Multifocal visual evoked potential evaluation for diagnosis of acute optic neuritis and for prediction of visual outcome and ganglion cell layer thinning following optic neuritis.

BACKGROUND: While damage to the optic nerve following optic neuritis (ON) is readily quantifiable, the evaluation of prognosis for visual function and neuroaxonal loss in the acute ON is challenging. OBJECTIVE: The objective of this study is to investigate the value of multifocal visual evoked potential (mfVEP) in acute ON, diagnostically for acute ON and prognostically for visual outcome and subsequent ganglion cell/inner plexiform layer thickness (GCLIPLt). METHODS: A prospective cohort study of mfVEP and full-field visual evoked potential (ffVEP) in acute, unilateral ON (onset < 31 days) …

MRI signs helpful in the differentiation of patients with anterior ischaemic optic neuropathy and optic neuritis.

BACKGROUND/AIMS: The aim of this study was to identify specific MRI characteristics of anterior ischaemic optic neuropathy (AION) and optic neuritis (ON) that would aid in the differentiation between these two diagnoses. METHODS: We retrospectively analysed a consecutive case series including all patients with an MRI study of brain and orbit and the clinical diagnosis of either ON or AION. We examined the scans for restricted diffusion of the optic nerve, optic sheath diameter, enhancement and location of enhancement of the optic nerve and distribution of the white matter lesions. RESULTS: F…

Structural and functional markers of optic nerve damage in myelin oligodendrocyte glycoprotein antibody-associated optic neuritis.

BACKGROUND: Optic neuritis (ON) occurs in immune-mediated disorders including multiple sclerosis (MS), aquaporin-4 antibody-positive (AQP4) neuromyelitis optica spectrum disorder (AQP4-NMOSD) and myelin oligodendrocyte glycoprotein (MOG) antibody-associated demyelination (MOGAD). Accurate determination of aetiology is critical for appropriate treatment and prognostication. OBJECTIVE: To evaluate demyelination and axonal loss in MOG-ON to facilitate differentiation from MS-ON and AQP4-ON. METHODS: 15 MOGAD patients with previous ON (25 eyes) underwent multifocal visual evoked potential (mfVEP…

Comparison between optic neuritis associated with antibody against myelin oligodendrocyte glycoprotein and presumed idiopathic optic perineuritis.

BACKGROUND: The aim of this study was to determine important clinical and radiological features that aid in distinguishing presumed idiopathic optic perineuritis (OPN) from optic neuritis (ON) associated with antibody against myelin oligodendrocyte glycoprotein (MOG-Ab). METHODS: This retrospective, case-control study recruited patients with MOG-Ab-associated ON from 2011 through 2018 and idiopathic OPN from 2009 through 2011. The presence of MOG-Ab was not investigated in idiopathic OPN, because MOG-Ab testing was not available until 2011. The clinical and radiological features and the disea…

Screening for Leber's hereditary optic neuropathy associated mitochondrial DNA mutations in patients with prominent optic neuritis.

Previous case reports demonstrated the presence of Leber's hereditary optic neuropathy (LHON) associated mitochondrial (mt) DNA mutations in patients presenting with prominent optic neuritis (PON). By screening the mtDNA, we have excluded the presence of these mutations in 22 patients with PON, indicating that the frequency of these mutations is less than 4.5% in our selected patient population. Reviewing the clinical data of these patients revealed that severe optic nerve atrophy developed in association with both the benign and the severely disabling form of Multiple Sclerosis (MS). This obs…

Optic neuritis and meningoencephalitis associated with an ovarian teratoma in a dog.

A 3-year-old intact female Pit Bull Terrier mix-breed dog was examined because of acute vision loss. Ophthalmologic examination was consistent with bilateral optic neuritis. Abnormalities were not detected on the remainder of the neurological examination. Brain MRI identified multifocal lesions involving the optic nerves and cortical gray matter, while cerebrospinal fluid analysis demonstrated mixed pleocytosis. Abdominal ultrasonography revealed a cystic ovarian mass. Ovariohysterectomy was performed and histopathologic examination was consistent with an ovarian teratoma. Neural tissue within…

Optic Neuritis in a Child With COVID-19: A Rare Association.

The recent coronavirus disease-2019 (COVID-19) pandemic has caused significant mortality and morbidity, affecting patients of all ages. COVID-19 affects various tissues and systems in the body, including the central and peripheral nervous systems. However, COVID-19 has rarely affected the eyes and caused optic neuritis. We report a unique case of COVID-19-related unilateral optic neuritis in a 10-year-old girl.

Current understanding of the epidemiologic and clinical characteristics of optic neuritis.

Optic neuritis is an ocular disorder whose pathogenesis has not been fully determined, although autoimmune mechanisms have been suggested to be involved in its development. In recent years, anti-aquaporin-4 antibody (AQP4-Ab) and anti-myelin oligodendrocyte glycoprotein antibody (MOG-Ab) have been shown to play major roles in the development of optic neuritis. Because these two antibodies target different tissues, optic neuritis can be classified by the type of antibody. AQP4-Ab-positive optic neuritis responds poorly to steroid therapy and has a poor prognosis in terms of visual acuity. On th…

A putative role for calpain in demyelination associated with optic neuritis.

Calcium activated neutral proteinase (calpain) is an endopeptidase present in the central nervous system which degrades myelin proteins. To examine the role of calpain in demyelination associated with optic neuritis, immunocytochemical expression of calpain was evaluated in Lewis rats with experimental optic neuritis. Calpain expression was increased in activated microglia, infiltrating macrophages, activated T cells, and reactive astrocytes in experimental optic neuritis compared to controls. Calpain activity and translational expression were also examined by Western blotting studies measurin…

The clinical and paraclinical profile of optic neuritis: a prospective study.

To define the clinical and paraclinical profile of optic neuritis (ON), patients with a suspicion of ON among a population of 1.5 million were examined over 2.5 years. A diagnosis of monosymptomatic ON was established in 74 patients. Cerebrospinal fluid (CSF) studies in 73 patients revealed oligoclonal IgG bands in 67% and 32 of 60 patients examined (53%) had three or more high signal lesions on magnetic resonance imaging (MRI). A strong correlation was found between oligoclonal bands and abnormal MRI. In 52 patients, two or more CSF examinations revealed strong variations in individual patien…
