Disruption of the podosome adaptor protein TKS4 (SH3PXD2B) causes the skeletal dysplasia, eye, and cardiac abnormalities of Frank-Ter Haar Syndrome.

Frank-Ter Haar syndrome (FTHS), also known as Ter Haar syndrome, is an autosomal-recessive disorder characterized by skeletal, cardiovascular, and eye abnormalities, such as increased intraocular pressure, prominent eyes, and hypertelorism. We have conducted homozygosity mapping on patients representing 12 FTHS families. A locus on chromosome 5q35.1 was identified for which patients from nine families shared homozygosity. For one family, a homozygous deletion mapped exactly to the smallest region of overlapping homozygosity, which contains a single gene, SH3PXD2B. This gene encodes the TKS4 pr

Neurocutaneous disorders identified in the neonatal period and infancy: Hypomelanosis of Ito.

Hypomelanosis of Ito is a rare neurocutaneous disorder named after the Japanese dermatologist Minoru Ito who in 1952 described the condition. Characteristically, the hypopigmented lesions in this condition follow Blaschko lines. Extracutaneous manifestations of the disease include neurological, musculoskeletal, cardiac, endocrine, and renal signs and symptoms. The most common neurological manifestation is cognitive impairment. Seizures, developmental delay, and abnormal tone can also be present. In this review we discuss the condition's clinical presentations, its diagnostic criteria, and cons

Attenuated mucopolysaccharidosis type VI (Maroteaux-Lamy syndrome) due to homozygosity for the p.Y210C mutation in the ARSB gene.

We describe the case of a boy recently diagnosed with an attenuated form of mucopolysacchararidosis VI (MPS VI, Maroteaux-Lamy syndrome). The Y210C mutation has not been described previously in the homozygous state, although this is a common ARSB mutation. His phenotype is essentially musculoskeletal. Urine screening tests based on measuring total GAG may miss this presentation as total GAGs were not elevated in the patient (although the electrophoresis pattern was clearly abnormal). In this phenotype the benefit of ERT remains to be established.

Hepatitis A infection mimicking adult onset Still's disease.

Fever, rash, and arthritis may be components of the prodrome of viral hepatitis. In the absence of jaundice and abnormal liver function tests, this form of polyarthritis is easily confused with primary autoimmune diseases. Whereas the association of systemic illness with musculoskeletal symptoms and numerous viral infections is well known, such an association with hepatitis A has only been rarely reported. We describe a case of hepatitis A infection mimicking adult onset Still's disease, and review the pathogenesis and differential diagnosis of Still's disease and the extraarticular manifestat

Neutral Mechanical Alignment and Prevalence of Constitutional Varus Knees Among the Saudi Population at King Saud University Medical City.

Background Various studies have described the restoration of the neutral mechanical alignment of the lower limb in total knee replacement (TKR) as a key factor of knee implant durability. Mechanical alignment of the knee requires that the tibial and femoral cuts are perpendicular to the mechanical axis of each bone in the coronal plane. Objectives The aim of this study is to investigate mechanical knee alignment and the prevalence of constitutional varus knee among the Saudi population at a single tertiary center with no history of musculoskeletal abnormalities. Methodology This is a retrospec…

Abnormal epiphyseal development in a feline model of Sandhoff disease.

Sandhoff disease (SD) is caused by decreased function of the enzyme β-N-acetylhexosaminidase, resulting in accumulation of GM2 ganglioside in tissues. Neural tissue is primarily affected and individuals with the infantile form of the disease generally do not survive beyond 4 years of age. Current treatments address neurometabolic deficits to improve lifespan, however, this extended lifespan allows clinical disease to become manifest in other tissues, including the musculoskeletal system. The impact of SD on bone and joint tissues has yet to be fully determined. In a feline model of infantile S

[X-linked hypophosphatemia caused by a novel variant in the PHEX gene].

X-linked hypophosphatemia (XLH) (OMIM #307800) is a rare, chronic, and progressive genetic musculoskeletal disorder caused by loss-of-function variants in the PHEX gene located at locus Xp22.11. Clinical manifestations of XLH include short stature, dental abnormalities, and lower limb deformities, although the phenotype is variable. Diagnosis is clinical, biochemical, and radiological, but genetic testing is also recommended. This case report describes a 29-year-old female patient with no family history related to her disease, presenting with short stature, dental abnormalities, lower limb def

Photon-counting CT in musculoskeletal imaging: New horizon for understanding bone health in osteoarthritis.

