Prevalence and Predictors of Chronic Health Conditions of Inmates Newly Admitted to Maximum Security Prisons.

Almost all patients with liver disease, especially advanced liver disease, have some evidence of malnutrition, including mineral/vitamin deficiency. A major health trend in the United States has been the significant growth in the use of complementary and alternative medicine (CAM), including nutrition supplements and herbal agents. In the 1990s, the United States government created the National Center for Complementary and Alternative Medicine (NCCAM), as well as the Office on Dietary Supplements, to extend our knowledge in these areas. CAM users are often highly educated and frequently use CA

[Neurological disorders related to vitamin B12 deficiency in prisons in Guinea: a 22-case study].

Neurological disorders related to vitamin B12 deficiency are common in prisons of tropical Africa. We collected 22 cases (20 men and 2 women). They all showed vitamin B12 deficiency associated with neurological signs that were represented by sclerosis combined with bone marrow (n = 9), peripheral neuropathy (n = 6), cerebellar syndrome (n = 2), a pyramidal syndrome of the lower limbs (n = 4) and optic neuropathy (n = 1). Laboratory tests showed a mean hemoglobin concentration of 7.2 ± 1.5 g/dl, mean 104 ± 28 fl, macrocytic anemia in 10 patients. Biermer's disease was identified in 9 patients, …

A literature review of international implementation of opioid substitution treatment in prisons: equivalence of care?

BACKGROUND/AIMS: Opioid substitution treatment (OST) is an effective treatment for heroin dependence. The World Health Organization has recommended that OST be implemented in prisons because of its role in reducing drug injection and associated problems such as HIV transmission. The aim of this paper was to examine the extent to which OST has been implemented in prisons internationally. METHODS: Literature review. RESULTS: As of January 2008, OST had been implemented in prisons in at least 29 countries or territories. For 20 of those countries, the proportion of all prisoners in OST could be…

How do people in prison access palliative care? A scoping review of models of palliative care delivery for people in prison in high-income countries.

BACKGROUND: An ageing prison population with complex health needs combined with punitive sentencing practices means palliative care for incarcerated individuals is increasingly important. However, there is limited evidence regarding the models of care delivery in high-income countries, and their associated challenges and benefits. AIM: To develop a typology of models of palliative care provision for incarcerated individuals, synthesise evidence of their outcomes and describe facilitators of and challenges in delivering different models of palliative and end-of-life care in prisons. DESIGN: S…

Examining risk factors for HIV infection amongst male inmates in Peruvian prisons: a cross-sectional study using national prisoner census data.

INTRODUCTION: The prison population in Peru exceeds 90,000 individuals, many of whom face conditions that increase their vulnerability to human immunodeficiency virus (HIV), such as exposure to illicit substances, risky sexual behaviors, and discrimination. MATERIALS AND METHODS: A secondary analysis was carried out, using data from the 2016 national prison population census to identify factors associated with HIV risk in the incarcerated population of Peru. A total of 75,613 male inmates from prisons across the country participated, answering a comprehensive questionnaire consisting of 405 i…

Prescription drugs with potential for misuse in Irish prisons: analysis of national prison prescribing trends, by gender and history of opioid use disorder, 2012 to 2020.

BACKGROUND: Pharmacotherapy is essential for the delivery of an equivalent standard of care in prison. Prescribing can be challenging due to the complex health needs of prisoners and the risk of misuse of prescription drugs. This study examines prescribing trends for drugs with potential for misuse (opioids, benzodiazepines, Z-drugs, and gabapentinoids) in Irish prisons and whether trends vary by gender and history of opioid use disorder (OUD). METHODS: A repeated cross-sectional study between 2012 and 2020 using electronic prescribing records from the Irish Prison Services, covering all pris…

Longer than prison? A comparison of length of stay in a medium security hospital and prison for perpetrators of violent crimes other than homicide or attempted homicide.

BACKGROUND: Concerns have been raised that people detained in secure hospitals after a criminal act spend longer in a locked institution than people sent to prison for similar offending, but research evidence is scarce and conflicting. AIMS: To compare the length of secure hospitalisation of people convicted of crimes for which flexible sentencing is allowed with length of time in prison for people serving a prison sentence for similar crimes who were not found mentally ill. METHODS: A records-based, retrospective study was conducted comparing length of stay of all inpatients in one medium s

From Smoking-Permitted to Smokefree Prisons: A 3-Year Evaluation of the Changes in Occupational Exposure to Second-Hand Smoke Across a National Prison System.

OBJECTIVES: Prisons in Scotland were one of the few workplaces exempt from the 2006 comprehensive smoking ban in indoor public places, excluding the prison workforce from the health benefits of smokefree workplaces. The November 2018 introduction of comprehensive restrictions on smoking in Scottish prisons aimed to protect prison staff and people in custody from the harmful impacts of second-hand smoke (SHS) exposure. This study presents SHS exposure data gathered after smokefree policy implementation and compares these with data gathered during and before policy development. METHODS: Dylos D…

'Mankind owes to the child the best that it has to give': prison conditions and the health situation and rights of children incarcerated with their mothers in sub-Saharan African prisons.

