Safety of cyanoacrylate-based adhesives in creating layered septal cartilage grafts during open structure septorhinoplasty.

PURPOSE: Patients having suffered severe nasal trauma or having undergone prior septal surgery present particular problems during open structure septorhinoplasty. Septal cartilage deficient noses often require costal or conchal cartilage grafting or the use of allograft material, resulting in secondary donor site morbidity, longer operative times and variable results. The present study evaluated the utility of cyanoacrylate-based adhesives (CBA) in creating layered septal cartilage grafts from remnant septal cartilage for L-strut reconstruction during open septorhinoplasty. STUDY DESIGN: A re

A comparison of female versus male insanity acquittees in Colorado.

This study was undertaken to investigate the authors' clinical impression that there are significant differences between the male and female insanity acquittees in Colorado, and that these differences result in significantly different treatment needs. The study sample included 149 patients: 112 men and 37 women committed to the Colorado Mental Health Institute at Pueblo as not guilty by reason of insanity (NGRI). Data were collected from a computerized data system and from chart reviews. The study provides descriptive data regarding demographic, legal, and mental health parameters of these acq

Health impacts with telework on workers: A scoping review before the COVID-19 pandemic.

BACKGROUND: Telework has dramatically increased due to the coronavirus disease 2019 (COVID-19) pandemic, and the health impacts related to telework have become major concerns. Some studies have shown that telework has both positive and negative impacts. However, during the pandemic, the influence of COVID-19 is too strong to estimate the health effects of telework. Therefore, this scoping review investigated a comprehensive overview of those impacts based on studies conducted before the COVID-19 pandemic. METHODS: We searched keywords related to telework in five databases: PubMed, Scopus, Cum

Sacroiliac joint pain in the pediatric population.

OBJECT: The sacroiliac (SI) joint can be a pain generator in 13%-27% of cases of back pain in adults. These numbers are largely unknown for the pediatric population. In children and especially girls, development of the pelvic girdle makes the SI joint prone to misalignment. Young athletes sustain repeated stress on their SI joints, and sometimes even minor trauma can result in lasting pain that mimics radiculopathy. The authors present a series of 48 pediatric patients who were evaluated for low-back pain and were found to have SI joint misalignment as the cause of their symptoms. They were tr

Advancing Our Understanding of Hidradenitis Suppurativa: A Comprehensive Guide for Clinicians Designed to Improve Therapeutic Outcomes for Patients-Part 2.

Hidradenitis suppurativa (HS) is a chronic inflammatory cutaneous disorder that often starts in the second or third decade of life, induces both inflammatory and cicatricial cutaneous lesions, and frequently involves intertriginous and anogenital anatomic sites. Abscesses, nodules, and draining tunnels are reversible inflammatory lesions that cycle over time through unpredictable periods of exacerbation; cicatricial lesions are permanent and persist as fibrotic scars and nondraining tunnels, the latter reflecting the shells of prior draining tunnels. HS has a profound adverse impact on overall

Are Imaging Evaluations of Soft-Tissue Masses Before Referral to a Specialized Center Being Performed Properly? A Systematic Review.

BACKGROUND/OBJECTIVES: Since the initial approach towards the clinical presentation of soft-tissue masses is challenging for frontline physicians, some countries use clinical practice guidelines. Proper imaging work-up is crucial to differentiate between soft-tissue tumors. Recently, ultrasonography and magnetic resonance imaging (MRI) have been widely used. But there is some controversy about whether pre-referral evaluations are being conducted properly. Thus, this study aims to assess whether the ultrasonography investigation of soft-tissue masses prior to referral to the musculoskeletal tum

Parietal-Prefrontal Feedforward Connectivity in Association With Schizophrenia Genetic Risk and Delusions.

