Intraoperative stereotactic navigation for reconstruction in zygomatic-orbital trauma.

Reconstruction of the maxillofacial skeleton after traumatic injury is not always straightforward because of its three dimensional complex anatomy and aesthetic significance. In case of zygomatic-orbital injury induced by trauma resulting in enophthalmos, the patient is affected on both function and cosmesis. To prevent diplopia or conspicuous asymmetry, exact correction of the orbital symmetry is required. The aim of this case report is to demonstrate the use of an intraoperative image guided navigation system for identification of adequate reconstruction of the orbital floor and orbital symm

[Skating fun and fractures of the maxillofacial bones].

Skating is a favourite sport in the Netherlands. Injury data on skating were collected from the emergency departments of 6 hospitals in the province of Friesland in the Netherlands during the Eleven Cities ice skating marathon over 200 kilometers in January 1997, with 16,688 participants. Among a total of 55 fractures, the maxillofacial skeleton had a relative high fracture frequency (n = 7; 13%), especially the zygomatic complex (n = 6); one patient had a mandibular fracture. An innocent-looking black eye, unilateral numbness of the upper lip and malocclusion following a skating accident meri

Intracranial neurological injuries associated with orbital fracture.

We attempted to define the central nervous system (CNS) concomitants of various types of orbital fractures in children by reviewing the records of 95 inpatients with admission diagnoses including orbital fracture who presented to the Children's National Medical Center from 1987 through 1994. Patients were divided into three age groups: group I: 0-5 years; group II: 6-12 years; group III: older than 12 years. Orbital fractures were classified by location: roof alone (A); orbital roof plus another orbital wall (B), and orbital fractures sparing the roof (C). Mechanisms of injury included falls (

Does a Submental Airway Compared to a Tracheostomy Reduce Length of Stay in Craniomaxillofacial Trauma?

BACKGROUND: Patients with craniomaxillofacial trauma (CMT) are at increased risk of airway compromise and may necessitate airway stabilization via a tracheostomy (TO) or cricothyroidotomy. The submental airway (SMA) is an alternative airway and can avoid complications related to TO. PURPOSE: The purpose of this study was to compare the SMA to the TO with respect to hospital length of stay (LOS) and number of days with an advanced airway, termed airway days (ADs). STUDY DESIGN, SETTING, SAMPLE: This single-center retrospective cohort study included subjects with isolated CMT who required oper

Facial Trauma in Sports.

Injuries resulting from facial trauma are common in all sports. Athlete-to-athlete contact, falls, and blows from equipment account for the majority of these events. Appropriate knowledge of basic science, relevant anatomy, and clinical skills is required to provide the correct medical care. While true medical emergencies are infrequent, a prompt accurate diagnosis is essential in developing targeted management and return to play options.

Management of a severe cranio-orbito-facial trauma with a dislocation of the globe into the maxillary sinus.

We report two cases of severe cranio-orbito-facial trauma associated with a dislocation of the globe into the maxillary sinus. Simultaneous surgical repositioning the globe in the orbit and reconstruction of the orbital floor defect with an autogenous bone was performed in the first case. Postoperatively, the globe was in a normal position with a moderately reduced motility. The patient's visual acuity was reduced to light perception while indirect ophthalmoscopy was normal. In the second case, no surgery could be attempted because the patient never regained consciousness and died 7 days after

[New type of combined surgical treatment of frontobasal injuries (author's transl)].

The surgical treatment of frontobasal injuries by a new, combined method has been reported. The frontobasal injury was exposed by bifrontal craniotomy. After the intradural examination and the dressing of the brain injuries the dural defect was also extradurally searched and dressed. Thereafter--taking the advantage of the bifrontal exploration--the paranasal sinus (frontal sinus, ethmoideal system) was cleaned, then the base of the anterior scala was covered with pedicled galea flap or periosteum plate. In case of the treatment of the open injuries, following the removal of the bone fragments

Fracture Injury Risk of the Restrained Mandible to Anterior-Posterior Blunt Impacts.

