Ambulatory blood pressure monitoring: practical considerations.

Ambulatory blood pressure monitoring (ABPM) allows one to evaluate the blood pressure (BP) profile over a 24-hour period in the patient's natural environment. Casual pressure measurements in the physician's office can be affected by alarm reactions, thus causing "white coat" hypertension. ABPM allows one to evaluate these reactions and determine the average pressure and variability of BP along with the effects of physical activity and emotional arousal on BP patterns while at work, at home, and during sleep. Average pressures determined by ABPM are more predictive of target organ involvement a

Chorionic gonadotropin.

One of the most exciting and useful developments in modern oncology has been the recognition that certain biochemical tags can be used as cell markers in diagnosis, in monitoring the effects of anti-tumor therapy, and in follow-up to detect tumor recurrence. The measurement of chorionic gonadotropin (CG) in women with choriocarcinoma and/or gestational trophoblastic disease was the first test that could be relied upon entirely for this purpose. Furthermore, the amount of functional neoplastic tissue could be approximated by determining the production rate of this hormone which in normal physio

Cardiopulmonary monitoring in low-resource settings.

PURPOSE OF REVIEW: To discuss cardiopulmonary monitoring strategies during resuscitation of shock in resource-limited settings. RECENT FINDINGS: Despite substantial advances in critical care over recent decades, major disparities in access to cardiopulmonary monitoring persist worldwide. In resource-limited settings, shock management must rely on the optimal use of basic monitoring, including clinical examination and low-cost physiological surrogates of cardiac output and tissue oxygenation. However, each of these tools has important limitations and no single variable accurately predicts flui

Technique, indications, and clinical use of ambulatory 24-hour gastric pH monitoring in a surgical practice.

BACKGROUND: Prolonged pH monitoring is used increasingly to assess alterations of gastric luminal pH caused by gastroduodenal secretory and motor abnormalities. The clinical value of gastric pH monitoring, however, has been debated. METHODS: We obtained normal values for 24-hour ambulatory gastric pH monitoring in 50 healthy volunteers and evaluated its clinical use by monitoring pH in 285 consecutive patients with nonspecific foregut symptoms and duodenal ulcers (n = 33), gastric ulcers (n = 21), antral gastritis (n = 123), or no mucosal injury (n = 108). RESULTS: Patients with duodenal ulc

Heart Rate Variability Responses to Visually Induced Motion Sickness.

Analysis of heart rate variability (HRV) can reveal a range of useful information regarding the dynamics of the autonomic nervous system (ANS). It is considered a robust and reliable tool to understand even some subtle changes in ANS activity. Here, we study the "hidden" characteristic changes in HRV during visually induced motion sickness; using nonlinear analytical methods, supplemented by conventional time- and frequency-domain analyses. We computed HRV from electrocardiograms (ECG) of 14 healthy participants measured at baseline and during nausea. Primarily hypothesizing evident difference

Adrenal insufficiency in pregnancy: Physiology, diagnosis, management and areas for future research.

Adrenal insufficiency requires prompt diagnosis in pregnancy, as untreated, it can lead to serious consequences such as adrenal crisis, intrauterine growth restriction and even foetal demise. Similarities between symptoms of adrenal insufficiency and those of normal pregnancy can complicate diagnosis. Previously diagnosed adrenal insufficiency needs monitoring and, often, adjustment of adrenal hormone replacement. Many physiological changes occur to the hypothalamic-pituitary-adrenal (HPA) axis during pregnancy, often making diagnosis and management of adrenal insufficiency challenging. Pregna

GASTROESOPHAGEAL SYMPTOMS AFTER LAPAROSCOPIC GASTRIC BYPASS: MISTAKES IN PERFORMING THE PROCEDURE?

AIM: Laparoscopic Roux-en-Y gastric bypass (LGB) is the recommended procedure for morbidly obese patients with gastroesophageal reflux disease (GERD). However, there have been reported gastroesophageal reflux symptoms or esophagitis after LGB. Few functional esophageal studies have been reported to date. To evaluate the anatomic and physiologic factors contributing to the appearance of these problems in patients who underwent LGB. METHODS: This prospective study included 38 patients with postoperative gastroesophageal reflux symptoms submitted to LGB. They were subjected to clinical, endoscop

Orthostetrics: Management of Orthopedic Conditions in the Pregnant Patient.

Managing orthopedic conditions in pregnant patients leads to challenges that must be carefully considered so that the safety of both the mother and the fetus is maintained. Both perioperative and intraoperative considerations must be made based on physiologic changes during pregnancy, risks of radiation, and recommendations for monitoring. Operative timing, imaging, and medication selection are also factors that may vary based on trimester and clinical scenario. Pregnancy introduces unique parameters that can result in undesirable outcomes for both mother and fetus if not handled appropriately

A comparison of oral and intravenous diazepam sedation for periodontal surgery.

