Toxic shock-like syndrome due to severe hemolytic group A streptococcal infection.

A 33-year-old woman suffering from anal erosions developed severe illness with fever, diarrhea, ischalgia, hypotension, acute abdominal pain, dyspnea, renal and hepatic impairment, myalgia, desquamation of the skin, leukocytosis, anemia, hypocalcemia, decreased serum albumin, and cholesterol levels. Exploratory laparotomy did not reveal pathologic findings. Hemolytic group A streptococci were grown from peritoneal swabs and pleural exudate in bacteriologic cultures. The patient slowly recovered after intense penicillin and tobramycin therapy.

[Surgical treatment of complicated cancer of the rectum].

The authors make an analysis of results of the surgical treatment of 246 patients with rectum carcinoma complicated by partial or complete ileus, pararectal abscess and perforation of the tumor. The data obtained show that primary resection of the tumor especially with a one-step reestablishment of the colon continuity is one of the main prerequisites for the elimination of postoperative complications and lethality.

[Perianal streptococcal dermatitis].

BACKGROUND: Pediatric perianal streptococcal dermatitis (PSD) is a well-defined clinical entity. However, its highly uniform presentation remains surprisingly unrecognized by many practitioners 33 years after its first description. CASE REPORT: A seven-year-old girl had a three-week history of perianal and vulva redness with well-defined margins. Functional symptoms associated perirectal tenderness and pain during defecation, which was responsible for constipation. At onset she also presented with a sore throat, which resolved spontaneously, and she had been complaining for a few days about a

[Pathogenetic treatment of chronic paraproctitis].

Operations were performed on 865 patients for pararectal fistulas. The fistulas occurred after injury in 18 patients, histological examination revealed fistulous form of Crohn's disease in 14 patients, 12 patients had tuberculous paraproctitis. The late-term results of surgical treatment of chronic paraproctitis were studied in 560 patients according to the presence of concomitant hemorrhoids. One-stage excision of the fistula and hemorrhoidectomy was conducted in 329 patients. The fistula recurred in 1.3% of cases. Hemorrhoidectomy was not undertaken in 231 patients; a recurrence of fistula o…

[Surgery of cryptoglandular anorectal fistula and abscess. With special reference to complicated infections].

In an unselected personal consecutive series of anal fistulous abscesses (n = 253) 21 patients (8%) had a complex fistula, i.e. a translevatoric pelvirectal extension of an ischiorectal track (n = 7), a high intersphincteric fistulous abscess with an intramural or a pelvirectal extension (n = 7), a suprasphincteric fistula (n = 2), an intralevatoric fistulous abscess (n = 5). Treatment procedures were an ischiorectal laying open with translevatoric coring out, an intraluminal laying open into the lower rectum, and partial external sphincter division with seton drainage, respectively, as propos

Conditions that mimic inflammatory bowel disease. Diagnostic clues and potential pitfalls.

Many of the features that identify idiopathic inflammatory bowel disease are also found with other colorectal conditions that are often encountered by the primary care physician. Although, initially, symptoms of these disorders may appear to be caused by ulcerative colitis or Crohn's disease, the cause could be bacterial, viral, parasitic, or fungal infection. Ischemic colitis and radiation colitis are other conditions that are similar in presentation to ulcerative colitis. In most cases, the physician should be able to make a differential diagnosis from a thorough history and physical examina

Anal abscess imaged with 99mTc-labeled leukocytes. Case report.

Abscess imaging with leukocyte scintigraphy is described in a case report. Autologous leukocytes were separated from whole blood in a patient with localized inflammation in the perineal region and labeled with 99mTc, an ideal radionuclide for clinical examination with gamma camera. Scintigraphic investigations demonstrated a deep-lying anal abscess. The procedure may prove useful for detection of occult infections and may provide a new diagnostic approach in fever of unknown origin.

[Chronic recurrent paraproctitis].

The authors analyze the problem of prophylaxis of chronic paraproctitis (rectum fistulas). One of its links is the correct understanding of etiology and treatment of recurring pararectal abscesses. This form of inflammation with recurring abscesses without fistulas in the stage of remission is considered to be chronical. The authors propose to name it the chronic recurring paraproctitis. The experience with the treatment of 202 patients showed the usual opening the abscess to be not radical. To prevent further recurrings it is necessary to eliminate the altered crypt and the latent infection i…

[Surgical procedure in combined proctologic diseases].

In 301 (23%) out of 1264 surgical cases with various pathologies of the rectum, pararectal and sacrococcygeal regions some combined diseases were observed. Such combinations as chronic paraproctitis (rectal fistulae) with hemorrhoids and with anal fissure are the most common ones. 176 one-stage operations and 63 double-stage operations were carried out. The average period of hospitalization was 16 days after one-stage operation and 24 days after double-stage operation. The authors believe that timely diagnosing of combined pathology allows to prevent the development of possible complications a…
