Insight, symptomatic dimensions, and cognition in patients with acute-phase psychosis.

AIMS: This study was designed to evaluate the relationship between insight and the severity of psychotic symptomatology in a sample of patients in an acute phase of psychosis, as well as to analyze the relationship between insight and the symptomatic profile of the patient. In addition, the role of general cognitive abilities in this relationship was explored. METHOD: Cross-sectional observational study of 96 acute psychotic adults. To evaluate psychopathology we used the Positive and Negative Syndrome Scale; for insight, the Scale of Unawareness of Mental Disorder; and for general cognitive

Psychosis in the Deaf Population: Prevalence, Phenomenology, and Diagnostic Challenges: A Narrative Review.

Deaf and hard-of-hearing (DHH) individuals face unique challenges in psychiatric assessment, particularly in the diagnosis and treatment of psychotic disorders. Communication barriers, cultural misunderstandings, and clinician unfamiliarity with deaf language and norms may contribute to misdiagnosis, diagnostic uncertainty, and inadequate care. This narrative review synthesizes current literature on the prevalence, phenomenology, and diagnostic challenges of psychotic disorders in the deaf population, aiming to identify trends, gaps, and future research needs. A comprehensive search was conduc…

Intervention in individuals at ultra-high risk for psychosis: a review and future directions.

OBJECTIVE: Over the last 15 years, a focus on early intervention in psychotic disorders has emerged. Initially, the early psychosis movement focused on timely recognition and phase-specific treatment of first-episode psychosis. However, early psychosis researchers suspected that pushing the point of intervention even further back to the prodromal phase of psychotic disorders may result in even better outcomes. This article reviews intervention research in the ultra-high-risk phase of psychotic disorders. DATA SOURCES: A literature search of intervention trials with ultra-high-risk cohorts pub…

How does drug and alcohol misuse develop among people with psychotic illness? A literature review.

BACKGROUND: There is little evidence contributing to the understanding of why people with schizophrenia and other psychotic illnesses use dr … ices? RESULTS: There is some evidence to support the idea that people with schizophrenia and other psychotic disorders use substances to reduce general dysphoria, and possibly negative symptoms. Social environment and experiences are also likely to be factors in the development of substance misuse in this group, but there is a dearth of empirical evidence. CONCLUSI …

Association between Serotonin-Related Polymorphisms in 5HT2A, TPH1, TPH2 Genes and Bipolar Disorder in Korean Population.

OBJECTIVE: The aim of the present study was to examine the association between serotonin-related gene polymorphisms and bipolar disorder in the Korean population. In addition, we sought to explore the relationship between the clinical characteristics of bipolar patients and serotonin-related gene polymorphisms. METHODS: Inpatients with bipolar disorder (n=103) and control subjects (n=86) were genotyped for 5HT2A 1438A/G, tryptophan hydroxylase 1 (TPH1) 218 A/C, and TPH2 703G/T. We divided patients with bipolar disorder into two groups according to the presence of psychotic symptoms. The sever

Neurocognitive profile in psychotic versus nonpsychotic individuals with 22q11.2 deletion syndrome.

The 22q11.2 deletion syndrome (22q11DS) is associated with increased rates of psychotic disorders and cognitive deficits, but large scale studies are needed to elucidate their interaction. The objective of this two-center study was to identify the neurocognitive phenotype of individuals with 22q11DS and psychotic disorders. We hypothesized that psychotic 22q11DS individuals compared to nonpsychotic deleted individuals would have more severe neurocognitive deficits, especially in executive function and social cognition. These deficits would be present when compared to IQ- matched individuals wi…

Psychotic reactions to daily life stress and dopamine function in people with severe hearing impairment.

BACKGROUND: Minor stresses measured in daily life have repeatedly been associated with increased momentary psychotic experiences, both in individuals with psychotic disorders and in persons who are genetically at an increased risk for these disorders. Severe hearing impairment (SHI) is an environmental risk factor for psychotic disorder, possibly due to the experience of social exclusion. The aim of the current study is to investigate whether people with SHI exhibit higher levels of psychotic reactivity to social stressors in daily life than normal-hearing controls and whether this reactivity …

Variant Creutzfeldt-Jakob Disease Presenting as New-onset Psychosis.

The case of a patient with a first presentation psychotic episode secondary to variant Creutzfeldt-Jakob Disease (vCJD) is presented. While psychiatric symptoms are considered a prominent feature of vCJD, they may precede characteristic neurological symptoms, which can delay diagnosis. The psychotic symptoms in this case differed in quality from typical psychotic presentations, which could have helped with earlier diagnosis. The patient's symptomatology suggested that errors in cognition and perception were largely contributing to his psychiatric symptoms. These errors appeared to be the resul

Prevalence and treatment of psychiatric disorders other than psychosis in children and adolescents with 22q11DS: Examining associations with social and role functioning.

Individuals with chromosome 22q11 deletion syndrome (22q11DS) have high rates of psychotic disorders. Less is known about their psychopathology and how it is treated prior to the peak period of risk for psychotic disorder. There is also a lack of evidence on how functioning is impacted by psychopathology in this population. The aim of this study was to investigate the prevalence and treatment of non-psychotic psychiatric disorders, and how these factors are associated with psychosocial functioning in children and adolescents with 22q11DS. 126 individuals with 22q11DS aged 8-17 participated in …

Use of quetiapine in children and adolescents.