BACKGROUND: In recent times, sub-Saharan African (SSA) prisons have seen a substantial increase in women prisoners, including those incarcerated with children. METHODS: A scoping review mapped what is currently known about the health situation and unique rights violations of children incarcerated with their mothers in SSA prisons. A systematic search collected and reviewed all available and relevant published and grey literature (2000-2018). Following application of exclusion measures, 64 records remained, which represented 27 of the 49 SSA countries. These records were charted and thematical…

Critical illness in prisons: a multi-methods analysis of reported healthcare safety incidents in England.

BACKGROUND: Prisoners have disproportionately poorer health and complex needs compared with the general population. Prisons should provide care that is equivalent to community care to achieve equitable health outcomes, which includes managing physical deterioration. AIM: To characterise reported patient safety incidents involving critically unwell prisoners and identify opportunities to improve prison healthcare systems. DESIGN AND SETTING: Secondary multi-methods analysis of incident reports submitted from English prisons to the National Reporting and Learning System (NRLS) between 2018 and…

Rates, Timing and Correlates of Suicide in Norwegian Prisons: A 23-Year National Cohort Study.

OBJECTIVE: Suicide is a major public health concern, disproportionately impacting society's most vulnerable, including people in prison. Given the multifactorial etiology of suicide within correctional settings, a comprehensive investigation is critical. We aimed to examine timing of suicide according to imprisonment and associations with sociodemographic, criminological, and clinical factors, in a national prison population over two decades. METHOD: We used cohort data from the Norwegian Prison Release study (nPRIS) including all individuals incarcerated in Norwegian prisons in the period 1.…

Maximising the public health benefits of smokefree prisons: a synopsis from a mixed-methods study.

BACKGROUND: High levels of smoking among people who experience imprisonment contribute to their high mortality and morbidity rates and to inequalities. Scotland's prisons became smokefree in 2018. However, questions remain about how to prevent high relapse to smoking post release. OBJECTIVES: Summarise evidence on supporting people to reduce tobacco-related harms post release. Understand experiences, opportunities and challenges for reducing tobacco-related harms for people leaving smokefree prisons and for families. Feasibility test a household-targeted intervention to support people release…

Social capital and COVID-19 vaccination in prisons: a survey of attitudes, beliefs, and motivations.

BACKGROUND: UK prisons were profoundly affected by the COVID-19 pandemic. Despite this, uptake of the COVID-19 vaccination remained low in these settings. Social roles, relationships, and freedoms influence vaccine uptake. Prisons are isolated by design and may foster mistrust that negatively influences vaccine uptake. Prisons are also unique communities with their own social networks and relationships. METHODS: We undertook a questionnaire survey across all six prisons in Wales, UK, to gather data on vaccine behaviour, attitudes, and other influencing factors. We fitted binomial generalised …

The Prevalence of Narcissistic Vulnerability in Men in English Prisons After Criminal Conviction for Stalking.

BACKGROUND: In earlier research with prisoners, we observed that convicted stalkers had skill deficits in interpreting their experiences of stalking and their motivations for it, suggesting narcissistic vulnerability. AIMS: Our primary aim was to explore the prevalence of narcissistic vulnerability in men serving a prison sentence in England and to investigate differences in narcissistic vulnerability and attachment styles between men convicted of stalking offences and men convicted of other offences but serving similar sentences. METHODS: Participants were from across 16 closed custodial se

[Right to food and health in prisons: managing Ramadan for incarcerated individuals of Islamic faith].

Health status and nutrition are fundamental determinants of overall well-being among incarcerated populations, who constitute a particularly … uting to self-harm and, in severe cases, suicide, which is unfortunately on the rise within Italian prisons. Implementing a more attentive and inclusive food system can therefore improve detainees' health outcomes and promote greater emotional and social stability within correctional facilities.

Risk factors of suicide in prisons: a comprehensive retrospective cohort study in France, 2017-2020.

PURPOSE: Suicide is a leading cause of death in prison and the suicide rates are several times higher in the prison population than in the general population in most countries. Of the studies that have investigated risk factors for suicide in prison, few have controlled for possible confounding factors. The aim of this study is to identify risk factors of suicide among people in French prisons, over a four-year period. METHODS: All incarcerations that occurred in France during 2017-2020 were eligible. Sociodemographic, criminal and prison characteristics were collected for each incarceration …

Health, Access to Care, and Financial Barriers to Care Among People Incarcerated in US Prisons.

IMPORTANCE: Decades-old data indicate that people imprisoned in the US have poor access to health care despite their constitutional right to care. Most prisons impose co-payments for at least some medical visits. No recent national studies have assessed access to care or whether co-pays are associated with worse access. OBJECTIVE: To determine the proportion of people who are incarcerated with health problems or pregnancy who used health services, changes in the prevalence of those conditions since 2004, and the association between their state's standard prison co-payment and care receipt in …
