Rhabdomyolysis is a clinical emergency characterized by severe muscle damage, resulting in the release of intracellular muscle components, which leads to myoglobinuria and, in severe cases, acute kidney failure. Rhabdomyolysis is caused by genetic factors linked to increased disease susceptibility in response to extrinsic triggers. Recessive mutations in TANGO2 result in episodic rhabdomyolysis, metabolic crises, encephalopathy and cardiac arrhythmia. The underlying mechanism contributing to disease onset in response to specific triggers remains unclear. To address these challenges, we created…

von Hippel-Lindau (VHL) disease is an inherited syndrome manifesting with benign and malignant tumors. Deficiency of carnitine palmitoyltransferase type II (CPT2) is a disorder of lipid metabolism that, in the muscle form, manifests with recurrent attacks of myalgias often associated with myoglobinuria. Rhabdomyolytic episodes may be complicated by life-threatening events, including acute renal failure (ARF). We report on a male patient who was tested, at 10 years of age, for VHL disease because of family history of VHL. He was diagnosed with VHL but without VHL-related manifestation at the ti…

BACKGROUND: Patients with injuries that cause significant muscle death often develop rhabdomyolysis. The subsequent release and entry of myoglobin into the systemic circulation leads to myoglobinuria, renal injury, and potentially acute renal failure.
METHODS: Large (5 kg) adult rabbits (n = 8) were anesthetized and a 15-kg weight placed on the posterior compartment for 4 hours. After this time, the weight was removed and releasing incisions were made. Subatmospheric pressure (125 mm Hg) was continuously applied to the wounds of four rabbits. Systemic serum samples were obtained at the time o…

Magnetic resonance spectroscopy (MRS) is a useful tool for detecting in vivo metabolic status and in vitro analysis of chemical metabolites. We performed MRS on a 15 year-old mentally retarded male patient with recurrent myoglobinuria due to phosphoglycerate kinase (PGK) deficiency. Muscle histochemistry was normal, although electron microscopy showed a significant increase in glycogen content. Biochemical analysis using muscle specimens revealed profound reduction of PGK activity to 4.4% of the control mean. In vivo MRS using 31P revealed a significant accumulation of sugar phosphates after i…

We experienced 5 cases of acute renal failure due to rhabdomyolysis during the last two years and investigated those etiologies. Diagnosis of rhabdomyolysis was established by the detection of elevated serum creatine phosphokinase, myoglobin, aldolase, myoglobinuria as well as by the clinical course. The respective underlying illness of the 5 cases were grand mal seizures, infection (high fever), heat stroke, diabetes mellitus with hyperosmolar nonketotic coma and cerebral infarction treated by barbiturate. In this investigation, however, any single cause was not enough as the etiologies of rh…

Malignant hyperthermia developed in a 4-year-old Thoroughbred horse following 3 hours and 15 minutes of halothane anesthesia, with supplementary succinylcholine. Clinical signs included fever, sweating, hyperventilation, tachycardia, and decreased blood pressure followed by a rapid increase in blood pressure. Biochemical aberrations included hypocalcemia, hyperkalemia, hyperphosphatemia, myoglobinuria, and high creatine phosphokinase and ornithine carbamyl transferase activities. Treatment consisted initially of surface cooling with cold water, alcohol and ice, IV administration of cooled bala…

Pontocerebellar hypoplasia type 2 is an autosomal recessive disorder characterized by hypoplasia and atrophy of the cerebellum and pons, leading to microcephaly, dystonia/dyskinesia, seizures, and severe cognitive impairment. Until lately it was considered a CNS-refined disease, but recent reports have associated it with muscular defects, as well. A 5-year-old boy with genetically confirmed pontocerebellar hypoplasia type 2 is described. The patient had all the clinical and radiological features of the disease, but he, additionally, exhibited two episodes of rhabdomyolysis precipitated by resp

Rhabdomyolysis is a syndrome involving the breakdown of skeletal muscle causing myoglobin and other intracellular proteins and electrolytes to leak into the circulation. The development of rhabdomyolysis is associated with a wide variety of diseases, injuries, medications and toxins. While the exact mechanisms responsible for all the causes are not fully understood, it is clear that muscle damage can occur from direct injury or by metabolic inequalities between energy consumption and energy production. Rhabdomyolysis is diagnosed by elevations in serum creatine phosphokinase (CPK), and while t

A 21-year-old patient developed rhabdomyolysis during his nineteenth week of treatment with clozapine for drug-resistant schizophrenia. No risk factors for rhabdomyolysis were found, but the calcium-dependent potassium efflux, normally responsible for membrane hyperpolarization and muscle refractoriness, was severely decreased in the patient's red blood cells. Clozapine is speculated to cause rhabdomyolysis in patients with defective calcium-activated K+ channels.

The effect of plasma exchange (PE) was evaluated in 4 patients with rhabdomyolysis. A single 21 PE produced a transient fall of creatinine phosphokinase and did not prevent renal failure. Theoretically PE would need to be performed very frequently to remove toxins in appropriate amounts. Since renal failure in myoglobinuira has a relative good prognosis, we do not recommend intensive PE as therapy.

Rhabdomyolysis was evaluated by measurement of total creatine kinase (CK) and lactic dehydrogenase (LDH) in 19 patients with severe sepsis; 12 developed acute renal failure (Group B) and 7 did not (Group A). Results were compared to 7 patients with trauma (Group C) and 6 patients with chronic renal failure and minor infections (Group D). CK was higher (p less than 0.005) in Group B than in A. Results in Group C were similar to those in A. Elevation of CK correlated to increases in creatinine (r = 0.655, p less than 0.005). CK levels of Group D patients were lower than those of Group B. Blood p

A patient with fulminant MH is presented. The metabolic changes in muscle tissue, whole blood and plasma are compared with those obtained from the porcine model. The samples were taken 1 h after treatment (including dantrolene sodium) had been commenced. Increased concentrations of lactate, pyruvate and citrate were found in muscle, whereas the concentrations of potassium, free inorganic phosphate and glycogen were reduced. The ATP, ADP and phosphoryl creatine concentrations remained unchanged. In blood, increased concentrations of alanine, lactate and glycerol were observed. An increase in ca

The compartmental syndrome is not a rare complication of non-traumatic rhabdomyolysis. Early recognition and prompt surgical decompression of the affected area are essential. We feel that a lack of familiarity with the syndrome among nephrologists may contribute to delay in diagnosis and treatment and reduce the chances for functional neuromuscular recovery. Our experience with three cases following overdosage of alcohol (case 1), barbiturates (case 2) and a mixture of alcohol and sedatives (case 3) is reported. The pathogenesis, symptomatology and treatment of the compartmental syndrome in th

Amoxapine, a new tricyclic antidepressant, has been recently released in the United States and Europe. There has been limited experience with amoxapine overdose, and no serious reactions or sequelae were observed in the reported cases. A case of amoxapine overdose associated with rhabdomyolysis and acute renal failure is reported. Amoxapine overdose could cause convulsions or prolonged coma leading to atraumatic rhabdomyolysis and myoglobinuric renal failure. Direct muscle or renal toxicity from this drug and its metabolites should not be excluded until evidence to the contrary is available. A
