In-111 pentetreotide has been recommended for the detection of somatostatin receptor (SSR) positive tumors. Sites with increased numbers of activated lymphocytes may also show In-111 pentetreotide with Graves' ophthalmopathy (GO) compared to various other orbital disorders. Five patients with GO, three patients with necrotizing scleritis, and two patients with orbital pseudotumor were imaged at 4 hours, 24 hours, and 48 hours after injection of In-111 pentetreotide. Although planar images did not show considerable uptake, SPECT slices showed marked and persistent uptake in the orbits unilatera…

BACKGROUND: The anatomy of the orbital arteries is well known, and their physiology is under intensive investigation. Their age-related or other pathologic alterations, however, are still largely unknown.
METHOD: We obtained at autopsy 16 orbits from 8 persons aged > 70 years, and 4 orbits from 2 persons between 40 and 50 years. Specimens were taken from 20 locations along the orbital arteries, from the internal carotid to the globe, and studied by light microscopy.
RESULTS: We observed the following arteriosclerotic changes, in order of decreasing frequency: intimal hyperplasia, medial atro…

Two patients with orbital neurofibromatosis associated with enophthalmos are presented. Assessment using 3D-CT scan shows an increase in the size of the orbital cavity and an enlargement of the inferior orbital fissure, which allows orbital fat to prolapse into the infratemporal fossa, causing enophthalmos.

BACKGROUND: The authors define orbital infarction as ischemia of all intraorbital and intraocular structures. It is a rare disorder due to the rich anastomotic vascularization of the orbit.
PATIENTS: The authors report three patients with orbital infarction with similar clinical presentations: acute blindness, orbital pain, total ophthalmoplegia, and anterior and posterior segment ischemia.
RESULTS: Etiologies of orbital infraction syndrome include occlusion of the common carotid artery (case 1), giant-cell arteritis (case 2), and mucormycosis (case 3).
CONCLUSION: Three cases discussed in …

The patient with a tender, erythematous, swollen eyelid represents a complex clinical challenge to the physician, who must arrive at a correct diagnosis from numerous differential possibilities. Knowledge of the anatomy of the orbit and surrounding structures and proper clinical and radiologic examination are necessary to accurately diagnose these conditions. Proper selection of antibiotic therapy and timely surgical intervention, directed at both the orbital infection and the underlying condition, are essential to avoid serious morbidity from these infections. This article reviews in detail p…

Retrobulbar haemorrhage is a rare complication of trauma or surgery involving the orbit or periorbital region. Permanent blindness may result if early diagnosis is not made and treatment carried out. A simple scheme of orbital observations to aid early diagnosis of this complication and other sequelae of orbital trauma and surgery has been used at St Lawrence Hospital since 1983.

We describe a 39-year-old man in whom progressive pulsatile proptosis developed 2 days after he suffered a head injury. Computed tomography revealed an isolated blow-in fracture of the orbital roof with herniation of the left frontal lobe into the orbit. Neurosurgical intervention to repair the orbital defect led to full recovery.

Orbital hypertelorism as defined by increased bony interorbital distance is a complex manifestation of many congenital anomalies. As such, it is associated with a multitude of orbital, adnexal, and surrounding soft-tissue deformities. Treatment of these problems requires extensive experience and a comprehensive team, permitting adequate preoperative evaluation, diagnosis, and treatment. The results obtained may be dramatic. The overall safety of the procedures has been demonstrated. Continued refinements and long-term follow-up will undoubtedly continue to make significant improvements.

A case of mucormycosis in a 46-year-old woman with acute lymphoblastic leukemia is described. The typical clinical findings, diagnostic procedures, and therapeutic considerations are discussed. Mucormycosis is a rare complication in immunocompromised patients. Early ophthalmological diagnosis and rigorous therapy may improve the course of this otherwise fatal disease.

A method for the clinical evaluation of patients with orbital and midfacial fractures is presented. Pertinent anatomic relations, classification of fractures, and significant associated injuries are reviewed. Principles of treatment are offered.

The orbital apex syndrome is a very rare complication of fractures of the facial skeleton, as well as other conditions and is characterized by blindness, fixed dilated pupils, proptosis, ptosis of the eye and ophthalmoplegia. We are reporting such a case we had the opportunity of treating.

Seemingly trivial adnexal injuries may be associated with extensive injuries to the globe, orbit, and brain. A meticulous ophthalmologic examination and orbital and brain computed tomography (CT) scans (axial and coronal views) are essential for complete evaluation of these patients. The ophthalmologist must remain an active participant in the management of these patients to preserve the integrity of the globe.

Blindness from orbital hemorrhage following surgery, direct trauma, and remote trauma is not common. Compression cyanosis is a rare condition which may cause blindness. Non-ocular injuries may direct attention elsewhere and emergency surgical procedures may be necessary to save life. Orbital decompression can be carried out simultaneously with the other surgery. One case of blindness from abdominal compression and another with orbital, chest, and arm trauma are presented of patients who had emergency orbital decompression along with their other surgical procedures.

We report two patients who underwent orbital exploration yielding the diagnosis of sclerosing orbital pseudotumor. The presenting symptoms were exophthalmos, visual loss, abnormal ocular mobility, and ocular pain. Computed tomographic (CT) scans showed masses in the orbital apex. Steroids were ineffective. Orbital pseudotumor is a heterogeneous diagnostic category of lymphoid infiltrations of the orbit with a wide spectrum of pathological conditions and intraorbital locations. The clinical presentation typically includes the sudden onset of pain, diplopia, lid edema, and exophthalmos. Visual l…

A small number of patients suffer penetrating injury to the orbit in the absence of trauma to the globe. Hospital records from the Jules Stein Eye Institute and affiliated hospitals were reviewed to obtain such cases. Five cases of penetrating orbital trauma, in which the globe was spared, are discussed. Many of the injuries initially appeared trivial but subsequent complications were severe. A review of these cases suggests that the shape and speed of the penetrating object, the anatomical location of the entry wound, and the physical characteristics of intraorbital structures determine the c…

An 11-year-old Indian girl living in England developed proptosis due to tuberculosis of the orbit. The proptosis regressed and she recovered fully after chemotherapy. While malignancy, developmental anomalies, and nontuberculous infections are commoner causes, a tuberculin test should be included among the investigations of children with proptosis.