OBJECTIVE: Conceptualizations of delusion formation implicate deficits in feedforward information updating across the posterior to prefrontal cortices, resulting in dysfunctional integration of new information about contexts in working memory and, ultimately, failure to update overfamiliar prior beliefs. The authors used functional MRI and machine learning models to address individual variability in feedforward parietal-prefrontal information updating in patients with schizophrenia. They examined relationships between feedforward connectivity, and delusional thinking and polygenic risk for sch

Gemcitabine and bendamustine is a safe and effective salvage regimen for patients with recurrent/refractory Hodgkin lymphoma: Results of a phase 1/2 study.

BACKGROUND: Both gemcitabine and bendamustine have been evaluated in patients with recurrent/refractory Hodgkin lymphoma but to the authors' knowledge not as a doublet. The authors completed a phase 1/2 trial to identify the optimal dose and frequency of administration and to assess the efficacy of this combination in patients with recurrent/refractory Hodgkin lymphoma. METHODS: Patients were treated up to a maximum dose of gemcitabine (1000 mg/m2 on day 1) and bendamustine (120 mg/m2 on days 1 and 2), which was determined to be the recommended phase 2 dose, administered every 21 days for up

Conflicting Data on the Incidence of Leukopenia and Neutropenia After Heated Intraperitoneal Chemotherapy with Mitomycin C.

BACKGROUND: During heated intraperitoneal chemotherapy (HIPEC), neutropenia rates of 20 to 40% have been reported when mitomycin C (MMC) is dosed by weight or body surface area (BSA). This study investigated the authors' HIPEC experience using a fixed 40-mg dose of MMC, per consensus guidelines, and analyzed predictors for severe leukopenia and neutropenia. METHODS: Patients who underwent MMC-HIPEC from 2007 to 2016 at a single tertiary care center were retrospectively reviewed. RESULTS: Among 314 MMC-HIPEC cases, 72 patients in the early era of the authors' program received routine prophyla

Subdural hematoma as a major determinant of short-term outcomes in traumatic brain injury.

OBJECTIVE Early radiographic findings in patients with traumatic brain injury (TBI) have been studied in hopes of better predicting injury severity and outcome. However, prior attempts have generally not considered the various types of intracranial hemorrhage in isolation and have typically not excluded patients with potentially confounding extracranial injuries. Therefore, the authors examined the associations of various radiographic findings with short-term outcome to assess the potential utility of these findings in future prognostic models. METHODS The authors retrospectively identified 17

Perioperative seizure incidence and risk factors in 223 pediatric brain tumor patients without prior seizures.

OBJECT: The incidence of, and risk factors for, perioperative seizures and the need for perioperative antiepileptic drugs (AEDs) in previously seizure-free children with brain tumors remains unclear. The authors have undertaken a review of previously seizure-free pediatric patients with brain tumors undergoing resection to identify the incidence of seizures in the perioperative period, and to characterize risk factors for perioperative seizures in this population. METHODS: A retrospective review was conducted of all patients between 0 and 19 years of age without prior seizures who underwent i

Statin discontinuation and risk of acute myocardial infarction in patients with rheumatoid arthritis: a population-based cohort study.

OBJECTIVES: Screening for cardiovascular risk factors and treating hyperlipidaemia with statins are recommended to reduce the increased cardiovascular risk in individuals with rheumatoid arthritis (RA). However, poor compliance with statins may limit their therapeutic benefit. Our objective was to evaluate the impact of statin discontinuation on risk of acute myocardial infarction (AMI) among RA patients. METHODS: The authors conducted a population-based cohort study of RA patients with incident statin use followed from May 1996 to March 2006 using administrative health data. Primary exposure

Optimizing blepharoplasty outcomes in patients with previous laser vision correction.

BACKGROUND: Dry eye syndrome, often referred to as dysfunctional tear syndrome, can occur following laser vision correction surgery and routine blepharoplasty. Identifying patients prone to developing or worsening of dysfunctional tear syndrome following blepharoplasty can help optimize surgical outcomes. METHODS: The authors highlight the salient features of the dysfunctional tear syndrome including key steps in identifying at-risk patients. The authors discuss changes in the cornea that occur with keratorefractive surgery and their significance in patients seeking blepharoplasty. The author

Does Industry Influence Persist? Financial Conflicts and ADM Outcomes After the 2021 FDA Safety Alert.

