An update on the safety of biologics for the treatment of psoriasis.

INTRODUCTION: Biologic therapies have changed the management of moderate-to-severe psoriasis by targeting key immunologic pathways. With increasing real-world use, long-term safety profiles are important for clinical decision-making. AREAS COVERED: The objective of this research is to provide an update on the safety of biologics over the past five years in psoriasis, including infection risk, malignancy, immunogenicity, and long-term outcomes. An informal literature review through PubMed was done using key search terms from 2020 to 2025. EXPERT OPINION: Biologic therapies are safe compared t…

Linear psoriasis and Koebner phenomenon: A review.

Linear psoriasis (LP) is an unusual form of psoriasis with an unidentified prevalence and is characterized by psoriatic papules and plaques in a Blaschko linear distribution. Based on clinical features, this disorder is divided into two types: isolated LP and superimposed LP. Previous studies have suggested that LP presenting linear appearances is closely related to the Kobner phenomenon (KP) caused by external provocation, including trauma, skin incision, drugs, infections, and striae distensae. Pathogenesis of LP is mainly attributed to the concept of genetic mosaicism but not completely ill…

Home Phototherapy for Psoriasis: Evidence and Implementation.

INTRODUCTION: Home phototherapy for psoriasis is non-inferior to office-based care, yet adoption remains limited by implementation barriers, inconsistent patient selection, and underrecognized safety gaps. This narrative review synthesizes recent evidence to address these gaps and provide actionable clinical guidance. METHODS: PubMed and Embase were searched for studies published between January 1, 2020, and March 1, 2026, yielding 12 included studies. Study types included randomized controlled trials (RCTs), observational studies, cost analyses, implementation reports, and technical device c…

Ocular complications in psoriasis and psoriatic arthritis.

Ophthalmologic symptoms are reported in 10-67% of psoriasis (Ps) cases and affect up to 31% of patients with psoriatic arthritis (PsA). The most common include blepharitis, eyelid involvement, uveitis, dry eye syndrome, conjunctivitis, corneal changes, and cataract. Despite the relatively common eye involvement, these symptoms are often overlooked or inadequately managed. Therefore, dermatologists and rheumatologists should interview patients for eye-related complaints to effectively identify and treat patients with ocular manifestations. In some cases, direct collaboration with ophthalmologis…

Recent progress on DNA methylation in psoriasis.

Psoriasis is a chronic, recurrent, inflammatory disease that is affected by genetic, immunological, epigenetic, and environmental factors. With the development of biotechnology, research on the pathogenesis of psoriasis has deeply focused on the field of epigenetics, and great progress has been made. Epigenetics is the study of heritable changes in gene expression or cell phenotypes without altering the DNA sequence. DNA methylation (DNAm) alterations are the most common epigenetic phenomena and are widely studied. Many studies have shown that DNAm plays a key role in the pathogenesis of psori…

Psoriasis and atrial fibrillation: Exploring the intersection.

This review explores the emerging connection between psoriasis and atrial fibrillation (AF), focusing on shared inflammatory mechanisms, clinical implications, and research gaps. Psoriasis, characterized by chronic systemic inflammation, has been associated with increased AF risk, driven by elevated pro-inflammatory cytokines such as interleukin (IL)-6, IL-17, and tumor necrosis factor-alpha. These inflammatory mediators contribute to atrial remodeling, fibrosis, and conduction abnormalities, evidenced by prolonged P-wave dispersion and atrial electromechanical delay in psoriasis patients. Sev…

Establishment of Psoriasis Mouse Model by Imiquimod.

Psoriasis is a common, chronic, relapsing, and inflammatory skin condition characterized by thickened skin infiltration, scales, and erythema. The pathogenesis of the disease is highly complex, involving the interplay of various factors such as genetics, immunity, and environmental factors. To investigate the mechanisms underlying psoriasis, researchers have developed multiple animal models of the disease. However, the variability in methodologies has presented challenges for constructing these models consistently. In this study, we established and comprehensively evaluated a highly efficient …

Psychiatric evaluation and personality in psoriasis patients.

OBJECTIVES: Psoriasis is a chronic inflammatory, autoaggressive disease. It is known that psychiatric comorbidity in psoriasis contributes to the progression and exacerbation of the disease. This study aims to examine psychiatric disorders and personality traits in a sample of patients with psoriasis. METHODS: Personal information forms, General Health Questionnaire (GHQ-12), Hospital-Anxiety-Depression Scale (HADS), Five-Factor Personality Inventory Short Form (BFI) were used for all participants. Psoriasis patients were evaluated with clinical information form, Dermatology Life Quality Inde…

Pharmacotherapy of arterial hypertension in patients with psoriasis.

Psoriasis is a chronic systemic autoimmune disease associated with an elevated risk of developing cardiovascular disease. In patients with psoriasis, arterial hypertension treatment requires careful selection of antihypertensive drugs, as some drugs may worsen the skin manifestations of psoriasis. In this review, we summarize the available evidence regarding the risks and benefits of each group of antihypertensive drugs. We also suggest a scheme for optimizing antihypertensive treatment in patients with psoriasis, with emphasis on achieving effective control of blood pressure and cardiovascula…

Psoriasis: an update on topical and systemic therapies.

Psoriasis is an immune-mediated inflammatory disease with a genetic predisposition. Although manifesting predominantly as a hyperproliferative skin disorder, psoriasis is a systemic disease associated with a range of comorbidities including arthritis, cardiovascular disease and depression. Educating the patient that psoriasis is chronic and that there are possible comorbidities is paramount. Patients with a limited number of plaques and no systemic symptoms may be managed with intermittent use of topical therapies. Those with more extensive or debilitating disease may require phototherapy or s…

Apremilast in Psoriasis: Current Landscape and Perspectives.

Psoriasis is an immune-mediated skin disease manifested in more than 3% of Americans and over 125 million people worldwide. The inflammatory skin condition with an increased rate of keratinocyte turnover involves every level of the skin and exhibits various forms of the disease, including plaque, guttate, inverse, pustular, and erythrodermic psoriasis, as well as disease-associated conditions, such as psoriatic arthritis and nail psoriasis. Innovative treatment has highlighted the importance of Apremilast, an oral drug that belongs to the phosphodiesterase- 4 (PDE4) class, which was approved b…

The Role of Visfatin/NAMPT in the Pathogenesis of Psoriasis.

INTRODCUTION: Psoriasis is a complex, chronic, immunologically, inflammatory, and environmentally mediated disease which may affect not only the skin, but also nails, and joints. This dermatosis is known for hyperproliferation, parakeratosis, dysregulated differentiation of keratinocytes, lack of granular layer of the skin and impaired apoptosis of keratinocytes. Methods Fifty patients with psoriasis and twenty-eight healthy individuals were enrolled in the study. Serum samples were collected both from the psoriatic patients and patients with an inguinal hernia, who served as the control group…