This study describes the results of anterior-posterior impacts conducted on the mandibles of 22 male postmortem human subjects (PMHSs). The objective of this study was to develop an injury criterion for the mandible based on blunt impact while the jaw was restrained. Previous studies have attempted to characterize the injury risk of blunt impact to the mandible; however, due to the translation of the mandible during impact and a limited number of fractured specimens, previous studies were not able to produce an injury criterion. Blunt impact to a restrained mandible is relevant to a wide array

Transcatheter Arterial Embolization in the Treatment of Maxillofacial Fractures With Life-Threatening Hemorrhage.

INTRODUCTION: Transcatheter arterial embolization (TAE) has gained importance in the management of maxillofacial fractures with life-threating hemorrhage (MFH). However, clinical evidence supporting the use of TAE has not been clearly established in the literature. Therefore, we evaluated the effectiveness of TAE for MFH, based on data obtained from the Japan Trauma Data Bank. METHODS: Patients were identified from Japan Trauma Data Bank entries for the years 2004 to 2014. Inclusion criteria for MFH were defined using the Abbreviated Injury Scale code (Maxilla fracture, LeFort III; blood loss

Traumatic carotid-cavernous sinus fistula.

Traumatic carotid-cavernous sinus fistula is an uncommon situation resulting from severe trauma, with less than 40 cases having been reported. It is usually associated with a skull base, frontal or mid-facial fracture, but it may also be a spontaneous phenomenon of congenital, infective or degenerative origin. The blood shunts from the internal carotid to the cavernous sinus resulting in pulsating exophthalmos, orbital headache, pain, orbital or frontal bruit, loss of visual acuity, diplopia, ophthalmoplegia and the differential diagnosis should include superior orbital fissure syndrome, orbit

Penetrating maxillofacial trauma with transmaxillary migration of a metallic foreign body to the sphenoid sinus: A rare case.

This case report details a rare and potentially life-threatening injury of an initially overlooked maxillofacial trauma sustained during agricultural activity. A 19-year-old male presented to the emergency department with a headache and fatigue, 2 days following a right malar injury that had been previously sutured without advanced evaluation. A thorough clinical examination was conducted, which revealed the presence of periorbital swelling on both sides, accompanied by a right subconjunctival hemorrhage and chemosis. Radiographic imaging revealed the presence of a substantial, crescent-shaped

Antibiotic use in facial plastic surgery.

PURPOSE OF REVIEW: The aim of this study was to review and discuss recent literature regarding perioperative use of antibiotics in the context of facial plastic surgery. RECENT FINDINGS: Despite research efforts, there continues to be insufficient evidence in support of, or against, the use of antimicrobial prophylaxis. SUMMARY: Current available evidence regarding antibiotic use in facial plastic surgery procedures fails to demonstrate routine benefit. Therefore, the advantages accompanying administration of preoperative antibiotics should be weighed against any potential complications on a

Management of facial trauma in patients older than 75 years.

According to the literature and World Health Organization, elderly people are specified as persons older than 65 years of age. The authors believe that the indication for surgery patients over 65 is based on the restoration of functionality and the aesthetics. The behavior of the surgeon has to change with patients aged older than 75 years where the restoration of function and, therefore, a good quality of life is far more important than the aesthetic restoration. In a retrospective study of 1022 patients consecutively hospitalized at Maxillofacial department in Roma, Università "Sapienza," th

Management of epistaxis in the oral and maxillofacial surgery setting: An update on current practice.

Epistaxis is a common and, in most cases, benign event. Although most nosebleeds resolve spontaneously, some may be profuse and life-threatening. Severe or recurrent epistaxis can be a challenging management problem. In otolaryngologic practice, it is the most commonly seen emergency. Oral and maxillofacial surgeons will also encounter this clinical problem in varied settings. It is our aim to present an update on the contemporary management of epistaxis in maxillofacial practice. The etiology and relevant surgical anatomy are discussed. This is followed by an update on current treatment regim

[The use of a polyglycolic acid membrane in the reconstruction of the orbital floor and in loss of bone substance in the maxillofacial region].

The use of a new alloplastic biodegradable implant called BIOFIX made of polyglycolate is described. We used it to cover bone defects in maxillo-facial surgery, principally for orbital floor restoration and frontal sinus repair, with very good results. It is an excellent material of temporary support and guidance during the healing period and new bone formation. However, it will be necessary to wait for the results of a multicentric european study to confirm its superiority in maxillofacial surgery.