Intravenous and oral diazepam were evaluated as to their effectiveness in conscious sedation during two similar surgical episodes. Ten patients, six females and four males, from 30 to 60 years of age were included in the study. Patients received either 10 mg oral diazepam and saline intravenous injection or oral placebo and 10 mg intravenous (IV) diazepam at each trial. Half the patients received the oral diazepam first and the other half received the IV diazepam first. Patients were not informed as to the route of administration during each trial. Physiologic stress was measured by monitoring

Cardiovascular reserve as an integrative physiological construct: Current evidence, emerging technologies and future directions.

Cardiovascular reserve represents the integrated physiological capacity of the cardiovascular system to maintain tissue perfusion during increasing physiological stress. Although reserve has long been recognized conceptually, its direct assessment remains challenging. No universally accepted definition or gold-standard measure of cardiovascular reserve currently exists. Nevertheless, if cardiovascular reserve can be estimated reliably, reserve-based monitoring could permit earlier recognition of impending circulatory deterioration before conventional physiological indicators become abnormal. Anesthetic Management of a Pediatric Patient With Glucose-6-Phosphate Dehydrogenase Deficiency Undergoing Emergency Rigid Esophagoscopy: A Case Report.

A 23-month-old, 8-kg boy with known glucose-6-phosphate dehydrogenase (G6PD) deficiency presented with a coin lodged in the upper esophagus and required urgent rigid esophagoscopy under general anesthesia. Anesthetic management was planned to avoid drugs with oxidative potential and to minimize physiologic stressors that could contribute to hemolysis or methemoglobinemia. General anesthesia was performed using agents considered safe in patients with G6PD deficiency, with intraoperative management focused on maintaining normoxia using the lowest possible inspired oxygen concentration needed to

Analysis of physiological response to two virtual environments: driving and flying simulation.

As virtual reality technology continues to attract significant attention in clinical psychology, especially in the treatment of phobias, physiological monitoring is increasingly considered as an objective measurement tool for studying participants. There are few studies, however, of the normal physiological response to virtual environments or reactions to different virtual environments. The goal of this study is to analyze nonphobic participants' physiological reactions to two virtual environments: driving and flying. Eleven nonphobic participants were exposed to each virtual environment for 1

Marked Asymptomatic Creatine Kinase Elevation With Preserved Renal Function Identified Through Protocol-Driven Monitoring in a Clinical Trial Participant: A Case Report.

Elevated creatine kinase (CK) levels may occur in a wide range of clinical conditions including skeletal muscle injury, inflammatory myopathies, medication-associated myotoxicity, rhabdomyolysis, and strenuous physical activity. Distinguishing clinically significant muscle injury from transient physiologic hyperCKemia may be particularly challenging in asymptomatic patients undergoing routine laboratory surveillance. We report the case of a 62-year-old African American male clinical trial participant in whom marked asymptomatic hyperCKemia was identified through protocol-driven safety monitori

Relationship between intracranial pressure and phase contrast cine MRI derived measures of intracranial pulsations in idiopathic normal pressure hydrocephalus.

Phase contrast cine MRI with determination of pulsatile aqueductal cerebrospinal fluid (CSF) stroke volume and flow velocity has been suggested to assess intracranial pulsations in idiopathic normal pressure hydrocephalus (iNPH). We aimed to compare this non-invasive measure of pulsations to intracranial pressure (ICP) pulse wave amplitude from continuous ICP monitoring. We hypothesised that a significant correlation between these two markers of intracranial pulsations exists. Fifteen patients with suspected iNPH had continuous computerised ICP monitoring with calculation of mean ICP pulse wav

Synthetic molecular communication through microfluidic oscillating droplets for intrabody physiological data transmission.

We explore the capabilities of a microfluidic-based synthetic molecular communication (SMC) system for the transmission of physiological data within the human body. The system employs oscillating water droplets as a means of transmitting information through pressure variations. The validity of this approach for binary communications is validated through a combination of simulations and experiments. A case study focused on monitoring gastroesophageal reflux disease (GERD) has been considered. The prototype platform demonstrated the capacity to transmit both synthetic raw esophageal pH values an

Leveraging biometric data for meaningful outcome assessments in the care of patients with brain tumors: a systematic review.

PURPOSE: Wearable technologies are increasingly utilized in oncology to provide continuous, noninvasive monitoring of physiological and functional metrics. While feasibility has been demonstrated in several cancer populations, their role in neuro-oncology, particularly among brain tumor patients, remains underexplored. METHODS: We conducted a comprehensive literature search of PubMed, Scopus, and IEEE Xplore from 2009 to 2024. Extracted data included device characteristics, feasibility and adherence, physiologic monitoring, functional status, treatment monitoring, quality of life, and palliati

Diagnostic value of stand-alone videos after inconclusive inpatient EEG-video monitoring.

Inpatient electroencephalographic (EEG)-video monitoring occasionally fails to capture the events in question, leaving patients without a definitive diagnosis. Management after an inconclusive admission is generally limited to repeating monitoring or empirical adjustment of antiseizure medications. Although stand-alone videos are increasingly used in clinical practice, their diagnostic value in this setting has not been well established. We aimed to assess the diagnostic contribution, submission rate, and interrater reliability of stand-alone videos obtained after inconclusive inpatient EEG-vi
