Infant Botulism and Raised Intraocular Pressure.

Infant botulism is an exceedingly rare disease. Because confirmatory laboratory testing is not available for several days after time of presentation, infant botulism remains a clinical diagnosis. The authors demonstrate how raised intraocular pressure may provide an additional clinical clue to making the diagnosis.

Reliable time to estimate subglottal pressure.

Measuring subglottal pressure (P(s)) with complete interruption can be problematic due to unsteady plateaus in supraglottal pressure data traces during balloon valve interruption. Subjectively determining when the graph plateaus neglect the effects of laryngeal, auditory, and other physical reflexes may alter patient effort and glottal configuration. If the P(s) estimation was made at a consistent time before the onset of reflexes, the recorded pressure would not be dependent on subjective analysis by a clinician, and intrasubject data would be more precise. Previously collected data using the…

Pressure ulcers: prevention, evaluation, and management.

A pressure ulcer is a localized injury to the skin or underlying tissue, usually over a bony prominence, as a result of unrelieved pressure. Predisposing factors are classified as intrinsic (e.g., limited mobility, poor nutrition, comorbidities, aging skin) or extrinsic (e.g., pressure, friction, shear, moisture). Prevention includes identifying at-risk persons and implementing specific prevention measures, such as following a patient repositioning schedule; keeping the head of the bed at the lowest safe elevation to prevent shear; using pressure-reducing surfaces; and assessing nutrition and …

Intraabdominal pressure monitoring during fluid resuscitation.

PURPOSE OF REVIEW: Elevated intraabdominal pressure is commonly encountered in the critically ill, has detrimental effects on all organ systems, and is associated with significant morbidity and mortality. Serial intraabdominal pressure measurements are essential to the diagnosis, management, and fluid resuscitation of patients who develop intraabdominal hypertension and/or abdominal compartment syndrome. RECENT FINDINGS: Over the past year, the importance of intraabdominal pressure measurements and their accurate determination has been further defined. Several nonoperative therapies have been…

Continuous positive airway pressure and surfactant.

Nasal continuous positive airway pressure (nCPAP) is an effective treatment of respiratory distress syndrome. Due to long-standing experience of early nCPAP as the primary respiratory support option in preterm infants, this approach is sometimes labeled 'the Scandinavian Model'. Mechanical ventilation is potentially harmful to the immature lungs and cohort studies have demonstrated that centers using more CPAP and less mechanical ventilation have reduced rates of bronchopulmonary dysplasia. However, there is a lack of evidence in the form of larger, randomized controlled trials to prove the su…

Intraocular pressure and central corneal thickness.

From the results of the Ocular Hypertension Treatment Study emerged the conclusion that ocular hypertensive subjects with thinner central corneal thickness (CCT) are at increased risk of developing glaucoma. Although possible underlying biases that could have led to this conclusion are still under investigation, there is an increasing interest in the scientific community to understand the potential mechanisms of this increased risk profile. It has been proposed that interindividual differences in CCT might be purely responsible for inaccuracies of the tonometric readings with potential underes

High-pressure inactivation of dried microorganisms.

Dried microorganisms are particularly resistant to high hydrostatic pressure effects. In this study, the survival of Saccharomyces cerevisiae was studied under pressure applied in different ways. Original processes and devices were purposely developed in our laboratory for long-term pressurization. Dried and wet yeast powders were submitted to high-pressure treatments (100-150 MPa for 24-144 h at 25 degrees C) through liquid media or inert gas. These powders were also pressurized after being vacuum-packed. In the case of wet yeasts, the pressurization procedure had little influence on the inac…

Raised intracranial pressure in Apert syndrome.

BACKGROUND: Raised intracranial pressure is a well-known complication of Apert syndrome. The current policy in the authors' unit is to monitor these patients and only perform surgery when raised intracranial pressure has been diagnosed. The authors present their experience with this protocol, as it allows a more accurate picture of the natural history of raised intracranial pressure in Apert syndrome. METHODS: The records of 24 patients, aged between 7 and 14 years, with Apert syndrome who had been managed expectantly (i.e., with no routine "automatic" early surgery) were reviewed. Data were …

Estimating subglottal pressure via airflow redirection.

Subglottal pressure (SGP) is a valuable parameter in the research and clinical assessment of laryngeal function. The lungs serve as a constant pressure source during sustained phonation, and that pressure, SGP, can be used to determine the efficiency with which the larynx converts aerodynamic power to acoustic power. As the larynx serves as an aerodynamic transducer, the vocal efficiency (Ve) coefficient, defined as acoustic power (dB) divided by aerodynamic power (SGP x glottal airflow) has been shown to reliably reflect vocal health. However, current SGP measurement techniques are hesitantly…

Animal models in pressure ulcer research.

BACKGROUND/OBJECTIVE: Research targeting the pathophysiology, prevention, and treatment of pressure ulcers (PrUs) continue to be a significant priority for clinical and basic science research. Spinal cord injury patients particularly benefit from PrU research, because the prevalence of chronic wounds in this category is increasing despite standardized medical care. Because of practical, ethical, and safety considerations, PrUs in the human environment are limited to studies involving patients with pre-existing ulcers. Therefore, we are limited in our basic knowledge pertaining to the developme…

Difficult ventilation from increased abdominal pressure.

Airway management is one of the most important responsibilities of every anesthesiologist. A problematic situation arises when a patient is intubated correctly but not being ventilated. A case of a 4-year-old child who was involved in a car accident, presented for an exploratory laparotomy. Although she was successfully intubated, she could not be ventilated. The situation only improved after a large volume of fluid (urine) was drained from the abdomen.

[Clinical implications of central blood pressure].

The diagnosis and the treatment of hypertension are based on blood pressure readings traditionally taken at the level of the brachial artery. It is currently possible to determine central blood pressure using the aplanation tonometry technique and applying a transfer function to calculate the impact of reflecting waves on pressure prevailing in thoracic aorta. The reflected waves which reach the thoracic aorta in systole are more marked when the stiffness of the arterial wall is increased, leading to augmentation of central systolic blood pressure. Recent clinical trials have shown that, for t…

Walking with continuous positive airway pressure.

A ventilator-dependent child had been in the paediatric intensive care unit (PICU) ever since birth. As a result, she had fallen behind considerably in her development. After 18 months, continuous positive airway pressure was successfully administered via a tracheostomy tube with a novel lightweight device. This enabled her to walk in the PICU. With this device, the child was discharged home where she could walk with an action range of 10 m. Subsequently, her psychomotor development improved remarkably. To the authors' knowledge, this is the first case report of a patient, adult or paediatric,…

Acute infections, infection pressure, and atopy.

BACKGROUND: During the recent years, a new theory postulating that lack of early childhood infections would increase the prevalence rate of allergies has rapidly gained momentum. This hygiene hypothesis has been widely disseminated to the general public and it has been suggested that vaccinations would accordingly indirectly increase rates of atopy. We thus investigated associations between acute infections, infection pressure (i.e. number of daily child contacts) and atopy in one of the largest population-based medical surveys ever published in the medical literature. METHODS: Almost all Fin…

Blood pressure guidelines and screening techniques.

Recording and monitoring blood pressure should be a lifetime practice. Studies have shown that people with normal blood pressure readings between the ages of 55 and 65 still have an 80 to 90 percent risk of developing hypertension by the age of 80. The information in this course teaches the dental health professional to be aware of the causes of hypertension and how to determine and assess identifiable causes. Understanding the terms systolic, diastolic and pulse pressure along with knowledge of the various blood pressure categories and the factors that influence blood pressure is important. T…
