Abnormalities of peripheral nerve conduction in relation to thiamine status in alcoholic patients.

Thiamine status was evaluated using the erythrocyte transketolase activation assay in 20 alcoholic patients admitted on a voluntary basis to a Detoxification Unit. Electromyographic evaluation revealed significant reductions of motor and sensory conduction velocities in the alcoholic group. 38% of alcoholic patients showed significant erythrocyte transketolase activation deficits indicative of severe thiamine deficiency. In the case of peroneal nerve, reduced conduction velocities were negatively correlated with abnormal transketolase parameters. These findings are consistent with a contributo

Mononeuropathy in sickle cell anemia: anatomical and pathophysiological basis for its rarity.

Peripheral neuropathy is a rare complication of sickle cell disease. We report a young black woman with sickle cell anemia who developed a proximal median mononeuropathy in the setting of sickle cell crisis. The clinical and electrodiagnostic features are consistent with an ischemic mechanism from the sickling process. The pathophysiological basis for the rarity of this complication may be related to the rich anastomotic microvasculature of peripheral nerve and the unique large size of the capillaries of this vascular network.

Electrophysiological improvement following decompression surgery in tarsal tunnel syndrome.

Plantar nerve conduction studies 14 months to 3.5 years after decompression surgery in 3 cases of tarsal tunnel syndrome showed an improvement in motor conduction as well as in sensory nerve conduction. This electrophysiological improvement was associated with clinical improvement. However, minor abnormalities still existed in sensory nerve conduction in all 3 cases.

Electrophysiological studies of various graft lengths and lesion lengths in repair of nerve gaps in primates.

Electrophysiological studies were used to evaluate neurological recovery in 14 rhesus monkeys with different nerve lesion lengths and graft lengths. After exposure of both sciatic nerves in each animal, baseline evoked nerve action potentials, muscle action potentials, and muscle strength values were determined for the posterior tibial nerves. Each nerve was then crushed over a measured distance. Three weeks later, the crushed segments were resected and the defects repaired with sural nerve grafts. In seven animals, 20-mm resection sites were repaired by 4 x 20-mm grafts in one leg and by 4 x

Ischaemic lumbosacral plexopathy in acute vascular compromise: case report.

Anterior spinal artery syndrome (ASAS) is a well reported cause of spinal cord injury (SCI) following thoracoabdominal aortic surgery. The resultant deficits are often incomplete, typically attributed to the variable nature of the vascular distribution. Our Physical Medicine and Rehabilitation (PM and Rehabilitation) service was consulted about a 36-year-old patient with generalised deconditioning, 3 months after a stab wound to the left ventricle. Physical examination revealed marked lower extremity weakness, hypotonia, hyporeflexia, and a functioning bowel and bladder. Further questioning di

Neurophysiology of postviral fatigue syndrome.

The exact pathophysiology of excessive fatigue in patients with postviral fatigue syndrome (PVFS) remains uncertain in spite of increasing investigation. One objective abnormality of neuromuscular function is the increased jitter on single fibre EMG studies. While this is a sensitive technique which indicates a disturbance in the peripheral part of the motor unit, it is non-specific and its role in the pathophysiology remains unclear. Impaired muscular activation with added force in response to superimposed electrical stimulation suggests an extra-muscular and/or central component of fatigue.

Vasculitic neuropathy. A clinical and pathological study.

The clinical, electrophysiological and pathological features and prognosis of 34 patients with peripheral neuropathy caused by necrotizing vasculitis were evaluated. The causes included polyarteritis nodosa and its Churg-Strauss variant, rheumatoid arthritis, undifferentiated connective tissue disease, Wegener's granulomatosis, primary Sjögren's disease, and chronic lymphocytic leukaemia with cryoglobulinaemia; 2 patients had no evidence of systemic vasculitis. Mononeuritis multiplex was the most common clinical manifestation, followed by asymmetrical polyneuropathy and distal symmetrical poly

Inclusion body myositis. An electrophysiologic study.

Inclusion body myositis is a rare and slowly progressive myositis associated with cytoplasmic inclusions and fibrillar nuclear material. These histopathologic findings are of unknown significance. The clinical presentation of IBM has marked similarities to that of chronic polymyositis with proximal greater than distal weakness and muscle wasting more pronounced in the lower than upper extremities. In contrast to polymyositis, however, relatively few individuals report neck flexor weakness or dysphagia. Corticosteroid treatment is usually ineffective. The clinical, histopathologic and electroph

Diaphragm motor unit recruitment during ventilatory and nonventilatory behaviors.

