Artificial intelligence classification of pediatric middle ear effusion using consumer-grade otoscopes.

INTRODUCTION: Early and accurate diagnosis of middle ear effusion in primary care settings enables timely intervention, improving long-term patient outcomes while preventing overtreatment with antibiotics. Diagnosis requires assessment of tympanic membrane mobility, but this is hampered by access to and inexperience with pneumatic otoscopy or tympanometry. This research aims to train an artificial intelligence algorithm, using tympanic membrane images captured with consumer-grade otoscopes, to accurately predict the presence or absence of pediatric middle ear effusions. METHODS: Using an over

Randomized clinical trial of post-operative outcomes following posterior versus anterior tympanostomy tube placement: preliminary results at 2-12 week follow-up.

INTRODUCTION: Tympanostomy tubes (TTs) are a common otolaryngologic procedure. Historically, tubes placed in the anterior inferior (AI) quadrant were thought to last longer, reduce ossicular damage, and minimize hearing loss. However, perforation risk remains at 2-16 %. AI perforations may require more complicated repair compared to PI perforations. This study aims at comparing early hearing outcomes between AI and PI TT placement after 3 months. METHODS: Randomized controlled trial was conducted at a tertiary pediatric hospital. Children (6 months-14 years) undergoing initial TT placement we

Association between congenital nasolacrimal duct obstruction and otitis media.

BACKGROUND: Anatomic abnormalities causing congenital nasolacrimal duct obstruction (cNLDO) may be coincident with abnormalities predisposing to otitis media (OM). We evaluated the association between cNLDO and OM, considering surgical intervention as a marker of more severe disease. METHODS: We performed a retrospective cross-sectional study of children <5 years of age who received care from both a pediatrician and ophthalmologist at Children's Hospital of Philadelphia. Primary outcomes were associations between diagnoses of cNLDO by ophthalmologists and OM by pediatricians or otolaryngologi

Effect of Tympanostomy Tube Removal Technique on Surgical Success and Operative Time.

OBJECTIVE: Tympanostomy tube removal is a common procedure, the most common complication of which is persistent perforation. Multiple surgical techniques exist. This study seeks to determine whether the technique used affects success rate or surgical time. METHODS: This was a retrospective cohort study in a tertiary-care children's hospital. Tube removal procedure codes were used to extract charts from 2008 to 2023. Data were collected on surgical technique, success, operative time, indications, and patient factors. Primary outcome measure was surgical success, defined as no residual perforat

Epidemiology of Pediatric Tympanostomy Tube Placement in the United States.

The prevalence of pediatric tympanostomy tube placement (TTP) in the United States has not been reassessed in the past decade. To assess the prevalence of TTP and frequent ear infections (FEI), the National Health Interview Survey for the calendar year 2014 was used. Among 73.1 million children, 6.26 million (8.6%) had TTP. The incidence of FEI was 3.49 million (4.8%). Males (9.6%) were more likely than females (7.5%) to undergo TTP (P = .004). Among children under 2 years of age, 9.1% reported FEI, compared to 3.9% of children aged 3 to 17 years. Among children under 2 years of age, 25% with

Rate and risk factors for post-adenotonsillectomy complications in children under 24 months and 24 to 36 months old.

PURPOSE: The primary purpose of this study was to assess the overall rate of postoperative complications after adenotonsillectomy in children under 24 months old relative to children 24-36 months old. Our secondary goal focused on quantifying specific preoperative risk factors that predispose children to postoperative complications. METHODS: We retrospectively reviewed 248 patients who underwent adenotonsillectomy at our ENT office from 2006 to 2011. We stratified these patients into two groups: under 2 years old; and 2-3 years old. We identified 42 preoperative risk factors and 22 postoperat

Effect of Tympanostomy Tube Placement on Intraoperative Auditory Brainstem Response.

BACKGROUND: Intraoperative auditory brainstem response (ioABR) testing under general anesthesia is commonly performed on children when sleep-deprived ABR and behavioral testing are not reliable or feasible. Several studies have reported potential confounding results when tube insertion is combined with ABR testing. PURPOSE: This article evaluates whether a temporary threshold shift (TTS) occurs following placement of tympanostomy tubes (TTs) in children who undergo ioABR testing. RESEARCH DESIGN: A case-control prospective study. Patients scheduled for combined TT and ioABR procedures were e

Randomized controlled trial of juzen-taiho-to in children with recurrent acute otitis media.

OBJECTIVE: Recurrent acute otitis media (AOM) in young children is rapidly increasing worldwide. Repeated antibiotic use leads to antibiotic-resistant pathogen development. Complementary and alternative medicine approaches have been suggested as a supplemental treatment option to conventional antimicrobial medicine. This randomized, parallel-group, open-label, non-herbal medicine controlled trial assessed the efficacy of a traditional Japanese herbal medicine, juzen-taiho-to (JTT) for AOM prevention in otitis-prone children. METHODS: Children prone to recurrent AOM aged 6-48 months were recru

Parent-reported otorrhea in children with tympanostomy tubes: incidence and predictors.

PURPOSE: Although common in children with tympanostomy tubes, the current incidence of tympanostomy tube otorrhea (TTO) is uncertain. TTO is generally a sign of otitis media, when middle ear fluid drains through the tube. Predictors for otitis media are therefore suggested to have predictive value for the occurrence of TTO. OBJECTIVE: To determine the incidence of TTO and its predictors. METHODS: We performed a cohort study, using a parental web-based questionnaire to retrospectively collect data on TTO episodes and its potential predictors from children younger than 10 years of age with tym