Osteoarthritis is a heterogeneous disease affecting many joint tissues. The significant role of bone, and in particular abnormalities in the subchondral bone, in OA is increasingly understood. Bone marrow lesions are believed to play an important role in OA symptom perception and progression of OA. Consequently, there is an increasing interest in the characterization of subchondral bone and bone marrow lesions by means of imaging. Computed tomography (CT) has shown to be a suitable imaging method to visualize various bony abnormalities in OA, however, the spatial resolution of conventional CT

Anatomical reduction for focal fasciculations in peroneus brevis spondylolisthesis: a case report suggesting a mechanism of peripherally derived tremor.

CASE: A 9-year-old girl presented with visible, rhythmic fasciculations in the peroneal muscle groove of her right ankle at rest, accompanied by pain and mild movement limitation. Dynamic ultrasonography showed persistent intrasheath subluxation of the peroneus brevis tendon, with fasciculations correlating with tendon dislocation. After excluding motor neuron disease and metabolic disorders, she underwent surgery involving fibular groove deepening and retinaculum repair. At the 6-month follow-up, ultrasound demonstrated tendon stability and no recurrence of fasciculations at rest or during ex

Musculoskeletal Infection: Role of Molecular
Imaging in Diagnosis and Monitoring Treatment Response.

Molecular imaging tests frequently are performed as part of the diagnostic workup of musculoskeletal infection. Three-phase bone scintigraphy reliably diagnoses osteomyelitis in bones not affected by underlying conditions. The test is less useful, because of decreased specificity, in patients with underlying bony abnormalities or alterations such as fractures, orthopaedic hardware, arthritic changes, and tumors. At one time gallium-67 scintigraphy was used as a complement to bone scintigraphy to improve the specificity of diagnosis. With the introduction of labeled leukocytes and, more recentl

Deformity or dysfunction? Osteopathic manipulation of the idiopathic cavus foot: A clinical suggestion.

Observed gait abnormalities are often related to a variety of foot deformities such as the cavus foot, also known as pes cavus, cavovarus, uncompensated varus, and the high arched foot. When gait abnormalities related to cavus foot deformities produce symptoms or contribute to dysfunctional movement of the lower extremity, foot orthotics are commonly used to accommodate the deformity and optimize the function of the lower extremity. In more severe cases, surgical intervention is common. Hypomobility of the many joints of the foot and ankle may be mistaken as an idiopathic cavus foot deformity.

Carpenter syndrome.

Carpenter syndrome is a rare autosomal recessive disorder that belongs to a group of rare craniosynostosis syndromes (Bull Soc Med Paris 1906;23:1310). Carpenter syndrome is the rarest, with only occasional patients seen. There are 3 common features in all of these syndromes: craniosynostosis (skull base abnormalities, with early fusion in different sutures), midface hypoplasia, and musculoskeletal abnormalities. Clinical features of Carpenter syndrome include peculiar facies, asymmetry of the skull, polydactyly, brachymesophalangy, mild soft tissue syndactyly, obesity, hypogenitalism, congeni…

Metabolic abnormalities associated with skeletal myopathy in severe anorexia nervosa.

The aim of this study was to characterize the metabolic disturbance associated with the skeletal myopathy resulting from extreme weight loss in anorexia nervosa. Muscle function was examined in eight female patients with severe (40%) weight loss due to anorexia nervosa and histologically confirmed myopathy. A wide range of biochemical and hematologic investigations were carried out, including serum enzymes and the response of plasma lactate to ischemic exercise of forearm muscles. All patients showed proximal muscular weakness. A diminished lactate response to ischemic exercise was a consisten

Coccidioidomycosis in horses: 15 cases (1975-1984).

Fifteen confirmed cases of equine coccidioidomycosis that originated in California and Arizona were studied retrospectively. Age, breed, and sex varied among affected horses. The most common historical problems were chronic weight loss (53% of cases) and persistent cough (33% of cases). The most frequent physical examination abnormalities were related to the respiratory tract (60% of cases). In 27% of cases, horses had signs of musculoskeletal pain. Horses consistently had hyperproteinemia, hyperfibrinogenemia, leukocytosis, and neutrophilia. An antemortem etiologic diagnosis was made for 11 (

Congenital genetic murine (ch) hydrocephalus. A structural model of cellular dysplasia and disorganization with the molecular locus of deficient proteoglycan synthesis.

The recessively inherited ch hydrocephalic mouse has been sporadically investigated over the past 40 years as a genetic murine model for congenital hydrocephalus. Since central nervous system anomalies are commonly associated with congenital anomalies in the musculoskeletal and urogenital systems, and since congenital hydrocephalus is also associated with other developmental abnormalities, an understanding of the pathophysiology at the cellular and molecular levels of the genetic defects of this hydrocephalic murine model has application to related human disorders associated with hydrocephalus
