Diagnosis and Management of Osteoporosis.

Osteoporosis-related fractures affect approximately one in two white women and one in five white men in their lifetime. The impact of fractures includes loss of function, significant costs, and increased mortality. The U.S. Preventive Services Task Force recommends using dual energy x-ray absorptiometry to screen all women 65 years and older, and younger women who have an increased fracture risk as determined by the World Health Organization's FRAX Fracture Risk Assessment Tool. Although guidelines are lacking for rescreening women who have normal bone mineral density on initial screening, int

Post-Up Study: Postpartum Depression Screening in Well-Child Care and Maternal Outcomes.

OBJECTIVES: Postpartum depression often remains unaddressed. Screening in well-child care (WCC) may improve early detection, promote maternal recovery, and reduce effects on child development. We assessed the effectiveness of screening for postpartum depression in WCC compared with care as usual (CAU) on outcomes at mother and child levels. METHODS: In a prospective, quasiexperimental, comparative design, mothers visiting Dutch WCC centers were exposed either to screening at 1, 3, and 6 months postpartum (n = 1843) or to CAU (n = 1246). Assessments were at 3 weeks (baseline), 9 months (the Mi

Oral Health in Somalia: Unmasking a Neglected Dimension of Public Health.

Oral health is a fundamental but neglected component of public health, with important consequences for nutrition, communication, dignity, quality of life, and noncommunicable disease (NCD) prevention. In Somalia, oral health remains poorly integrated into health policy and service delivery within a context shaped by health-system fragility, displacement, humanitarian pressures, and limited workforce capacity. This perspective synthesizes available Somalia-specific evidence alongside modeled national estimates and relevant regional evidence to examine the burden, determinants, access barriers,

Provider Discussions of Genetic Tests With U.S. Women at Risk for a BRCA Mutation.

INTRODUCTION: The U.S. Preventive Services Task Force recommends that primary care providers screen unaffected women with a family history of BRCA mutation-associated cancers, but without a personal history of BRCA-related cancer, for referral to genetic counseling and potential genetic testing. METHODS: The 2015 National Health Interview Survey was analyzed in January 2017 to determine the rates at which unaffected adult women with a positive family history of BRCA-related cancers, assessed using the Family History Screen-7, reported discussing genetic testing with a provider, using genetic

Access to syringes in three Russian cities: implications for syringe distribution and coverage.

BACKGROUND: We report findings from a multi-method study investigating drug injectors' access to needles and syringes in three large Russian cities (Moscow, Volgograd, Barnaul). METHODS: We undertook 209 qualitative interviews among drug injectors, and supplemented these with baseline data from a community-recruited survey of 1473 drug injectors. FINDINGS: Almost all (93%; 1277) injectors used pharmacies as their main source of clean injecting equipment, and only 7% (105) reported ever having had contact with city syringe exchange projects. Good access to syringes has coincided with the expa

The influence of gender on the doctor-patient interaction.

OBJECTIVE: This paper discusses the research focused on gender issues in healthcare communication. METHODS: The majority of papers discussed here are based on a research study in which 509 new adult patients were prospectively and randomly assigned to family practice or internal medicine clinics at a university medical center and followed for one year of care. RESULTS: There are significant differences in the practice style behaviors of female and male doctors. Female doctors provide more preventive services and psychosocial counseling; male doctors spend more time on technical practice beha

Missed Opportunities for Chlamydia Screening in Title X Family Planning Clinics.

BACKGROUND: Annual chlamydia (CT) screening is recommended for women younger than 25 years, yet less than half of young women seeking health care are screened annually. We analyzed Title X family planning service data from the Northwest United States to assess factors associated with missed opportunities for CT screening. Our primary hypothesis was screening coverage is higher during annual preventive health visits compared to other visit types. Study objectives were: (1) identify gaps in screening coverage by patient demographics, visit characteristics, and clinic measures; and (2) examine th

Good practice in reaching and treating refugees in addiction care in Germany - a Delphi study.

BACKGROUND: Health and adequate access to health care are human rights. Refugees are at risk for substance abuse. Despite the known structural and personal risk factors for abuse, refugees in Germany continue to face barriers to adequate addiction prevention and care, which is a violation of the fundamental human right to health care. The question arises as to how barriers for refugees in reaching addiction services and care can be overcome. In the presented study, strategies for good practices to deconstruct these barriers were identified. METHOD: A total of 21 experts participated in a thre

Are obese women more likely to participate in a mobile mammography program?

