Clinical Reasoning: A 63-Year-Old Female Patient Presenting With Orthostatic Hypotension and Ataxia.

A 63-year-old female patient presents with orthostatic hypotension and slowly progressive cerebellar ataxia. Neurologic examination revealed ataxia, pyramidal signs, and vibration/proprioception deficits. Neuroimaging showed diffuse white matter abnormalities, cord atrophy, and corticospinal tract involvement. This case highlights how a careful history, with special attention to tempo of symptoms and family history, combined with examination findings and neuroimaging can be used to reach a rare diagnosis.

Labyrinthectomy in the elderly.

This study involved elderly patients with Meniere's disease with incapacitating vertigo. Several of these patients underwent surgery for relief of symptoms that was tailored to the individual's general health and degree of physical activity. Factors that might affect postoperative rehabilitation and recovery were considered, including vision, vertebrobasilar ischemia, proprioception (such as neuropathy resulting from diabetes), and basal ganglia disease. The postoperative results of this tailored approach have been completely satisfactory.

Comparison of NASM Corrective Exercise Alone Versus with Kinesio Taping on Navicular Drop and Lower Limb Function in Female Adolescents with Flexible Flatfoot: A Randomized Clinical Trial.

Functional flatfoot is a common condition in adolescents that can impair postural control and increase the risk of lower limb injuries, maki … re (MOXFQ). Secondary outcomes included dynamic balance, evaluated by the Y-Balance Test, and ankle proprioception. Results showed that both intervention groups achieved significant improvements in NDT, function, balance, and proprioception compared with the control group (p = 0.001), confirming the effectiveness of NASM corrective exercise. However, no addition …

Motor learning in children with hemiplegic cerebral palsy and the role of sensation in short-term motor training of goal-directed reaching.

AIM: Our aim was to determine if improved upper limb kinematics in children with cerebral palsy (CP) during a reach-to-grasp task could be r … ek for 5 weeks (total 15 h). Evaluations consisted of sensory modalities (tactile threshold, touch, proprioception, stereognosis) and upper limb kinematics during reach-to-grasp of an object located near and far from the body (five assessments: three pre-intervention, immediately post-intervention and 3 mo post-intervention). RESULTS: Motor improvements could b …

Deficits in detection of inversion and eversion movements among subjects with recurrent ankle sprains.

STUDY DESIGN: Observational cross-sectional design. OBJECTIVES: To determine whether a deficit exists in the ability to perceive inversion and eversion movements among subjects with recurrent ankle sprains. BACKGROUND: Although unproven, ankle sprains are thought to recur because proprioception is impaired subsequent to the original sprain. Proprioception has been widely studied, however, the specific property of movement perception has not been rigorously examined for both inversion and eversion movements. METHODS AND MEASURES: The ability to perceive passive inversion and eversion movemen…

Evidence of occipitoatlantoaxial malformations independent of the HOXD3 mutation in an Arabian horse.

Occipitoatlantoaxial malformations (OAAM) are developmental disorders occurring at the occiput, atlas and axis. OAAM type 1 is a familial disorder affecting Arabian horses. Currently, a single genetic mutation in the HOXD3 gene region can be tested to confirm the diagnosis. This case describes a 3-year-old Arabian gelding affected with OAAM. Clinical examination revealed a proximal cervical deformation on the right side and severe stiffness of the neck in the sagittal plane without associated pain. Neurological examination demonstrated a generalized proprioceptive ataxia. Radiographs revealed

Subtalar Arthroereisis for Peroneal Spastic Flatfoot in Adolescents With Accessory Anterolateral Talar Facet Impingement.

This is a case of an 11-year-old boy who experienced pain and peroneal spastic flatfeet due to the accessory anterolateral talar facet impingement (AATFI). Multiple conservative treatments have been used, but with poor results. Finally, he was referred to our department, and the lidocaine injection test in the sinus tarsi area was used to assist the diagnosis. The patient was effectively treated with subtalar arthroereisis. To our knowledge, using subtalar arthroereisis to treat peroneal spastic rigid flatfoot associated with AATFI has not been previously reported. This method can not only pre

Physiotherapy in Bell's Palsy Secondary to Acute Otitis Media: A Case Report.

Acute suppurative otitis media can occasionally result in facial paralysis, which calls for prompt diagnosis and treatment. Facial paralysis, a synonym for Bell's palsy, is a condition that causes rapid weakening of one side of the face muscles, leading to drooping of the face on that side. A major factor in determining the course of the condition is rehabilitation through physiotherapy. Here, we present a case of a 26-year-old female who felt discomfort in her left ear on February 21, 2024, but chose to ignore it then. She observed an abrupt deviation in her mouth and visited a rural hospital

Apraxia after a superior parietal lesion.

