Gene Expression Profiling of Tuberculous Meningitis Co-infected with HIV.

Tuberculous meningitis (TBM) is a fatal form of Mycobacterium tuberculosis infection of the central nervous system (CNS). The similarities in the clinical and radiological findings in TBM cases with or without HIV make the diagnosis very challenging. Identification of genes, which are differentially expressed in brain tissues of HIV positive and HIV negative TBM patients, would enable better understanding of the molecular aspects of the infection and would also serve as an initial platform to evaluate potential biomarkers. Here, we report the identification of 796 differentially regulated gene

[Diagnosis and therapy of fetal kidney and urinary tract malformations].

Of 23 patients with abnormal prenatal sonographic findings 17 also showed malformations of the kidneys and urinary tract in the postpartal period. Although even minor abnormalities could be detected by antenatal ultrasound, the identification of the abnormal findings was not possible in every case. Diagnostic problems arose in 8 foetuses showing mild dilatation of the collecting system, since it was not possible to distinguish between functional dilatation and dilatation from vesicoureteral reflux or minimal ureteral obstruction. Difficulties in identification of large malformations also occur

Raynaud phenomenon of the nipple: a rare finding in rheumatology clinic.

Many clinicians are familiar with the common presentation of Raynaud phenomenon affecting the hands and feet. Patients with Raynaud phenomenon, even in the absence of systemic disease, are frequently treated by rheumatologists. Raynaud phenomenon of the nipple is an important entity to recognize as a cause of severe nipple pain with breast-feeding and is perhaps underrecognized by patients and physicians. We describe a patient with Raynaud phenomenon of the nipple to improve identification of this clinical entity so that appropriate treatment may be instituted, thus allowing mothers to continu

Optical Biopsy of Dysplasia in Barrett's Oesophagus Assisted by Artificial Intelligence.

Optical biopsy in Barrett's oesophagus (BE) using endocytoscopy (EC) could optimize endoscopic screening. However, the identification of dysplasia is challenging due to the complex interpretation of the highly detailed images. Therefore, we assessed whether using artificial intelligence (AI) as second assessor could help gastroenterologists in interpreting endocytoscopic BE images. First, we prospectively videotaped 52 BE patients with EC. Then we trained and tested the AI pm distinct datasets drawn from 83,277 frames, developed an endocytoscopic BE classification system, and designed online t

Proteomics and bioinformatics strategies to design countermeasures against infectious threat agents.

The potential devastation resulting from an intentional outbreak caused by biological warfare agents such as Brucella abortus and Bacillus anthracis underscores the need for next generation vaccines. Proteomics, genomics, and systems biology approaches coupled with the bacterial ghost (BG) vaccine delivery strategy offer an ideal approach for developing safer, cost-effective, and efficacious vaccines for human use in a relatively rapid time frame. Critical to any subunit vaccine development strategy is the identification of a pathogen's proteins with the greatest potential of eliciting a prote

Risk prediction model for early detection of urinary tract infection in a hospital setting in Australia.

BACKGROUND: Hospital-acquired complications have detrimental effects on patient outcomes and recovery, with increased morbidity and mortality burdens, and hospital efficiency. The Australian Commission on Safety and Quality in Healthcare has identified 16 high-priority complications, including healthcare-associated infections, as potential targets of clinical risk mitigation strategies. Within the North Metropolitan Health Service in Western Australia, the prevalence of urinary tract infections (UTIs) was recognised as one of the most ubiquitous hospital-acquired complications and thus, there

Echocardiographic assessment of patient hemodynamics in heart failure.

Heart failure (HF) is a clinical syndrome which is due to cardiac structural and/or functional abnormalities that result in elevated intra-cardiac pressures and/or inadequate cardiac output. Hemodynamic assessment in HF allows the identification and characterization of cardiac dysfunction, systemic and/or pulmonary congestion and the eventual impairment of systemic perfusion which are fundamental to phenotype HF, risk stratify HF patients and to guide their treatment. Patient hemodynamics can be characterized invasively with right heart catheterization but also non-invasively with the use of e

Deep Dermatophytosis Caused by Zoophilic Strain of Trichophyton interdigitale with Successful Treatment of Itraconazole.

We report a 66-year-old female patient with deep dermatophytosis caused by zoophilic strain of Trichophyton interdigitale, a rare granulomatous presentation of Trichophyton species infection in patients with underlying systemic diseases, and she was successfully cured by itraconazole. Since the identification of Trichophyton mentagrophytes complex had been misused for years, a brief discussion of molecular diagnosis and taxonomy of T. mentagrophytes complex is given. The pathogenesis and comparison with cases reported in the literature are also discussed.

Detection of toxoplasma tachyzoites in the cerebrospinal fluid of a COVID-19 positive SLE patient: a case study.

The recurrence of latent toxoplasmosis is a frequent cause of toxoplasmic encephalopathy in individuals with immunosuppressive conditions or medication. If left untreated or diagnosed is delayed, it often resulting in fatality. The pathogen of Toxoplasma gondii has been identified in cerebrospinal fluid cytology and a limited number of documented cases of cerebral toxoplasmosis, typically observed in those with acquired immunodeficiency syndrome (AIDS). We report the identification of Toxoplasma gondii tachyzoites in the cerebrospinal fluid of a patient diagnosed with systemic lupus erythemato

Nonlinear System Identification of Tremors Dynamics: A Data-driven Approximation Using Koopman Operator Theory.

