An ethnographic exploration of drug markets in Kisumu, Kenya.

BACKGROUND: Illegal drug markets are shaped by multiple forces, including local actors and broader economic, political, social, and criminal justice systems that intertwine to impact health and social wellbeing. Ethnographic analyses that interrogate multiple dimensions of drug markets may offer both applied and theoretical insights into drug use, particularly in developing nations where new markets and local patterns of use traditionally have not been well understood. This paper explores the emergent drug market in Kisumu, western Kenya, where our research team recently documented evidence of

Challenging logics of complex intervention trials: community perspectives of a health care improvement intervention in rural Uganda.

Health systems in many African countries are failing to provide populations with access to good quality health care. Morbidity and mortality from curable diseases such as malaria remain high. The PRIME trial in Tororo, rural Uganda, designed and tested an intervention to improve care at health centres, with the aim of reducing ill-health due to malaria in surrounding communities. This paper presents the impact and context of this trial from the perspective of community members in the study area. Fieldwork was carried out for a year from the start of the intervention in June 2011, and involved

Examining localized patterns of air quality perception in Texas: a spatial and statistical analysis.

Environmental and human health issues associated with outdoor air pollution, such as ozone, sulfur dioxide, and other pollutants in metropolitan regions, are an area of growing concern for both policy officials and the general public. Increasing attention from the news media, new health data, and public debate over the effectiveness of clean air regulations have raised the importance of air quality in the public consciousness. While public perceptions of air quality have been studied thoroughly dating back to the 1960s, little empirical research has been conducted to explain the spatial aspect

The Challenges of Transition From Donor-Funded Programs: Results From a Theory-Driven Multi-Country Comparative Case Study of Programs in Eastern Europe and Central Asia Supported by the Global Fund.

INTRODUCTION: In the era of declining development assistance for health, transitioning externally funded programs to governments becomes a priority for donors. However, the process requires a careful approach not only to preserve the public health gains that have already been achieved but also to expand on them. In the Eastern Europe and Central Asia region, countries are expected to graduate from support from the Global Fund to Fight AIDS, Tuberculosis and Malaria in or before 2025. We aim to describe transition risks and identify possible means to address them. METHODS: Using a theory-based

Bridging the Access Gap for Comprehensive Sickle Cell Disease Management Across Sub-Saharan Africa: Learnings for Other Global Health Interventions?

BACKGROUND: Sickle cell disease (SCD) is a major unresolved global health issue, with the highest disease burden in sub-Saharan African countries; yet, SCD care has not proportionally reached patients in these regions, and the disease has received limited attention in the past. Addressing the burden of SCD in sub-Saharan Africa requires a holistic, collaborative approach to ensure solutions are both comprehensive - i.e., cover the entire continuum of care from early diagnosis to treatment - and sustainable - i.e., are co-created and co-owned with local partners and integrated into existing loc

Global Spread of Hemorrhagic Fever Viruses: Predicting Pandemics.

As successive epidemics have swept the world, the scientific community has quickly learned from them about the emergence and transmission of communicable diseases. Epidemics usually occur when health systems are unprepared. During an unexpected epidemic, health authorities engage in damage control, fear drives action, and the desire to understand the threat is greatest. As humanity recovers, policy-makers seek scientific expertise to improve their "preparedness" to face future events.Global spread of disease is exemplified by the spread of yellow fever from Africa to the Americas, by the sprea

PRECISION MEDICINE - The Golden Gate for Detection, Treatment and Prevention of Alzheimer's Disease.

During this decade, breakthrough conceptual shifts have commenced to emerge in the field of Alzheimer's disease (AD) recognizing risk factors and the non-linear dynamic continuum of complex pathophysiologies amongst a wide dimensional spectrum of multi-factorial brain proteinopathies/neurodegenerative diseases. As is the case in most fields of medicine, substantial advancements in detecting, treating and preventing AD will likely evolve from the generation and implementation of a systematic precision medicine strategy. This approach will likely be based on the success found from more advanced

Inflation, Race, and Legislation-The Erosion in the Real Value of Monetary Compensation for Miners' Occupational Lung Disease in South Africa, 1973-2024.

BACKGROUND: For much of the 20th century, the South African mining industry had a statutory compensation system for pneumoconiosis and tuberculosis characterized by gross racial inequality. This study examines the impact of inflation over the period 1973-2024 on the real value of miners' lung disease compensation, including the effect of the dropping of formal racial discrimination after 1993. METHODS: Sources of information included legislation, government reports, notices, and Gazettes, and mining industry reports. RESULTS: From 1973 to 1993, high rates of inflation hollowed out the value

The Road to Elimination of Mother-to-Child Transmission of Syphilis in Malawi: A Mixed-Methods Analysis of Health System Readiness.

BACKGROUND: This study evaluated health system readiness in Malawi for elimination of mother-to-child transmission (EMTCT) of syphilis against World Health Organization (WHO) validation criteria by identifying key health system bottlenecks. It also developed a replicable toolkit to assess bottlenecks to syphilis EMTCT in other countries. METHODS: A mixed-methods design integrated quantitative and qualitative data. A National Clinic Readiness Survey assessed coverage indicators for prevention of maternal and congenital syphilis at 58 health centers. Exit surveys included 9513 mothers at antena

The Use of Classic Hallucinogens/Psychedelics in a Therapeutic Context: Healthcare Policy Opportunities and Challenges.

