Chromosome 16p13.11 Microdeletion Syndrome in a Newborn: A Case Study.

Chromosome 16p13.11 microdeletion syndrome is a rare copy number variant that carries increased risks for complications in the neonatal period and throughout the life span. Clinical manifestations and associated defects known to present in the neonatal period include motor delay, facial dysmorphisms, microcephaly, gastroesophageal reflux disease (GERD), and congenital heart defects. Management in the neonatal period focuses on associated comorbidities, including motor delay with or without GERD, which commonly manifests as feeding difficulties. Life span implications of chromosome 16p13.11 mic

Clinical experience with ceftriaxone treatment in the neonate.

The safety of ceftriaxone has been evaluated in 80 neonates who were treated empirically for suspected infection with either ceftriaxone and ampicillin (group A, age 0-72 h) or ceftriaxone and vancomycin (group B, age greater than 72 h). Within 48 h after birth 2 group A patients died from sepsis (Haemophilus influenzae, Streptococcus pneumoniae, 1 case each); 1 group B patient died from sepsis (Pseudomonas aeruginosa). All bacterial isolates from group A patients were susceptible to ceftriaxone, but in 4 of the 8 group B patients with positive cultures a change in antibiotic therapy was requi

Pericardiocentesis Simulation Model: Low Cost, Easy to Assemble, Effective Task Trainer.

Pericardial effusion is a potentially lethal condition. If it is discovered early, it can be treated by pericardiocentesis under controlled condition with imaging by experienced care providers. If it is diagnosed at a later stage with clinical compromise, then an emergent procedure might be necessary. Since it is encountered infrequently, many providers may have little or no experience in managing the condition and performing a life-saving pericardiocentesis. This is a technical report that describes the creation of a neonatal model for pericardiocentesis. This is a high-fidelity, low-cost mod

Maternal-infant bonding for the opioid exposed dyad: A rodgers' evolutionary concept analysis.

AIM: This concept analysis aimed to offer insight into how opioid exposure affects the quality and construction of the maternal-infant bond. BACKGROUND: Maternal-infant bonding can be influenced by maternal opioid use disorder and the neonatal intensive care unit environment. Many interventions for mothers with opioid use disorder focus on enhancing parental skills, but often the emotional and relational features of the maternal-infant bond have been overlooked. DATA SOURCE: Literature from Cumulative Index to Nursing and Allied Health Literature (CINAHL), Embase, PubMed, and PsycINFO publis

Effectiveness of eHealth interventions for parents of preterm infants during transition from neonatal intensive care unit-to-home: A systematic review and meta-analysis.

BACKGROUND: Early intervention programmes increasingly use eHealth components to extend family-centred support beyond neonatal intensive care unit discharge. However, their effectiveness on parental and infant health outcomes during the transition from neonatal intensive care unit to home remains unclear. OBJECTIVE: To assess the effectiveness of eHealth early intervention programmes supporting parents of preterm infants during the transition from neonatal intensive care unit to home on parental and infant outcomes. DESIGN: Systematic review and meta-analysis. DATA SOURCES: Medical Literatu

Interaction of iron and folate during reproduction.

Biochemical evidence collected from both clinical and non-institutionalized populations indicate that iron and folate deficiencies frequently occur simultaneously. Supplementation trials of iron-deficient patients with either iron or iron and folate has helped to illustrate that a complex interrelationship exists between these two nutrients. Controlled animal trials in which dietary iron and folate content has been systematically manipulated reveal that iron deficiency can cause altered folate utilization. The impact of iron deficiency on folate metabolism is most dramatic during the reproduct

Measurement of acute pain in infants: a review of behavioral and physiological variables.

BACKGROUND: The use of non-validated pain measurement tools to assess infant pain represents a serious iatrogenic threat to the developing neonatal nervous system. One partial explanation for this practice may be the contradictory empirical data from studies that use newborn pain management tools constructed for infants of different developmental stages or exposed to different environmental stressors. PURPOSE: The purpose of this review is to evaluate the evidence regarding the physiologic and behavioral variables that accurately assess and measure acute pain response in infants. METHODOLOGY

Waiting to be weighed: a pilot study of the effect of delayed newborn weighing on breastfeeding outcomes.

