low-magnesium d…

In developed countries, the recommended dietary amounts of magnesium have been set at 6 mg/kg day. The magnesium requirements for optimal health in the adult population depend on mesological and constitutional conditioning factors. They may intervene at every stage of magnesium metabolism: absorption, circulation, storage and excretion. The influence of other nutrients is more significant on magnesium absorption than on urinary excretion. Among the multiple interactions it is important to emphasize the maintenance of a Ca/Mg ratio close to 2 in the intake. Magnesium deficit and stress reinforc…

Although the tocolytic effect of magnesium sulfate is well known, it has generally been used for this purpose for only brief periods. In this study, we administered intravenous magnesium sulfate tocolysis, either alone or in combination with other tocolytics, to 111 women as follows: 1) 60 (54%) received the drug for 3 or fewer days (short-term group); 2) 29 (26%) received the drug for 3-10 days (intermediate group); and 3) 22 (20%) received the drug for 10 days or longer (long-term group). Side effects (ileus and/or constipation, visual blurring, headache) were more common in the intermediate…

Two groups of patients with multifocal atrial tachycardia (MAT) were treated with the intramuscular and continuous intravenous magnesium sulphate regimens used in pre-eclampsia. Both routes of administration were successful in causing reversion to sinus rhythm but the intramuscular regimen, by attaining a higher and more sustained serum magnesium concentration, converted the arrhythmia to normal sinus rhythm in a shorter period of time (1-2 hours) than the intravenous regimen (4-8 hours). Magnesium-sparing diuretics should be used in the treatment of patients with chronic obstructive pulmonary…

In vitro studies led us to a new, coherent hypothesis for the pathogenesis of toxic shock syndrome (TSS). These studies show a hitherto unsuspected role of Mg in the pathogenesis of a serious microbial disease, and suggest one of several possible approaches to the prevention of the disorder.

Hypomagnesemia is probably the most underdiagnosed electrolyte deficiency in current medical practice. Patients with cardiovascular disease who are at greatest risk for the development of magnesium deficiency are those treated with diuretics or digitalis. Both potassium and magnesium deficiencies are associated with increased ventricular ectopy and may increase the risk of sudden unexpected death. Refractory potassium repletion can be caused by concomitant magnesium depletion, and can be corrected with magnesium supplementation. Routine serum magnesium determination is recommended whenever the…

Evidence is given that cardiovascular damage due to systemic stress is potentiated by a coexisting Mg deficit. Supplementation with Mg is not only a suitable measure to eliminate this risk factor, but can also increase resistance against stressors in experimental animals, livestock and probably in man, too. When Mg is supplied in larger amounts by the oral route, care must be paid to bioavailability and acid base status.

An investigation of 527 consecutive patients showed that 24 (4.5%) had hypomagnesaemia (plasma magnesium level, less than 0.70 mmol/L) and 26 (4.9%) had hypermagnesaemia (plasma magnesium level, greater than 1.00 mmol/L). The magnesium levels returned to normal values without specific treatment in 15 of the hypomagnesaemic patients. Magnesium levels of less than 0.60 mmol/L occurred only in patients with disorders that are known to cause magnesium deficiency. Seventeen of the hypermagnesaemic patients had plasma creatinine values in excess of 0.20 mmol/L; in the remainder, the plasma magnesium…