Psychedelics or serotonergic hallucinogens are a group of substances that share the agonism of serotonergic 5-HT2A receptors as their main mechanism of action. Its main effects include changes in perception, cognitive process, and mood. Despite being used for centuries by different cultures in ritual contexts, these substances have currently aroused the interest of science and industry for their promising antidepressant, anxiolytic, and anti-addictive effects. Considering this evidence, this article aims to explore some of the possible health policy challenges to integrate these therapeutic to

Cholera in the time of war: implications of weak surveillance in Syria for the WHO's preparedness-a comparison of two monitoring systems.

BACKGROUND: Public health breakdown from the Syrian government's targeting of healthcare systems in politically unsympathetic areas has yielded a resurgence of infectious diseases. Suspected cholera recently reappeared but conflict-related constraints impede laboratory confirmation. Given the government's previous under-reporting of infectious outbreaks and the reliance of the WHO on government reporting, we sought to assess the reliability of current surveillance systems. METHODS: We compared weekly surveillance reports of waterborne diseases from the Syrian government's (WHO-associated) Ear

Setting goals and targets for performance standards within the Swedish health care system.

The development of any health care system towards setting goals and targets and intended outcomes--with national guidelines, a legislative framework, limited resources, consumer influence and competitive forces--makes great demands on the control mechanisms required. The Swedish health care system has no tradition of goal formulation of this type. Hence, the purpose of this article is to clarify the goal-setting process of performance standards, and to examine whether goal setting is a relevant method within the organization of a Swedish county council. Goal setting can be seen partly as a con

How Stable Are Individual Preferences for Health Expenditure in Germany?

BACKGROUND/OBJECTIVES: This study analyzed the stability of individual preferences for the allocation of expenditure in the healthcare system using an experimental setting. Understanding these preferences can support policy decisions aimed at achieving a more needs-based allocation of scarce resources in healthcare systems. Stability in preferences might be essential in order to avoid frequent legislative changes and can potentially enhance public satisfaction with the healthcare system. METHODS: Individual preferences were assessed through two questionnaire-based experimental studies conduct

Reframing Efficiency: How Public Deliberation Strengthens Public Health Policy.

With the efforts to downsize US public health infrastructure, a national conversation has emerged about how to use government resources efficiently while maintaining a robust public health system. Drawing on ethical frameworks, we describe 5 types of efficiency important for public health policy. We argue that well-designed public deliberation can enhance these dimensions of efficiency by improving the quality and contextual fit of policies, rebuilding shared factual baselines in a polarized environment, and increasing trust and adherence to policy outcomes. Through case studies from US influe

From PHEIC to PHECs: reclaiming Africa's agency in global health security governance.

In the wake of COVID-19 pandemic, the African Union elevated the Africa Centres for Disease Control and Prevention (Africa CDC) to autonomous status, empowering it to declare Public Health Emergencies of Continental Concern (PHECs). This mechanism was first operationalized in 2024 in response to sustained mpox transmission across multiple African countries, despite the World Health Organization's (WHO) earlier lifting of the Public Health Emergency of International Concern (PHEIC). This article examines the PHECs as a decolonial intervention in global health governance. Applying the Critique,

This article aims to highlight the scope of community-led interventions in LGBTQI+ mental health promotion, and to emphasize the importance of analyzing tensions between community health and public health policies. Sexual and gender minorities represent populations that are particularly vulnerable to psychological distress, while simultaneously being hesitant to seek out healthcare services due to fears of stigma or inadequate support. Faced with this reality, LGBTQI+ communities have historically come together to develop community-led peer support tools tailored to their specific needs, despi

Dengue fever in Bangladesh: rising trends, contributing factors, and public health implications.

Dengue fever has emerged as a major public health crisis in Bangladesh, with an unprecedented surge in cases and fatalities in recent years. This paper analyzed the epidemiological trends, contributing factors, and public health implications of the rising dengue burden in the country. Surveillance data revealed a staggering 203,406 dengue cases and 989 deaths between January and September 2023, marking a 1.9-fold increase compared with the entire year of 2019. The capital bears the brunt of the outbreak and accounts for over half of all cases and deaths. Climatic factors, rapid urbanization, p

[Bacterial zoonoses of public health importance in Germany-incidence, distribution, and modes of transmission].

Bacterial zoonotic pathogens are often the cause of diseases, sometimes with severe outcomes. They are mutually transferable between animals (both wild and domestic) and humans. The transmission paths are very variable and include oral intake via food, respiratory infection via droplets and aerosols, or infections via vectors such as tick bites or rodent contact. Furthermore, the emergence and spread of antibiotic-resistant bacterial pathogens is of paramount public health concern. The likelihood of further spread is influenced by various factors. These include the increase in international tra

Navigating trans visibilities, trauma and trust in a new cervical screening clinic.

Trans and gender diverse people are globally recognised as being under-served in clinical services, with significant implications for their health. During a national reorientation of the Australian cervical screening programme - from Papanicolaou smears to human papillomavirus screening - we conducted interviews with 12 key informants in cancer policy, sexual and reproductive health and trans health advocacy to understand how trans people's needs and experiences were being accounted for and addressed in health policy and practice. Themes captured the complexities of increasing visibility for t
