Effectiveness of wraparound versus case management for children and adolescents: results of a randomized study.

In this study, we compared service experiences and outcomes for youths with serious emotional disorder (SED) randomly assigned to care coordination via a defined wraparound process (n = 47) versus more traditional intensive case management (ICM; n = 46) The wraparound group received more mean hours of care management and services; however, there ultimately were no group differences in restrictiveness of residential placement, emotional and behavioral symptoms, or functioning. Wraparound implementation fidelity was found to be poor. Organizational culture and climate, and worker morale, were po…

Ensuring palliative care to critically ill patients is also vital to support the ICU staff.

Although intensive Care Unit (ICU) personnel share a moral imperative to provide palliative care, their compassionate intentions are often undermined by communication constraints, structurally inadequate systems and care environments, for example electronic health record demands superseding time spent with a dying patient, or family separation at the time of death. Moral distress and burnout among ICU healthcare professionals are a growing global concern, exacerbated by the complex demands of critical care settings. High patient acuity, emotional strain, ethical dilemmas, and exposure to death

The Ethics of Bodyoid Use.

This paper argues that there may not be a decisive moral reason to prohibit the use of bodyoids for research, clinical education, or organ procurement, provided that such entities can be ethically and feasibly created. The first part of the paper argues that, even if one grants that bodyoids have moral status, that fact alone may be insufficient to show that their use is impermissible, because bodyoids are plausibly not the kinds of beings that can be harmed in a morally relevant sense. The second part argues that, even if the use of bodyoids could have deleterious effects on the character of

Legal harmonization and reproductive tourism in Europe.

Legislation of ethical issues illustrates the uneasy mix of ethics and politics. Although the majority has the political right to express its moral views in the law, a number of important ethical values like autonomy, tolerance and respect for other people's opinions urge the majority to take the minorities' position into account. Ignoring pluralism in society will inevitably lead to reproductive tourism. Although European legislation and harmonization in the domain of medically assisted reproduction is presented as a partial solution to this phenomenon, it is argued that European legislation

Pediatric Mental Health Integration and Perceived Quality of Care.

OBJECTIVE: This study examined how access to behavioral and mental health (B/MH) professionals varied across general and subspecialty pediatricians and whether access to, and degree of, B/MH integration in clinical settings was associated with pediatricians' perceptions of health care quality and moral distress. METHODS: A voluntary survey was administered to pediatricians during enrollment in the 2022 American Board of Pediatrics Maintenance of Certification program. The survey assessed access to, and level of, integration with B/MH professionals in their clinical practice, perceived quality

Pathogen disgust is associated with interpersonal bias among healthcare professionals.

BACKGROUND AND OBJECTIVES: Pathogen avoidance is a fundamental motive that shapes many aspects of human behavior including bias against groups stereotypically linked to disease (e.g. immigrants, outgroup members). This link has only been examined in convenience samples and it is unknown how pathogen avoidance processes operate in populations experiencing prolonged and heightened pathogen threat such as healthcare professionals. We examined whether healthcare professionals demonstrate the same link between pathogen disgust and intergroup bias as has been documented among the general population.

The Healer's Heartache: Reforming Medicine from the Rough Ground.

Current and future clinicians are experiencing a malaise that is at once deeply felt yet difficult to articulate. While "burnout" and "moral injury" have come to dominate many accounts, these terms do not fully capture the experience of clinicians who struggle to name a sense of loss, lament, and love for a beloved profession. Therefore, we propose "heartache" as a more fitting description. While burnout comes from the world of industry and moral injury from the world of war, heartache comes from the world of love. Burnout depends on individuals to cultivate resilience, and moral injury encour

Bodily contrast experiences in cultivating character for care.

Since 2008, in Flanders, we organize immersion sessions in a simulated context with the aim of stimulating student nurses and health professionals to learn virtuous caring. In this contribution, we first outline the purpose of this experiential learning: the cultivation of moral character. We come to the core of what we mean by moral character for care. We refer to Joan Tronto and Stan van Hooft to claim that caring is central to all aspects of nursing practice and is the basis of its moral quality. We also stipulate that caring involves the integration of action with emotions, motivations, an

[Eduard Reich's "Hygiene of Culture"-A retrospective on the occasion of the 100th anniversary of his death].

In contrary to the developing significance of the bacteriology and the chemical and physical noxious influences of the environment for Eduard Reich the main cause of decision between health and illness are the functions of the "soul". Components of this as important for the normal life are virtue (i. e. solidarity, performance of duty, self-knowledge), good sense to realize correlations, morality, charity and mercifulness (social behaviour). The goal of the hygiene therefore is to promote the good and to hinder the evil, as well for a complete person or only one of his organs as for the whole

The Cost of Confession: Narratives of Guilt and Innocence in Youth Convicted of Sexual Offenses.