We studied a patient who had unilateral (left) limb apraxia associated with a lesion of her right superior (Brodmann areas 5 and 7) and inferior parietal lobes. In monkeys areas 5 and 7 contain "hand-manipulation neurons" that are thought to be important in proprioceptive guidance. Therefore, we tested our patient with her eyes open and closed. Her left hand performance with pantonime, imitation, and use of actual objects dramatically deteriorated in the absence of visual guidance. In addition, although the patient did not have optic ataxia and had normal proprioception, she had difficulty usi…

[Function of proprioceptors of the cervical spine in the cervico-ocular reflex].

Disease of the cervical spine may cause vestibular vertigo. The anatomical basis for this type of cervical vertigo are the proprioceptive stretch receptors of the upper cervical joints. These are connected to the vestibular nuclei, and, thus, to ocular reactions (nystagmus). Quantitative data about this cervico-ocular reflex are still scarce since the reflex is strongly suppressed in healthy persons. However, suppression ceases in bilateral complete vestibular deficiency. We measured the cervico-ocular reflex in 6 patients with complete bilateral labyrinthine deficiency. One patient showed a c

Effect of gaze direction recognition task on pain, rom and functional activities in cervicogenic headache patients.

BACKGROUND: Cervicogenic headache is characterized by unilateral headache potentially stemming from cervical spine mechanical dysfunction. Research indicates that proprioceptive exercises, specifically gaze direction recognition (GDR), are found to be effective in reducing cervical joint position error and enhancing the quality of cervical afferent signals to the central nervous system. PURPOSE: This study aimed to evaluate proprioceptive training's impact on headache pain intensity, functional limitation, and neck motion range in individuals experiencing cervicogenic headache. SUBJECTS AND

Aquatic vs. hopping exercises: A novel comparison of their impacts on motor and sensorimotor rehabilitation in chronic ankle instability.

BACKGROUND: Chronic ankle instability (CAI) is a common condition characterized by recurrent instability, reduced proprioception, and impaired balance, increasing the risk of further injuries. While various rehabilitation strategies exist, evidence comparing the efficacy of aquatic and hopping exercises in improving motor and sensorimotor functions in individuals with CAI remains limited. This study aimed to address this gap. METHODS: In this quasi-experimental study, 53 women with unilateral CAI were allocated to control (n = 18), aquatic exercise (n = 17), or hopping exercise (n = 18) group…

Sensory Dysfunction in ALS and Other Motor Neuron Diseases: Clinical Relevance, Histopathology, Neurophysiology, and Insights from Neuroimaging.

Background: The clinical profiles of MNDs are dominated by inexorable motor decline, but subclinical proprioceptive, nociceptive and somatosensory deficits may also exacerbate mobility, dexterity, and bulbar function. While extra-motor pathology and frontotemporal involvement are widely recognised in motor neuron diseases (MNDs), reports of sensory involvement are conflicting. The potential contribution of sensory deficits to clinical disability is not firmly established and the spectrum of sensory manifestations is poorly characterised. Methods: A systematic review was conducted to examine th

Ketolysis is Required for the Proper Development and Function of the Somatosensory Nervous System.

Ketogenic diets are emerging as protective interventions in preclinical and clinical models of somatosensory nervous system disorders. Additionally, dysregulation of succinyl-CoA 3-oxoacid CoA-transferase 1 (SCOT, encoded by Oxct1 ), the fate-committing enzyme in mitochondrial ketolysis, has recently been described in Friedreich's ataxia and amyotrophic lateral sclerosis. However, the contribution of ketone metabolism in the normal development and function of the somatosensory nervous system remains poorly characterized. We generated sensory neuron-specific, Advillin-Cre knockout of SCOT (Adv-

The effect of methocarbamol and mexiletine on murine muscle spindle function.

INTRODUCTION/AIMS: The muscle relaxant methocarbamol and the antimyotonic drug mexiletine are widely used for the treatment of muscle spasms … study we demonstrate that both muscle relaxants affect muscle spindle function, suggesting impaired proprioception as a potential side effect of both drugs. Moreover, our results provide additional evidence of a peripheral activity of methocarbamol and mexiletine.

The Effect of Electromagnetic Fields on Post-Operative Pain and Locomotor Recovery in Dogs with Acute, Severe Thoracolumbar Intervertebral Disc Extrusion: A Randomized Placebo-Controlled, Prospective Clinical Trial.

Spinal cord injury (SCI) attributed to acute intervertebral disc extrusions (IVDEs) is common in dogs and is treated by surgical decompression. Dogs with sensorimotor complete injuries have an incomplete recovery. Pulsed electromagnetic fields (PEMFs) reduce post-operative pain through anti-inflammatory effects and there is growing evidence for neuroprotective effects. This randomized, controlled clinical trial evaluated the effect of PEMF on post-operative pain and neurological recovery in dogs with surgically treated sensorimotor complete SCI attributed to acute IVDE. Sixteen dogs with surgi
