Clinical diagnosis in preclinical stage of Alzheimer's disease.

Preclinical diagnosis of Alzheimer's disease using biomarkers has become an area of great interest for both clinicians and researchers because, among other advantages, this would increase the response to new disease-modifying drugs. However, difficulties with compliance and economic costs marginalize many countries worldwide that do not have access to this new type of diagnosis. The opportunity exists to refine the conventional clinical method, without using biomarkers, to attempt an earlier diagnosis as there is information that supports the potential utility of a focused clinical interview,

Co-occurrence of parkinsonism and dementia in clinical practice: relevant differential diagnoses.

Co-occurrence of parkinsonism and dementia is commonly observed in the aging population. This narrative review gives an overview of disorders regularly presenting with these symptoms, e.g., idiopathic Parkinson disease with dementia, dementia with Lewy bodies, progressive supranuclear palsy, corticobasal degeneration syndrome, vascular cognitive impairment, drug-induced parkinsonism, and normal-pressure hydrocephalus. Both a thoroughly performed medical history and a comprehensive clinical examination can narrow down relevant differential diagnoses. Characteristic clinical and neuropsychologic

Ethical and medicolegal issues in neuromonitoring during thyroid and parathyroid surgery: a review of the recent literature.

PURPOSE OF REVIEW: Neuromonitoring of the recurrent laryngeal nerve is increasingly utilized in thyroid and parathyroid surgery. Many studies have examined this practice with respect to rates of nerve injury. We have reviewed this literature with specific focus on ethical and medicolegal issues. RECENT FINDINGS: The preponderance of data fail to show that neuromonitoring significantly reduces the rates of nerve injury in thyroid and parathyroid surgery. However, potential benefits can nevertheless be identified for patients in predicting nerve injuries and reducing risks of bilateral recurren

Chiari type I malformation: a mimicker of myasthenia gravis.

INTRODUCTION: Myasthenia gravis is an autoimmune disease, which commonly presents with extraocular muscle weakness, eyelid ptosis, bulbar dysfunction, and proximal limb weakness. We report an unusual differential diagnosis for myasthenia gravis. CASE REPORT: A 56-year-old woman presented with worsening blurry vision, double vision, eyelid droopiness, slurred speech, and fatigable limb weakness, worsening over a 6-month period. On neurological examination, she showed dysarthric speech, ptosis, and proximal limb weakness with preserved reflexes. Myasthenia gravis was considered strongly, but se

Electroclinical features of benign infantile seizures with mild gastroenteritis.

The purpose of this study was to analyze the electroclinical features of patients with benign infantile seizures with mild gastroenteritis and demonstrate the benign nature of this entity. From 30 patients who were included in the study, five were excluded (two with developmental delay, one with microcephaly and two lost during follow-up). Twenty-five patients who fulfilled the diagnostic criteria for benign infantile seizures with mild gastroenteritis were assessed and followed at the Pediatric Department, Armed Forces Hospital, Southern Region, Khamis Mushayt, Saudi Arabia, between January 2

[Progressive supranuclear palsy: what's new?].

Progressive supranuclear palsy (PSP) has been described as a clinical syndrome characterized by an impairment of voluntary control of gaze (supranuclear palsy), postural and gait instability, and behavioral and cognitive deficits including a frontal syndrome and psychic retardation. However, in the recent years, at least four other clinical forms of PSP have been recognized: PSP-Parkinsonism, "pure akinesia with gait freezing", PSP with cortico-basal syndrome, and PSP with speech apraxia. PSP-Parkinsonism mimics the signs and symptoms of idiopathic Parkinson's disease, including a significant

Cognitive deficits presenting as psychiatric symptoms in a patient with Moyamoya disease.

The case of a middle-aged man who had intraventricular hemorrhages secondary to Moyamoya disease (MMD), and initially, presented with psychiatric symptoms which did not respond to treatment is described. Neuropsychological assessment showed underlying significant cognitive deficits, mostly of complex attention and speed of information processing, visuospatial and constructional abilities, verbal and nonverbal memory, and executive functions. These deficits remained stable or slightly improved in follow-up assessments. Cognitive dysfunction should be suspected in case of psychiatric or psychiat

A case of central sleep apnea strictly dependent upon REM-sleep.