Receiving a sexual offense conviction often entails broad social rejection. The resulting pain is managed through telling stories addressing guilt and innocence, justification, and remorse. Building upon narrative interviews with seven young men convicted of sexual offenses, this article demonstrates their intricate post-conviction repair work. Our analysis yielded three types of narratives addressing guilt. In confessional narratives, the participant accepts responsibility and agrees that harm was caused. The moral self is damaged in such stories, but the route to believable desistance appear

Deontological Guilt Mediates the Effects of Personality on the Symptoms of Romantic Relationship Obsessive Compulsive Disorder (ROCD).

OBJECTIVE: Relationship obsessive-compulsive disorder (ROCD), a clinical variant of OCD, is associated with personality traits and guilt sensitivity. Previous studies have not investigated whether the guilt associated with ROCD stems from deontological or altruistic morality. The main aim of the present study was to explore the differentiated impact of deontological and altruistic guilt on ROCD symptoms in romantic relationships. The study also aimed to test the mediating role of guilt in the relationships between personality traits and ROCD symptoms. METHOD: Through linear regressions and pa

Exploitations and their complications: the necessity of identifying the multiple forms of exploitation in pharmaceutical trials.

Human subject trials of pharmaceuticals in low and middle income countries (LMICs) have been associated with the moral wrong of exploitation on two grounds. First, these trials may include a placebo control arm even when proven treatments for a condition are in use in other (usually wealthier) parts of the world. Second, the trial researchers or sponsors may fail to make a successful treatment developed through the trial available to either the trial participants or the host community following the trial. Many commentators have argued that a single form of exploitation takes place during human

Considering the Developing Entity in an Artificial Womb as a Patient.

Artificial womb (AW) prototypes are currently being developed with the aim of improving the medical care of extremely premature infants. Despite the seemingly imminent reality of partial ectogenesis (i.e., gestation partially outside a human womb), there is persisting debate about the moral status of the fetus transferred in an AW-henceforth, the "developing entity." For some, AWs are simply another neonatal intensive cares' technology. Thus, developing entities in AWs should deserve the same protections as newborns in incubators. Others consider that AWs are fundamentally different technologi

Asymmetric Power Relations in Gynaecological Consultations for Cervical Cancer Prevention: Biomedical and Gender Issues.

A generic qualitative research, using a poststructuralist feminist perspective, was conducted in a Spanish gynaecology unit with the following aims: (a) to analyse how asymmetric power relations in relation to biomedical knowledge and gender shape the medical encounters between gynaecologists and women diagnosed with cervical intraepithelial neoplasia and (b) to explore the cognitive, moral, and emotional responses expressed by patients. A total of 21 women diagnosed with cervical intraepithelial neoplasia were recruited through purposive sampling. Semi-structured interviews were recorded and

Neurodiversity and the Neuro-Neutral State.

Over the past decade, many philosophers have argued that to respect the moral equality of their citizens, states should be neutral toward certain forms of diversity among their populations. Areas in which the state neutrality has been advocated include, but are not limited to, citizens' different religions; languages; and sexual orientations. However, there remains an important area where its normative (ir)relevance has not been discussed: That of neurodiversity. After identifying several ways in which contemporary states disfavor the interests of neurodivergent groups relative to the neurotyp

Integrating family therapy into exposure-based CBT for a Spanish patient with obsessive scrupulosity.

Obsessive-compulsive symptom profiles vary widely among individuals and may be affected by cultural backgrounds. In the case of scrupulosity, moral and religious principles are the target of obsessive-compulsive symptoms. Cognitive biases and beliefs have special relevance in the origin and maintenance of obsessive scrupulosity. In addition, rigid and exaggerated beliefs about morality are held by these patients. Moral and religious principles are mainly transmitted by family. These influences may be more prominent in cultures, such as the Spanish culture, where family and religion are importa

Teaching ethics in an unethical setting: "doing nothing" is neither good nor right.

Does it make sense to teach ethics in an unethical setting? Should teachers who work in morally compromised institutions make an effort to introduce biomedical ethics to the curriculum? Using the medical establishment in contemporary India as a window to understanding the challenge of teaching ethics in an unethical setting, this article attempts to discuss issues pertaining to ethics education in institutions with a weak ethical climate. Putting ethics into practice is the essence of ethics education and in this the integrity of the teacher and the moral environment of the institution play si