Although breastfeeding initiation rates are rising in Canada, rates of continued breastfeeding remain far below international recommendations. Two factors that can have a positive influence on breastfeeding outcomes are maternal confidence and support and education provided by public health nurses (PHNs). The weighing of newborns by PHNs within the first days after birth is standard practice in monitoring neonatal health. However, little is known about the effect of the timing of PHN neonatal weighing on maternal confidence or on outcomes such as intended duration of breastfeeding and formula

Is visual assessment of jaundice reliable as a screening tool to detect significant neonatal hyperbilirubinemia?

OBJECTIVE: To assess the reliability of visual assessment of bilirubin levels (BiliEye) in newborns as a screening tool to detect significant neonatal hyperbilirubinemia. STUDY DESIGN: 5 neonatologists and 17 nurses estimated 3,532 BiliEye in 1,129 term and late preterm (> or = 35 weeks) infants before discharge from the nursery, at 62 +/- 24 hours. Total serum bilirubin (TSB) levels were measured concomitantly. RESULTS: Mean TSB and BiliEye were 6.7 +/- 2.9 mg/dL (range, 0.4-18.2 mg/dL) and 6.6 +/- 3.2 mg/dL (range, 0.0-17.2 mg/dL), respectively, with good correlation (Pearson's r = 0.752,

Outcomes of a Quality Improvement Initiative to Prevent Unnecessary Packed Red Blood Cell Transfusions Among Extremely Low Birth-Weight Neonates.

BACKGROUND: Extremely low birth-weight (ELBW) infants frequently receive packed red blood cell (PRBC) transfusions. Recent studies have shown that more restrictive PRBC transfusion guidelines limit donor exposure and reduce transfusion-related costs without any increase in adverse clinical outcomes. PURPOSE: We developed and implemented an evidence-based PRBC transfusion guideline for ELBW infants treated in our unit and then measured provider adherence to this guideline. METHODS/SEARCH STRATEGY: We performed a retrospective review of all PRBC transfusions given to ELBW infants in 2012 (preg

Developing a process to support perinatal nurses after a critical event.

The work of perinatal nurses sometimes includes emergencies involving death, or near death, which can leave health care providers with feelings of stress and grief. After experiencing a particularly stressful period, nurses at our organization identified processes to help themselves recover and to support each other. The result of this work is a written plan to facilitate the support of perinatal nurses after critical events. This article describes the development and implementation of this plan.

Interleukin 1beta level in human colostrum in relation to neonatal hyperbilirubinemia.

Breast-fed infants have higher bilirubin level than formula-fed infants which is of undetermined etiology. The cholestatic effect of cytokines (e.g. IL-1beta, IL-6) is believed to result from the repression of genes that normally mediated the hepatic uptake, metabolism, and biliary excretion of bile salts and bilirubin. The present study aimed to assess the level of interleukin 1beta (IL1beta) in early milk of nursing mothers and its relation to neonatal jaundice. Sixty full term neonates and their mothers were included. They were classified into two groups; group I included Forty neonates pat

Time domain correlation analysis of heart rate variability in preterm neonates.

BACKGROUND AND AIM: A fuller understanding of the neural control mechanisms of heart rate during the early stages of human development would be of great value to obstetric and neonatal management. In this paper, we investigate the correlation between heart rate variability (HRV) and other physiological parameters such as blood pressure and respiration in preterm neonates with the aim of developing a numerical model to explain and predict heart rate variability. STUDY DESIGN AND SUBJECTS: All the required data are readily available for premature babies who are routinely monitored while being n

The Mother Baby Comfort Care Pathway: The Development of a Rooming-In-Based Perinatal Palliative Care Program.

BACKGROUND: Traditionally, the provision of comfort care and support during the dying process for infants born with life-limiting diagnoses has occurred in the neonatal intensive care unit (NICU). A major goal for the families of these infants is often the opportunity to spend as much time as possible with their infant in order to make memories and parent their infant. PURPOSE: The objective of the Mother Baby Comfort Care Pathway is to implement a program of family-centered care with logistically flexible care delivery, allowing mothers and their families to share as normal a postpartum care
