Accessory nerve: anatomy and surgical identification.

The XIth cranial nerve or accessory nerve provides the motor supply to the sternocleidomastoid and trapezius muscles. It is frequently encountered during neck surgery, and as such is at risk of iatrogenic injury, resulting in the 'shoulder syndrome'. Historically, the nerve was sacrificed on oncological grounds during radical neck dissection. However, the basis for sacrifice is unfounded in the majority of cases, and accessory nerve sparing selective neck dissection has equal oncological efficacy. The path of the nerve in the neck is very variable, and there is not a wholly reliable landmark f…

Organ preservation in T4a laryngeal cancer: is transoral laser microsurgery an option?

The main objective of this study is to assess the feasibility of transoral laser microsurgery (TLM) in the treatment of T4a laryngeal cancer and to report the oncological and functional outcomes. This is a retrospective case-series study, held in a single-institute, academic tertiary referral center. Seventy-nine patients with previously untreated T4a glottic (n = 31, 39 %) or supraglottic laryngeal carcinoma (n = 48, 61 %) were included in this study. Five patients (6 %) were treated exclusively by TLM, 16 (20 %) had TLM and unilateral neck dissection, 27 (35 %) had TLM and bilateral neck dis…

Systematic review and meta-analysis of cervical metastases in oral maxillary squamous cell carcinoma.

BACKGROUND: Management of the node-negative neck in oral maxillary squamous cell carcinoma (SCC), encompassing the hard palate and upper alveolar subsites of the oral cavity, is controversial, with no clear international consensus or recommendation regarding elective neck dissection in the absence of cervical metastases. AIM: To assess the occult metastatic rate in patients with clinically node negative oral maxillary SCC; both as an overall metastatic rate, and a comparison of patients managed with an elective neck dissection at index surgery, compared to excision of the primary with clinica…

Effectiveness of lymphoscintigraphic sentinel node detection for cervical staging of patients with squamous cell carcinoma of the head and neck.

PURPOSE: To evaluate the feasibility and staging ability of the sentinel node (SN) technique for patients with squamous cell carcinoma of the oral cavity or oropharynx and clinically negative necks. This prospective study compares the histopathologic status of the SN with that of the remaining neck dissection tissues. PATIENTS AND METHODS: Thirty previously untreated patients with T1 to T4 squamous cell carcinoma of the oral cavity or oropharynx and clinically negative necks (N0) were included in the study. Injection of 99m Tic-radiolabeled sulfur colloid around the primary tumor and lymphosc…

Bow hunter's syndrome after contralateral vertebral artery dissection.

Bow hunter's syndrome is characterized by transient vertebrobasilar insufficiency that is elicited by neck rotation. This syndrome is has various causes, such as osteophytes, tumors, fibrous bands, infection, and trauma. We report a unique case of bow hunter's syndrome. The patient visited our hospital because of left nuchal pain. A magnetic resonance imaging scan revealed left vertebral artery (VA) dissection, which was the cause of his nuchal pain. He began to feel faintness upon turning his neck to the left after left VA dissection. Digital subtraction angiography revealed that the right VA

Robotic neck surgery using retroauricular approach - Experience of 60 procedures.

Remote access approaches to the neck have gained attention due to cosmetic concerns with conventional cervical incisions. Their safety, reproducibility, and oncologic outcomes remain to be fully validated. We retrospectively analyzed our initial experience with retroauricular robotic neck surgery using the Da Vinci system. Thirty-two patients were included. Data collected comprised demographics, procedure type, surgical features, and oncological follow-up. Thirty-five surgeries were performed: 13 posterolateral neck dissections (levels II-V) with central (VI), 5 modified radical dissections (I…

Variations in origin of the superior thyroid artery: an update for the head and neck surgeon.

Head and neck surgeons must have a thorough knowledge of head and neck vascular anatomy and its variations. This case report documents a variation in the superior thyroid artery encountered during a neck dissection and discusses the surrounding literature. A 55-year-old female with squamous cell carcinoma of the tongue underwent a partial glossectomy, right level I-IV neck dissection and reconstruction with a radial forearm free flap. During the procedure, an arterial branch was encountered arising 2-3cm caudal to the common carotid bifurcation. This variant branch was shown to represent the s…

Inoperable bulky melanoma responds to neoadjuvant therapy with vemurafenib.

A patient with a bulky inoperable stage IIIC melanoma involving the left axilla and neck from a primary of the left medial elbow received vemurafenib as neo-adjuvant treatment. Based on the molecular analysis, BRAF V600E mutation was present. After 4 months of vemurafinib treatment, the tumours shrank to less than 50% of original clinical size and allowed the surgeons to perform a left modified radical neck dissection and left radical axillary dissection. Pathological analysis of specimen revealed viable metastatic cells only in 1 of 40 nodes resected in the neck and axillary dissection, accou…

Choice of treatment for stage I floor-of-mouth cancer. A decision analysis.

Controversy exists over optimal treatment of stage I floor-of-mouth cancers, with advocation of either primary radiotherapy or surgical resection with or with-out a neck dissection. A decision analysis was undertaken to determine the outcomes of these treatment options. The quality-adjusted life expectancies were determined using best estimates from the literature of survival, recurrence, short-term morbidity, and quality of life for radiotherapy and surgical resection. The quality-adjusted life expectancies were as follows: for surgical resection with neck dissection, 18.50 years; for surgica…

A Muscle Variant of the Lateral Neck Region: The Cleido-Vertebral Muscle.

INTRODUCTION: Anatomical variations of the neck muscles have previously been reported, involving in particular the omohyoid and sternothyroid muscles. We herein report a novel variant neck muscle found during a routine surgical procedure. CASE REPORT: A 63-year-old women underwent a pelvi-mandibulectomy with bilateral neck dissection for a squamous cell carcinoma of the floor of mouth pT3N1. On the right neck dissection, the present peculiar muscle was discovered. It was located in the lateral region of the neck, deeply to the sternocleidomastoid muscle and caudally to the hyoid bone. It took…

Cervical lymph node metastatic patterns of squamous carcinomas in the upper aerodigestive tract.

The radical neck dissection specimens of 384 ethnically Chinese patients with different primary squamous carcinomas in the head and neck region were studied. Over 50 per cent of the specimens showed metastatic disease at one level in the neck. For oral cavity carcinoma, the levels of metastasis frequently involved were I, II and III while for carcinoma of the oropharynx, hypopharynx and larynx the levels were II, III and IV. Extracapsular spread was present in 112/384 of patients (29 per cent) and this increased with advancing N-stages. Based on these findings, different selective neck dissect…

[Identification of sentinel lymph node in neck-node-negative oral and pharyngeal carcinoma study of patients, it's feasibility, and problems].

We assessd the feasibility and problems associated with sentinel lymph node (SLN) study in 13 cases of oral and pharyngeal squamous cell carcinoma (SCC) that were neck-node-negative clinically. The primary sites were the tongue (n = 10), other sites in the oral cavity (n = 2), and the mesopharynx (n = 1). The day before surgery, tracer was injected into the submucosa around the tumor, and scintigraphic images were acquired 2 hours later. The SLN was identified intraoperatively with a handheld gamma probe, and neck dissection, including the SLNs, was performed. Radioactivity within the nodes wa…
