Managing the Operating Room Course for the Complex Pain Patient.

Patients with chronic pain require special attention when formulating the perioperative anesthetic plan. Understanding of the patient's preoperative medication regimen, physical and psychological limitations, type and location of pain, and presence of implanted analgesic devices is necessary to optimize the recovery process and minimize iatrogenic injuries. In general, it is recommended to continue the preoperative analgesic plan if not contraindicated and consider regional anesthetic techniques, multimodal analgesic adjuncts, and protocolized postoperative care plans (such as Enhanced Recover

Postoperative Pain Management in Orthognathic Surgery: Is There a Need for Opioids?

INTRODUCTION: Orthognathic surgery is traditionally considered painful, often prompting opioid use. In the context of rising opioid-related harm, this study assessed postoperative pain levels, factors influencing analgesic requirements, and the actual need for opioids after orthognathic procedures. METHODS: A retrospective analysis of 122 patients undergoing Le-Fort-I osteotomy, bilateral sagittal split osteotomy (BSSO), or bimaxillary surgery was performed. Pain levels (Numeric Analog Scale) were recorded 3 times daily for 5 postoperative days. Analgesic use was documented according to the W

What is the potential role of the nonopioid suzetrigine in pain management?

Effective pain management without the risk of addiction, abuse, or dependence associated with opioids remains a key clinical challenge in patient care. Suzetrigine, a newly approved drug for acute pain management, is a nonopioid with a novel mechanism of action: it selectively targets voltage-gated sodium channels present only on peripheral, pain-sensing nerves. In phase 2 and 3 clinical trials, suzetrigine was found to be more effective for post-abdominoplasty and -bunionectomy pain than placebo and was noninferior to an opioid-acetaminophen combination for pain following abdominoplasty. The …

Introducing Nurse Navigators to Improve Pain Management in the Community.

PURPOSE: Chronic pain among adults is a highly prevalent condition characterized by persistent gaps in care delivery, inequitable access to evidence-based interventions, and variable care continuity across clinical settings. Nurse Navigators are strategically positioned to help eliminate barriers by assisting in care coordination and person-centered resource maximization. A community-based nurse-led telehealth pilot program was implemented in the Northwestern United States as an extension of a state-funded evidence-based, online platform designed to promote nonpharmacologic pain management opt…

Towards an individualised approach to pain management in inflammatory arthritis.

Pain management in inflammatory arthritis (IA) remains challenging. The contribution of multiple mechanistic factors to the initiation and maintenance of pain, and the interplay of sensory, psychological and social factors that modulate perceptual outcomes, requires a multi-faceted approach that does not solely rely on inflammation-targeted therapies. Growing evidence points to the contribution of, and therefore the need to target, centrally mediated pain processes and/or peripheral sensitisation not associated with inflammatory burden. In this review, we examine current evidence on the mechan…

Pain associated with venous leg ulcers: the complexity of management.

Venous leg ulcers (VLUs) are a chronic and often debilitating condition, affecting a significant number of individuals worldwide. One of the most distressing aspects of VLUs is the persistent pain they can cause, which impacts quality of life, mobility and overall wellbeing. Despite advances in wound care, VLU-related pain remains under-recognised and poorly managed. Pain in VLUs is complex and typically includes nociceptive pain from tissue damage, neuropathic pain due to nerve involvement, and probable central sensitisation. Each type requires targeted treatment, yet clinical awareness and t

RATIONALE: Hereditary angioedema (HAE) is a rare genetic disorder caused by C1 esterase inhibitor (C1-INH) deficiency or dysfunction. Presentation with isolated abdominal pain is uncommon, often leading to diagnostic delays and inadequate pain management. Effective pain control is essential to improve patient comfort and prevent complications. PATIENT CONCERNS: A 21-year-old male with type II HAE presented exclusively with recurrent severe abdominal pain and experienced a transient loss of consciousness attributed to a pain-induced vagal reflex. DIAGNOSES: Diagnosis of type II HAE was confir…

Pain and fatigue in Parkinson's disease: advances in diagnosis and management.

Non-motor symptoms affect over 90% of people with Parkinson's disease at all stages, with pain and fatigue being among the most prevalent yet often neglected in clinical practice. Despite their significant impact, these symptoms are frequently underdiagnosed and inadequately treated, adding substantial burden to patients' lives. The lack of a clear definition and classification of these symptoms has led neurologists to underestimate their presence among people with Parkinson's disease and limited the ability to address them with mechanism-based care and effective treatments. Managing these sym

Pain Management in Brugada Syndrome: A Case Report and Review.

Brugada syndrome is a rare condition that increases the risk of life-threatening arrhythmias. Although there are existing anesthesia recommendations for patients with Brugada syndrome, guidance on pain management is limited. We present a novel approach to pain management in these patients, illustrated by the case of a young woman with Brugada syndrome who underwent ropivacaine peripheral nerve infusion and intravenous ketamine infusion for acute-on-chronic left upper limb pain. She received perioperative multimodal analgesia, which included neuropathic agents (pregabalin, duloxetine), and opio…

Advances in diagnosis, classification, and management of pain in Parkinson's disease.

With over 10 million people affected worldwide, Parkinson's disease is the fastest-growing neurological disorder. More than two-thirds of people with Parkinson's disease live with chronic pain, which can manifest in various stages of the disease, substantially affecting daily activities and quality of life. The Parkinson's disease Pain Classification System overcomes the limitations of previous classification systems by distinguishing between pain related to Parkinson's disease and unrelated pain, while also incorporating clinical and pathophysiological (mechanistic) descriptors such as nocice

Perioperative Pain Management for Mastectomy in Dogs: A Narrative Review.

Mammary tumours are the most common neoplasia in adult female dogs. Mastectomy leads to moderate to severe pain. Effective pain management is crucial in veterinary medicine. This review outlines analgesic techniques for managing perioperative pain in dogs undergoing mastectomy. A literature search on dog mastectomy analgesia was conducted from January 2001 to January 2025. Pre-emptive meloxicam reduces postoperative cardiovascular changes without affecting renal function. When combined with gabapentin, it lowers the need for rescue analgesic opioids, similar to robenacoxib. With regard to tram…
