To ascertain if nail biting (usually considered a manifestation of emotional tension) was associated with fibromyalgia, 387 patients attending the Rheumatism Clinic at the Leeds General Infirmary were studied prospectively. Bitten nails appeared to be a feature of youth rather than of fibromyalgia. Patients with a full set of dentures were less likely to bite their nails than others, but it can and was done by 7%. Posing the question about nail biting worried several patients, indicating the wisdom of seeking ethical approval even for "noninvasive" studies.

Two patients suffering from pemphigus vulgaris were found to have nail dystrophies which antedated the onset of mucocutaneous lesions by many years. The nail changes improved substantially on treatment of the bullous disease. One patient had nail matrix histology consistent with pemphigus, and both had positive direct immunofluorescence with intercellular IgG in the matrix epithelium, as well as at other body sites. We propose that dystrophic nails, as a non-specific indicator of autoimmune disease, are a genuine and relevant finding in pemphigus.

An 11-year-old girl developed blisters on her fingers after wiping up spilled nail polish remover. This reaction and other adverse effects and hazards of polish removers are reviewed. In addition to blister formation, polish removers may cause irritant and allergic contact dermatitis, onycholysis, paronychia, and brittleness. They represent a fire hazard and may cause systemic toxicity following excessive inhalation.

We report details of five patients with nail dystrophy associated with sarcoidosis. All had bone cysts in the underlying terminal phalanx and had a chronic course of the disease. Radiological examination is indicated in patients with sarcoidosis and nail dystrophy as bone cysts are frequently asymptomatic.

A case of pemphigoid is described which is complicated by ulcerative colitis and a cicatrizing nail dystrophy. A longitudinal nail biopsy revealed linear deposits of C3 and IgM at the basement membrane zone. The relevance of these findings is discussed.

In some sub- or pertrochanteric fractures of femur it is sometimes most difficult or even impossible to carry out an closed reduction on the extension table. In these cases an open reduction has to be performed. When such an open reduction is necessary, this should not give reason to dispense with the advantages offered by the Ender nailing method and select another technique of osteosynthesis. An early mobilisation, which is of particular importance to old patients, can be achieved best by the aforementioned osteosynthesis.

Two sporadic cases with congenital anonychia and hypoplastic nails combined with ectrophalangia or hypoplastic phalanges are reported. It is suggested that congenital anonychia and hyponychia may be 'bone territory' dependent disorders.