Central sleep apnea (CSA) is characterized by the inability to generate regular breathing patterns as a result of the loss of metabolic drive and failure of respiratory muscle control. We present the case of a 54-year-old woman with a severe CSA strictly dependent upon REM-sleep. Extensive diagnostic workup excluded typical underlying causes and serological analysis revealed acute borreliosis infection. The severity of sleep apnea decreased after repeated polysomnographic studies without a specific therapy. CSA is usually associated with non-REM stages of slow-wave-sleep. This report illustrat

Complex visual hallucinations as a presentation of temporal low-grade glioma.

We report a patient with an unusual presentation of a temporal low-grade glioma with visual symptoms of formed, coloured meaningful images without coexistent psychiatric symptoms or epileptiform activity consistent with a diagnosis of visual hallucinosis. The location and extent of the lesion on the MRI differs from the lesions commonly associated with this diagnosis.

Neurological symptoms in patients with biopsy proven celiac disease.

In celiac disease (CD), the gut is the typical manifestation site but atypical neurological presentations are thought to occur in 6 to 10% with cerebellar ataxia being the most frequent symptom. Most studies in this field are focused on patients under primary neurological care. To exclude such an observation bias, patients with biopsy proven celiac disease were screened for neurological disease. A total of 72 patients with biopsy proven celiac disease (CD) (mean age 51 +/- 15 years, mean disease duration 8 +/- 11 years) were recruited through advertisements. All participants adhered to a glute

An algorithm for identifying and classifying cerebral palsy in young children.

OBJECTIVE: To develop an algorithm on the basis of data obtained with a reliable, standardized neurological examination and report the prevalence of cerebral palsy (CP) subtypes (diparesis, hemiparesis, and quadriparesis) in a cohort of 2-year-old children born before 28 weeks gestation. STUDY DESIGN: We compared children with CP subtypes on extent of handicap and frequency of microcephaly, cognitive impairment, and screening positive for autism. RESULTS: Of the 1056 children examined, 11.4% (120) were given an algorithm-based classification of CP. Of these children, 31% had diparesis, 17% h

Clinical measurement of mobility and balance impairments in Huntington's disease: validity and responsiveness.

BACKGROUND: Mobility and balance in Huntington's disease (HD) are currently assessed in the clinic with three items from the unified Huntington's disease rating scale (UHDRS): walk, tandem and pull tests. These tests may not be optimal because they are scored on an ordinal scale and do not test anticipatory balance. We tested the validity and responsiveness of three clinical tests of mobility and balance. METHODS: Three clinical tests (FRT, timed up and go (TUG), Berg balance scale (BBS)) were validated with seven quantitative gait measures and two indicators of functional limitation (HD-ADL

[Familial trigeminal neuralgia].

Trigeminal neuralgia in its classic form is usually an isolated disease that occurs in sporadic fashion, but familial cases have been described. We report the case of a 45-year-old man who presented with left V2 trigeminal neuralgia. The neurological examination was normal and imaging and laboratory investigations were non-contributive to the etiological work-up. Carbamazepine gave excellent symptomatic relief. During patient evaluation, we became aware of a clustering of trigeminal neuralgia in four other family members over three generations. Familial trigeminal neuralgia has been expounded

Is this patient having a stroke?

CONTEXT: Patients suspected of having a stroke or transient ischemic attack require accurate assessment for appropriate acute treatment and use of secondary preventive interventions. OBJECTIVE: To update a 1994 systematic review of the accuracy and reliability of symptoms and findings on neurological examination for the evaluation of patients with suspected stroke or transient ischemic attack. DATA SOURCES: We identified potential articles dated between 1994 and 2005 by multiple search strategies of the MEDLINE database and review of article and textbook bibliographies along with private col

Deep brain stimulation and essential tremor.

Essential tremor (ET) is one of the most common movement disorders and can cause significant functional disability in some patients. Although medications can improve tremor in some patients, approximately 50% of patients have medication-resistant symptoms. In patients with ET who experience marked disability due to the tremor, surgical therapy, primarily deep brain stimulation (DBS) of the ventral intermediate (Vim) nucleus of the thalamus, is a viable treatment option. Multiple studies of DBS of the thalamus have shown that it is efficacious in the treatment of ET hand tremor, often with seco
