Management of early breast cancer with breast conservative surgery. An egyptian experience.

BACKGROUND: Breast conservative surgery (BCS) has been an established method for treatment of early breast cancer. The volume of the procedure varies from a wide local excision to partial mastectomy and ipsilateral axillary lymphadenectomy. The nipple and areola complex sparing, depends on their proximity to the tumour. Radiotherapy, chemotherapy and or hormonal treatment play an integral role in the treatment of early breast cancer. AIM OF THE STUDY: In the present study we present our experience at National Cancer Institute, Cairo University with breast conservative surgery in treatment of

Partial breast irradiation as second conservative treatment for local breast cancer recurrence.

PURPOSE: Mastectomy is the treatment of reference for local relapse after breast cancer (BC). The aim of this study was to document the feasibility and the results of associating lumpectomy with partial breast irradiation by interstitial brachytherapy (IB) as local treatment for an isolated ipsilateral BC local recurrence (LR). METHODS AND MATERIALS: Between 1975 and 1996 at Marseille and Nice Cancer Institutes, 4026 patients received lumpectomy and radiotherapy (RT) (50-80 Gy) for a localized breast cancer of which 473 presented a LR. Among these patients, 69 (14.6%) received a second lumpec

Ultrasound-guided Interfascial Plane Blocks for Non-anesthesiologists in Breast Cancer Surgery: Functional Outcomes and Benefits.

AIM: Acute post-operative pain following modified radical mastectomy (MRM) in patients with breast cancer is challenging for anesthesiologists. This study aimed to prospectively compare the quality outcome of interfascial plane blocks performed with ultrasound guidance, and evaluate the consequences of sharing tasks with the breast surgeon. PATIENTS AND METHODS: The study involved 255 patients scheduled for unilateral MRM, who were divided into two groups: Pecs group: General anesthesia plus ultrasound-guided modified pectoral nerves blocks type I and II, including serratus and parasternal in

Coexistence of pregnancy-like and cystic hypersecretory hyperplasia with invasive lobular carcinoma.

BACKGROUND: Lobular pregnancy-like hyperplasia (PLH) merged with cystic hypersecretory hyperplasia (CHH), designated PLH/CHH, is a rare multicystic breast lesion. A previous report has described the high rate of coexistent ductal carcinoma in situ in PLH/CHH; however, a PLH/CHH study involving a larger number of cases is necessary to unravel the clinical significance of this tumor type. CASE REPORT: We describe a unique case of PLH/CHH that coexisted with multifocal lobular neoplasm. Multifocal invasive lobular carcinoma with lobular carcinoma in situ was observed to be adjacent to the PLH/CH

[Effectiveness of a modified Madde-type mastectomy in patients with breast cancer of I-II stage].

Effectiveness of mastectomy with two breast muscles preserved and total lymphdissection (Malden surgical operation) in I-II stage breast cancer patients has been studied. The control group has been operated using classical mastectomy after Golsted and its modified variant named after Peity. The degree of the disease dissemination was confirmed pathohistologicaly based on the 5-th edition of TNM system. It was established that the 5 year survival period among patients with primary tumor size getting pT1-2 doesn't depend on a volume of a surgical operation but correlates with the number of regio

Relationship between quality of life and mood in long-term survivors of breast cancer treated with mastectomy.

This study sought to compare the mood and quality of life (QOL) of breast cancer survivors with those observed in low-risk breast cancer screening patients. A group of long-term stage I-III breast cancer survivors (n = 60) was compared with low-risk breast cancer screening patients (n = 93) on measures of depression, anxiety, and QOL. Patients without a previous psychiatric history were studied. Although the groups differed in age and education, correlations performed between age, education, and the outcome measures showed no association of age and education with the outcome measures. Pituitary adenoma and bilateral male breast cancer: an unusual association.

An unusual case is presented of bilateral breast cancer in a male patient with a long history of endocrine dysfunction due to a prolactinoma. The role of abnormal endocrine function in the development of male breast cancer is reviewed. The strongest association between aberrant endocrine function and male breast cancer occurs in patients with Klinefelter's syndrome, who have an approximate 3% lifetime risk of developing breast cancer. Retrospective case-control studies indicate that both estrogen excess and androgen deficiency may be involved in male breast cancer. Clinical studies of estrogen

Guillain-Barre syndrome following autologous bone marrow transplantation.

The pathogenesis of the Guillain-Barre syndrome (GBS) is felt to involve alterations in cellular and humoral immune responses. We report the case of a 44-year-old female with stage IV breast cancer who developed GBS 2 days after receiving high dose chemotherapy with bone marrow and peripheral progenitor cell support, and granulocyte-macrophage colony stimulating factor (GM-CSF). Her clinical status stabilized with plasmapheresis. This report lends further support to hypotheses regarding cellular immune alterations in the pathogenesis of GBS.

The transverse rectus abdominis musculocutaneous flap for breast reconstruction in obese patients.

