A Cold World: Pain Outcomes and Patient Experiences Utilizing an Iceless Cold Compression System After Hand Surgery.

BACKGROUND: Cold therapy devices (CTDs) have been shown to provide analgesia, reduce swelling, and improve outcomes following orthopedic procedures. However, comparative data on postoperative narcotic use and pain control between CTDs and traditional ice packs remain limited. This study evaluates patient satisfaction and opioid use among those using CTDs after carpometacarpal (CMC) arthroplasty or distal radius open reduction internal fixation (ORIF). METHODS: A prospective analysis was conducted on 124 patients who underwent hand surgery by a single fellowship-trained surgeon between June 20

New Gen Halifax Nail with Novel Tri-Wire Concept for Intertrochanteric Fracture.

INTRODUCTION: Intertrochanteric fractures are common in elderly patients and can lead to significant malunion and varus deformities if untreated. While the dynamic hip screw has been the gold standard, there is a continuous effort to evaluate newer fixation devices. This study assesses the functional and radiological outcomes of intertrochanteric femur fractures treated with the Halifax nail, aiming to provide a contemporary comparison to existing methods. MATERIALS AND METHODS: This prospective study was conducted at our institution from January 2019 to December 2024. We included patients wi

Optimising bionic pore designs in a spiral blade implant to enhance bone ingrowth and stability: A combined CFD and large-animal study.

PURPOSE: This study aimed to evaluate the effects of different Bionic Pore geometries (circular, triangular, and rectangular) and diameters (0.8 mm, 1 mm, 2 mm, and 3 mm) on fluid shear stress (FSS) distribution and bone ingrowth in a bionic spiral blade implant. METHODS: Twelve Bionic Pore structures were modeled using CAD software, and computational fluid dynamics (CFD) analysis was performed to simulate the FSS distribution in a bone environment. Based on these simulation results, the circular hole design was selected and manufactured via electron beam melting (EBM). It was then incorporat

Strategies to minimize soft tissues and septic complications in staged management of high-energy proximal tibia fractures.

BACKGROUND: Soft tissues (wound dehiscence, skin necrosis) and septic (wound infection, osteomyelitis) complications have been historically recognized as the most frequent complications in surgical treatment of high-energy proximal tibia fractures (PTFs). Staged management with a temporary external fixator is a commonly accepted strategy to prevent these complications. Nonetheless, there is a lack of evidence about when and how definitive external or internal definitive fixation should be chosen, and which variables are more relevant in determining soft tissues and septic complications risk. Postoperative Radiographic Reports After Anterior Cruciate Ligament Reconstruction: Are We Assessing What Really Matters?

Background/Objectives: Anterior cruciate ligament reconstruction, often combined with anterolateral ligament reconstruction, requires accurate anatomical positioning of bone tunnels, which is a key determinant of surgical success. In current clinical practice, postoperative radiographs are routinely performed for both medico-legal and clinical purposes. The objective of this study is to analyze the content of early postoperative radiology reports following ACL ± ALL reconstruction and assess their clinical relevance regarding tunnel positioning, as well as the terminology used to describe surg

Novel technique for relieving anastomotic tension using halo-vest immobilization after tracheal sleeve resection.

We describe a novel technique of using halo-vest-enforced immobilization to relieve anastomotic tension after tracheal sleeve resection. Immediately after the tracheal sleeve resection, four halo titanium pins were inserted in the skulls of the patients to secure the halo-vest. All patients fitted with halo-vests were able to eat and drink and their clinical course was good. Bronchoscopy confirmed the absence of anastomotic leaks and stenoses, and there were no complications associated with the halo-vest. We believe that ensuring neck flexion using a halo-vest after tracheal sleeve resection i