BACKGROUND: Financial conflicts of interest may be associated with outcome reporting in surgery. In March 2021, the FDA issued a safety communication highlighting complications associated with acellular dermal matrices (ADM) in breast reconstruction. This study assessed whether the association between industry payments and favorable ADM reporting persisted after this regulatory intervention. METHODS: We analyzed clinical ADM studies published from 2020 to 2025 in the four plastic surgery journals used in the prior benchmark study. Conclusions were coded as positive, neutral, or negative. Breakthrough Hemolysis in Paroxysmal Nocturnal Hemoglobinuria: Mechanistic Insights and Management Strategies.

BACKGROUND: Paroxysmal nocturnal hemoglobinuria (PNH) is a clonal hematopoietic stem cell disorder caused by somatic mutations in the PIGA gene, resulting in loss of glycosylphosphatidylinositol (GPI)-anchored proteins, including the complement regulatory proteins, CD55 and CD59. Their absence leads to complement-mediated intravascular hemolysis. Patients may present with anemia, hemoglobinuria, renal injury, and potentially life-threatening thromboses. Complement inhibitors have transformed treatment for PNH. Terminal complement inhibitors reduce intravascular hemolysis and transfusion requir

Right coronary artery takeoff plane and implications for brachiocephalic interventions and next generation transcatheter aortic valve intervention embolic protection devices.

OBJECTIVES: The authors hypothesized that the origin of the right coronary artery (RCA) is a direct continuation of the major aortic arch branches (MAAB) takeoff plane, which may have implications for brachiocephalic interventions and next generation transcatheter aortic valve intervention (TAVI) embolic protection devices (EPDs). METHODS: In this single-center, retrospective, cross-sectional study, the authors analyzed computed tomographic angiography (CTA) images from 92 patients undergoing TAVI evaluation to determine the spatial relationship between the origin of the RCA and the MAAB take

Systematic review of efficacy of hair transplantation in patients with classic lichen planopilaris and frontal fibrosing alopecia: revisiting the current evidence.

BACKGROUND: Lichen planopilaris (LPP) is the most common form of primary cicatricial alopecia. Areas of alopecia are permanent; thus, hair transplantation becomes a potential cosmetic alternative. OBJECTIVE: To review the evidence of the efficacy of hair transplantation in patients with LPP. METHODS: The authors conducted a systematic review across four databases: Web of Science, PubMed, SciELO, and Cochrane. The authors analyzed studies describing the use of hair transplantation in patients with LPP, in any of its subtypes. Articles published from the inception of each database through Octo

Arthroscopic Intra-articular Biceps Tenodesis With All-Suture Anchor.

BACKGROUND: Biceps tenodesis can be used to treat both superior labral anterior and posterior (SLAP) tears and degenerative biceps pathologies in the setting of rotator cuff pathology or as stand-alone techniques. Research has shown that arthroscopic biceps tenodesis has comparable outcomes to open tenodesis with the benefit of being less invasive. INDICATIONS: Surgical indications for intra-articular tenodesis of the long head of the biceps (LHB) include SLAP tear, failed SLAP repair, partial-thickness LHB tear, biceps instability, tenosynovitis, and clinical examination consistent with LHB

Respiratory Health Among U.S. Veterans Across Age and Over Time.

Many veterans and their advocates are concerned that military service may cause impaired respiratory function resulting from occupational exposures to environmental hazards (e.g., Agent Orange in the Vietnam War, burn pits in the Global War on Terror) or infectious diseases (e.g., pneumonia). Such exposures occurring in service members' young adulthoods may not have immediately discernible effects on lung health; some symptoms may emerge in the immediate months or years after separation from the military, and diagnosable conditions may not manifest until decades later. Health care providers se