Transverse rectus abdominis musculocutaneous (TRAM) flap breast reconstruction has often been considered contra-indicated in obese women. The morphological characteristics peculiar to this population, however, make obese women ideal candidates for this procedure because the reconstructed breast must often match a large ptotic contralateral breast. About one-third of our postmastectomy patients are corpulent, middle-aged women with "Mediterranean" body structures. Thirty-four obese women underwent TRAM flap breast reconstruction from 1985 to 1988. According to the Body Mass Index, 23 women had

Male breast cancer; analysis of 58 cases in Shiraz, South of Iran.

INTRODUCTION: male breast cancer (MBC) is very rare and accounts for less than one percent of all cancers in men. Because of the low incidence of MBC it has not attracted extensive studies. The current study is the first cohort of men with breast carcinoma reported in our country to date. MATERIAL AND METHODS: We retrospectively reviewed the medical records of 58 MBC patients who had been treated at Shahid Faghihi Hospital in Shiraz, Iran, between 1990 and 2007. Data regarding general characteristics of patients including: age at time of diagnosis, family history of breast cancer, site, stage

Two-stage expander implant reconstruction: recent experience.

BACKGROUND: Two-stage tissue expander/implant breast reconstruction has become increasingly popular, but reports have varied widely in the incidence of complications associated with this approach. A large consecutive patient experience using low-height tissue expanders in immediate breast reconstruction is presented and discussed in conjunction with other recent reports using this approach following mastectomy. METHODS: A total of 172 consecutive patients underwent immediate breast reconstruction following mastectomy, with placement of a total of 246 tissue expanders. All expanders used were

Bilateral orbital metastases from breast carcinoma. A case of false pseudotumor.

OBJECTIVE: To inform clinicians of a metastatic orbital neoplasm that frequently masquerades as an orbital inflammatory syndrome and to stimulate thought regarding this neoplasm's fundamental pathobiology specific to its orbital involvement. DESIGN: Interventional case report and literature review. PARTICIPANTS: A 47-year-old woman with an acquired, bilateral, diffusely infiltrative orbital process is described. INTERVENTION: The clinical and radiographic features of the patient's orbital process are reported. The review of two prior fine-needle aspiration biopsy specimens failed to reveal

Laparoscopic transverse rectus abdominus flap delay for autogenous breast reconstruction.

Laparoscopic ligation of the deep and superficial inferior epigastric vessels was done for ten mastectomized patients who elected to have autogenous reconstruction of their breast. All these patients had at least one indication for the delay which included obesity, smoking, or requirement of a large volume of tissue for their reconstruction. The procedure did not add any morbidity or mortality to our patients and was found to be comparable to the "open" delay in preventing partial tissue loss in all but two patients. We describe the use of a minimally invasive procedure to augment the deep sup

Breast magnetic resonance imaging alters patient selection for accelerated partial breast irradiation.

OBJECTIVES: To determine whether pretreatment contrast-enhanced breast magnetic resonance imaging (MRI) alters patient selection for accelerated partial breast irradiation (APBI). MATERIALS AND METHODS: Women aged 40 years or older with unifocal invasive or intraductal carcinoma ≤2.5 cm on physical examination, mammography, and ultrasound (US) were evaluated with breast MRI before enrollment on an APBI trial using single-fraction intraoperative radiotherapy (IORT) or fractionated 3-dimensional conformal radiotherapy. Abnormal MRI findings were evaluated with US-guided or MRI-guided biopsy. R

Hypofractionated and accelerated radiotherapy with cytoprotection (HypoARC): a short, safe, and effective postoperative regimen for high-risk breast cancer patients.

PURPOSE: Although the role of radiotherapy (RT) after mastectomy in reducing the local relapse rate is well established, its impact on overall survival is strongly questioned. Up to 70% of patients will not benefit from additional RT, and a "wait and see" policy is often adopted. Establishment of short, still safe, and effective RT regimens would render adjunctive radiotherapy more appealing. We evaluated the toxicity and efficacy of a Hypofractionated and intensively Accelerated RT regimen supported with amifostine Cytoprotection (HypoARC) in a cohort of 72 high-risk breast cancer patients tr

Case Report: Synchronous double primary male breast cancer and non-small cell lung cancer: diagnostic confirmation and multidisciplinary management.

BACKGROUND: Male breast cancer (MBC) is rare, accounting for approximately 1% of all breast cancers. When MBC is diagnosed simultaneously with a second primary lung cancer, treatment planning becomes particularly complex because diagnostic certainty, technical resectability, competing treatment windows, and radiotherapy safety must all be considered. CASE PRESENTATION: We report a 72-year-old man who was synchronously diagnosed with left invasive ductal breast carcinoma and right lower lobe poorly differentiated non-small cell lung cancer (NSCLC). The breast cancer was staged as pT3N2aM0, AJC

Treatment and Outcomes of Women With Large Locally Advanced Breast Cancer.

Advances in breast cancer research have made breast cancer a treatable disease. However, there is a population of women who present with large, advanced, or sometimes neglected breast cancers who can prove difficult to treat. These women often require multiple modality treatment including chemotherapy, surgery, and radiation. The purpose of our study is to examine the treatment and outcomes on women with large, locally advanced breast cancers (LABCs). We identified 8 individuals who presented with LABCs requiring extensive treatment. Patients with inflammatory or metastatic cancer at the time