The forces generated by the cat diaphragm (DIA) during different ventilatory and nonventilatory behaviors were determined by measuring transdiaphragmatic pressures (Pdi). The Pdi generated during eupnea was only approximately 12% of the maximum Pdi (Pdimax) generated by bilateral phrenic nerve stimulation. When the animals breathed a gas mixture of 10% O2 and 5% CO2, the Pdi increased to approximately 28% of Pdimax. During total airway occlusion, the Pdi generated by the diaphragm increased to approximately 49% of Pdimax. Only during the gag reflex and sneezing did Pdi reach maximal levels. A

[Value of somatosensory evoked potentials in thoraco- brachial outlet syndrome].

INTRODUCTION: Brachial plexus involvement in symptoms of thoracic outlet syndrome (TOS) is often difficult to assess from clinical data. Conventional EMG and nerve conduction studies (NCS) do not seem reliable to all authors. For this reason, our investigations of this syndrome were complemented by study of somatosensory evoked potentials (SEP) in order to compare the results of these different techniques. PATIENTS AND METHODS: Ten patients were studied, all of whom had prominent vascular symptoms which led to their consulting a vascular surgeon. Only one had hand wasting without hypoesthesia

[Motor function study in traumatic lesions of the brachial plexus].

The muscular tone of flexors and extensors was studied in dynamics in patients with trauma of the brachial plexus. The study involved measurement of mechanical resistance of a passive limb periodical movements at the elbow joint. The electrogram was registered synchronously with the mechanogram to study the reactions of the agonists and the antagonists. The results made it possible to judge the maintenance of nerve conduction along the efferent and afferent pathways. The method also enabled the authors to determine the degree of disorders and restoration of functioning of the neuro-muscular co

The response of a patient with von Recklinghausen's disease to succinylcholine and atracurium.

The anesthetic management of a 17-year-old female with von Recklinghausen's disease (multiple neurofibromatosis) is described. Multiple neurofibromatosis is a rare syndrome characterized by abnormal cutaneous pigmentation and numerous skin tumors. In addition to the usual stigmata of the syndrome, patients with neurofibromatosis have been reported to be either sensitive or resistant to succinylcholine. Further, all reports indicate that response to nondepolarizing muscle relaxants is exaggerated. In this patient, the nerve conduction studies were within limit of normal values. During surgery,

Continuous motor unit activity confined to the upper extremities.

We report a patient with clinical vermiform motor activity, muscle cramps, delayed relaxation of grip, and continuous motor unit discharges who developed mild symmetric symptoms in the upper extremities following a viral infection. Treatment with carbamazepine produced considerable symptomatic improvement. Needle electromyography (EMG) in several muscles showed irregular grouped spontaneous discharges composed of potentials resembling normal motor units. The discharges occurred asynchronously in distal muscles with identical and different peripheral nerve innervation but were not seen in more

An electrophysiological and histological study of myelinated axon regeneration after peripheral nerve injury and repair in the cat.

Electrophysiological and histological methods have been combined to obtain quantitative measures of the success of regeneration of myelinated axons in a cutaneous nerve after injury and repair by a variety of procedures. Following a simple transection injury more axons regenerated successfully when the nerve was repaired by epineurial suturing or stump suturing than when it was left unrepaired; both types of repair gave similar results. After loss of a 10-mm piece of the nerve trunk, repair with an autograft produced more regeneration than when the nerve was left untouched, but repair by stump

Determining neurapraxia in carpal tunnel syndrome.

In the evaluation of carpal tunnel syndrome by nerve conduction testing it is desirable to produce prognostic as well as diagnostic information. The finding of conduction block, or neurapraxia, is regarded as evidence that treatment can result in prompt recovery of nerve function. A technique is presented for stimulating the motor branch of the median nerve in the palm in order to detect the degree of neurapraxia due to entrapment in the carpus. In 23 normal subjects the response after palmar stimulation compared to a wrist stimulation site had a mean increase in amplitude of 0.56 mV. Persons

Phalen's test in the diagnosis of carpal tunnel syndrome.

The wrist flexion test of Phalen has been studied in 127 patients with carpal tunnel syndrome and 20 control subjects, all of whom also had nerve conduction studies. There was a statistically significant relationship between the severity of electrical changes and the probability of a positive Phalen's test. However 34% of patients had a negative test and 20% of control subjects a positive one.

[Role of the autonomic nervous system in the genesis of the electrophysiologic pattern of the double nodal pathway].

Thirty-five patients (age: 53 +/- 13 years) with dual A-V nodal pathways pattern were studied following autonomic blockade (propranolol 0.2 mg/kg and atropine 0.04 mg/kg). Six of them had "idionodal" paroxysmal reentrant supraventricular tachycardia. After autonomic blockade the dual A-V nodal pathways pattern remained in 20 patients (57%) and disappeared in 15; however, in the latter, the effective refractory period of the fast pathway decreased significantly and the disappearance of the slow pathway conduction may not have been real since the marked shortening of the fast pathway refractory