The atypical antipsychotic quetiapine has been used in different psychotic and non-psychotic disorders in children and adolescents in randomized clinical trials, open-label studies and chart reviews. Most of these studies suggest that quetiapine may be a promising agent with a potential for use in young patients. The aim of this paper is to critically review available literature on quetiapine in the treatment of children and adolescents with a variety of psychiatric disorders, including psychotic disorders, bipolar disorders (manic and depressive episodes), conduct disorder, autism spectrum di…

Prevalence, risk factors, and comorbidities of psychotic experiences in Afghanistan: a highly stressful environment.

OBJECTIVES: To estimate the prevalence and demographic, psychiatric, and trauma-focused correlates of psychotic experiences (PEs) in the Afghan general population. METHODS: Data were drawn from a cross-sectional household survey implemented in eight regions of Afghanistan (N = 4445). The CIDI structured instrument was administered to adults to assess psychiatric disorders and psychotic experiences; life events and PTSD were assessed using validated instruments. Weighted multivariate models integrated socio-demographics, regions, traumas as determinants of PE. RESULTS: PEs were frequently rep

The "black hole" as the basic psychotic experience: some newer psychoanalytic and neuroscience perspectives on psychosis.

The psychoanalytic treatment of psychotic disorders has had a long and complicated history because of the historic preference of psychoanalysis for neurotics. Nevertheless, it has survived the prejudice of psychoanalysts and empirical psychiatrists and now enters an interdisciplinary phase in which psychotic psychopathology is understood as primarily an emotional disorder, but one that must also be considered from the point of view of neurobiology and neuropsychology as well as sociology. In this contribution, I offer the idea that perhaps the most important subtext in the psyche of the psycho…

Clozapine for Quetiapine-Refractory Psychosis in Parkinson's Disease: A Long-Term Single-Center Retrospective Study.

Background: Hallucinations and delusions are severe long-term complications of Parkinson's disease (PD). Clozapine is the only antipsychotic with proven efficacy in PD psychosis (PDPsy) available outside the United States but apprehensions about its adverse event profile result in a substantial underuse of clozapine. Objectives: To investigate the practical use and long-term efficacy of clozapine in severe psychotic disorders in PD. Methods: In this retrospective study, the author used data collected over a period of 20 years and included all PD patients under his care, who were treated with c…

The mediating role of borderline personality features in the relationship between childhood trauma and psychotic-like experiences in a sample of help-seeking non-psychotic adolescents and young adults.

OBJECTIVE: Psychotic-like experiences (PLEs) often occur across different non-psychotic disorders in adolescent and young adult population and are related to early trauma. However, the mechanisms of how exposure to early trauma shapes the risk of PLEs are unclear. In our study, we investigated whether borderline personality features and further non-psychotic symptoms, i.e. factors related to both PLEs and childhood trauma, may mediate the relationship between childhood trauma and PLEs. METHODS: Two hundred inpatients aged 16-21 years who were treated due to non-psychotic disorders were includ…

Epidemiological analysis of alcohol and drug use as risk factors for psychotic experiences.

Clinical and laboratory studies link alcohol and other drug use to the occurrence of psychotic experiences, but epidemiologic evidence has been lacking. In this study, the quantitative relationships between alcohol or other drug use and psychotic experiences were examined by analysis of prospective data from 4994 adult household residents sampled in a multisite survey of mental disorders in the population, the NIMH Epidemiologic Catchment Area Program. After control for sociodemographic factors and preexisting psychiatric conditions, the risk for onset of self-reported delusions or hallucinati

Childhood trauma and auditory verbal hallucinations.

BACKGROUND: Hallucinations have consistently been associated with traumatic experiences during childhood. This association appears strongest between physical and sexual abuse and auditory verbal hallucinations (AVH). It remains unclear whether traumatic experiences mainly colour the content of AVH or whether childhood trauma triggers the vulnerability to experience hallucinations in general. In order to investigate the association between hallucinations, childhood trauma and the emotional content of hallucinations, experienced trauma and phenomenology of AVH were investigated in non-psychotic

Structural brain changes in first episode mania with and without psychosis: Data from the Systematic Treatment Optimization Program for Early Mania (STOP-EM).

OBJECTIVES: The neurobiological underpinnings of bipolar I disorder are not yet understood. Previous structural neuroimaging studies of bipolar disorder have produced rather conflicting results. We hypothesise that clinical sub-phenotypes of bipolar I disorder defined by their psychotic symptoms, especially those with mood-incongruent psychotic features, may have more extensive structural brain abnormalities. METHODS: We investigated structural brain alterations in patients with first-episode mania (n = 55) with mood-congruent (n = 16) and mood-incongruent (n = 32) psychotic features, as well

Familial High-Risk for Psychosis and Bipolar Disorder Amongst Youth in an Academic Healthcare System: A Real-World Assessment of Clinical Characteristics and Implications for Clinical Care.

OBJECTIVE: Family history is amongst the most significant risk factors for psychotic and bipolar disorders. Despite being clearly defined, easily and early identifiable, familial high-risk status is minimally accounted for in mainstream prevention paradigms. This study reports on the demographics and clinical characteristics of youth with a first-degree relative affected by psychotic or bipolar disorders within a large academic healthcare system. METHOD: In this quality improvement study, using electronic medical records, the authors examined demographics, psychiatric and medical diagnoses, s