Mobile mammography services are typically offered as a means to increase access and adherence to mammography screenings. As mobile mammography becomes a viable strategy to increase screening, a 3 year study of such a state-wide program in WV found surprisingly high rates of obesity within the study population. Thus, the objectives were to: (1) describe the demographic characteristics and comorbidities of women who utilized the WV program, and (2) determine the association between body mass index (BMI) and personal health and screening history, preventive care and wellness behaviors, nutrition

Survival, trust, and compassion: persistent HIV prevention concerns faced by Black sexual and gender minority communities in the US South - a qualitative study.

BACKGROUND: Members of Black sexual and gender minority (BSGM) groups in the Southern US experience disparities across all pillars of the HIV prevention continuum. Social network strategy (SNS) is an intervention that trains people with reasons to test for HIV to reach out to peers to increase HIV testing and may improve uptake of pre-exposure prophylaxis (PrEP). We conducted formative research to design an enhanced SNS to implement within partner services. METHODS: In 2022-2024, we conducted four focus groups among HIV public health services staff and 12 in-depth interviews with local health

OBJECTIVE: Describe the rationale and methodology of the Rapid Assessment of Avoidable Blindness applied in surveys at the national level in 2011-2013 in Argentina, El Salvador, Honduras, Panama, Peru, and Uruguay. METHODS: The survey includes individuals aged 50 years and older, minimizing required sample sizes, which vary from 2 000 to 5 000 people. It uses straightforward sampling and examination techniques, and data analysis is automatic and does not require a statistician. It is relatively inexpensive, as it does not take a long time, does not require expensive ophthalmic equipment, and

Alcohol and drug misuse prevention programs for American Indians: needs and opportunities.

General statistics have indicated that the problem of alcohol and drug misuse among American Indians has been in need of attention for years. A specific and critical examination of mortality and morbidity statistics yields a number of valuable insights to the ways of addressing the problem. The current status in many communities dictates intervention at three levels. First, high mortality and morbidity rates must be reduced through creative and innovative intervention with the social and physical environment. Alcohol legalization and other issues are discussed as distinct possibilities. Second

"There is no free here, you have to pay": actual and perceived costs as barriers to intermittent preventive treatment of malaria in pregnancy in Mali.

BACKGROUND: "There is no free here," the words of a Malian husband, illustrate how perceptions of cost can deter uptake of intermittent preventive treatment of malaria in pregnancy (IPTp). The Malian Ministry of Health (MOH) recommends a minimum of three doses of IPTp at monthly intervals. However, despite a national policy that IPTp be provided free of charge, only 35% of pregnant women receive at least one dose and less than 20% receive two or more doses. METHODS: This study explored perceptions and experiences of IPTp cost in Mali and their impact on uptake, using qualitative interviews an

Improving Access to and Quality of Sexual and Reproductive Health Services for Adolescents in the United States.

Equitable access to high quality adolescent sexual and reproductive health (ASRH) services can help reduce unintended pregnancies, sexually transmitted diseases, and disparities in these outcomes. The Centers for Disease Control and Prevention (CDC), Division of Reproductive Health, has a long history of working to improve access to and quality of ASRH services through applied research and public health practice. This report from CDC summarizes the evolution of these efforts from more than a decade of work-from community-based demonstration projects to an initiative to support wide-scale imple

Pertussis surveillance and control: exploring variations and delays in testing, laboratory diagnostics and public health service notifications, the Netherlands, 2010 to 2013.

Pertussis is most severe among unvaccinated infants (< 1 year of age), and still leads to several reported deaths in the Netherlands every year. In order to avoid pertussis-related infant morbidity and mortality, pertussis surveillance data are used to guide pertussis control measures. However, more insight into the accuracy of pertussis surveillance and control, and into the range of healthcare and public health-related factors that impede this are needed. We analysed a unique combination of data sources from one Dutch region of 1.1 million residents, including data from laboratory databases

Sexual behavior and urinary tract infection.

A study to identify factors which put young, sexually active women at risk to develop a urinary tract infection (UTI) was done. Reported here are differences in sexual behaviors in the study and control groups. All subjects were currently sexually active. Study subjects had a urine culture of greater than 10(5) single pathogen species. Control subjects were healthy women seeking preventive health care services. Both groups completed a self-administered questionnaire consisting of demographic data, past personal and family history, health status, habits of daily living and sexual activity and b

Differences in the Health Status of Older Adults in Community and Hospital Cohorts.

BACKGROUND: Older adults frequently utilize healthcare services across diverse medical settings to maintain continuity of care. However, data from the hospital records of older adults is often not linked to their health status in community settings, preventing a full understanding of their healthcare needs. METHODS: This study aimed to compare the multidimensional health status (i.e., self-rated health, depression, physical function/activity, mobility, nutrition, lifestyle factors, blood laboratory, and diseases) of older adults between community and hospital cohorts. The goal was to identify