People who suffer from tremors have difficulty performing activities of daily living. Efforts in developing a model of a limb with tremors can pave the way for non-surgical tremor suppression techniques. However, due to the nonlinearity, developing an accurate model of tremors is challenging. This paper implements a data-driven method for approximating the Koopman operator, which is capable of presenting nonlinear dynamics in a linear framework and is promising for predicting the nonlinear system. A dynamic model of tremors is developed with ultrasound (US) image data collected from a patient

[Classification and diagnosis of immunodeficiency syndromes].

Primary immunodeficiency diseases of the adult are rare disorders, but often lead to serious consequences. Therefore an early diagnosis is critical. The variety in the clinical presentation, the complexity of the immune system and the ongoing discovery of new defects render it a difficult area for the involved physician. Due to the often imprecise complaint of a weak immune system the primary task is the identification of patients with true immunodeficiency. Subsequently, the immune defect needs to be identified in collaboration with a center for immunodeficiency disorders. The diagnostic proc

Intralobar sequestration: radiologic-pathologic correlation.

Intralobar sequestration accounts for 75% of pulmonary sequestrations. The lesion consists of lung tissue that lacks normal communication to the tracheobronchial tree, has systemic arterial supply, and shares the pleura of the parent lobe. The majority of intralobar sequestrations are probably acquired lesions. Patients usually present before the age of 20 years with recurrent infection. At pathologic examination, intralobar sequestration is characterized by inflammation and fibrosis. At radiologic examination, intralobar sequestration typically appears as a consolidation or mass, with or with

[First case of Malassezia globosa isolation in Serbia].

Today is known that genus Malassezia includes seven species: M. furfur, M. sympodialis, M. obtusa, M. globosa, M. restricta, M. sloofflae and M. pachydermatis, but role of each of the species in the pathogenesis of disease has not been elucidated yet, so further laboratory isolation and identification are necessary. We report the first case of isolation of Malassezia globosa in Serbia (Belgrade), in a patient suffering from Pityriasis versicolor. Identification of M. globosa was based on macroscopic, microscopic and biochemical characteristics. Isolation was done on Leeming and Notman medium a

Leptomeningeal metastases.

Leptomeningeal metastases typically have been a rare complication of systemic cancers. The incidence is increasing for cancers of the breast and lung, primarily because of more effective treatment modalities and longer survival. Presentation consists of multiple symptoms related to areas in the central nervous system, including the brain, cranial nerves, and spinal nerve roots. Diagnosis is made by magnetic resonance imaging with contrast enhancement and cerebral spinal fluid pathology. Treatment frequently involves combination therapy with radiation and intrathecal chemotherapy. Outcomes are

[Electromyography in patients with spastic torticollis].

The article describes the results of electromyographic examination of the muscles of the neck in 26 patients with spasmodic torticollis. Electromyography was performed before and after stereotaxic operative interventions on the nucleus of Cajal or in combination with the diencephalic structures. It is shown that electromyographic examination of patients, especially with the use of functional motor and oculomotor tests, widens essentially the possibilities of the diagnosis by facilitating the identification or specification of the predominantly involved group of muscles. The discovery of concur

Cheap and rapid in-house method for direct identification of positive blood cultures by MALDI-TOF MS technology.

BACKGROUND: Rapid and accurate pathogen identification in blood cultures is very important for septic patients and has major consequences on morbidity and mortality rates. In recent years, matrix-assisted laser desorption ionization-time of flight mass spectrometry (MALDI-TOF MS)-based technology has become useful for highly specific and sensitive identification of bacteria and yeasts from clinical samples including sterile body fluids. Additional in-house methods enabled direct identification from blood cultures following various preparation protocols. METHODS: Blood culture (5 ml) was harve

Axillary ultrasound examination is useful for selecting patients optimally suited for sentinel lymph node biopsy after primary systemic chemotherapy.

BACKGROUND: Controversy surrounds the reliability of sentinel lymph node biopsy after primary systemic chemotherapy. In this study, we assessed axillary ultrasound for selecting patients most likely to optimally benefit from biopsy. METHODS: The study included 87 patients who received primary systemic chemotherapy and underwent a sentinel lymph node biopsy followed by axillary lymph node dissection. Lymph nodes >10 mm in diameter, irregularly swollen, round, and homogeneously hypoechoic without an echo-rich center were considered axillary ultrasound positive. RESULTS: In axillary ultrasound-

A comparison of disseminated intravascular coagulation scoring systems and their performance to predict mortality in sepsis patients: A systematic review and meta-analysis.

BACKGROUND: Disseminated intravascular coagulation (DIC) is a common complication in sepsis patients which exacerbates patient outcomes. The prevalence and outcomes of DIC in sepsis is wide-ranging and highly depends on the severity of the disease and diagnostic approaches utilized. Varied diagnostic criteria of DIC have been developed and their performance in diagnosis and prognosis is not consistent. Therefore, this study aimed to determine the score positivity rate and performance of different DIC scoring systems in predicting mortality in sepsis patients. METHODS: Four databases, includin
