Effect of whole-body vibration therapy on performance recovery.

PURPOSE: To compare whole-body vibration (WBV) with traditional recovery protocols after a high-intensity training bout. METHODS: In a randomized crossover study, 16 athletes performed 6 × 30-s Wingate sprints before completing either an active recovery (10 min of cycling and stretching) or WBV for 10 min in a series of exercises on a vibration platform. Muscle hemodynamics (assessed via near-infrared spectroscopy) were measured before and during exercise and into the 10-min recovery period. Blood lactate concentration, vertical jump, quadriceps strength, flexibility, rating of perceived exer

The influence of muscle mass, strength, fatigability and blood flow on exercise capacity in cachectic and non-cachectic patients with chronic heart failure.

BACKGROUND: The influence of age, skeletal muscle function and peripheral blood flow on exercise capacity in chronic heart failure patients is controversial, possibly due to variations in skeletal muscle atrophy. METHODS AND RESULTS: To assess predictors of exercise capacity in patients with clinical cardiac cachexia, we studied 16 cachectic and 39 non-cachectic male chronic heart failure patients of similar age and ejection fraction. All cachectic patients were wasted (% ideal body weight: 81.2 +/- 1.9 vs 105.2 +/- 2.1, P < 0.0001, mean +/- SEM) and had documented weight loss (5-30 kg). Peak

Isokinetic exercise system modification for short below-the-knee residual limbs.

The use of isokinetic exercise has been shown to be an effective way of strengthening debilitated muscles. In the below the knee amputee, significant quadriceps and hamstring muscle wasting has been documented. Although isokinetic strengthening of the debilitated knee extensors and flexors in the below the knee amputee would be beneficial, there are no fully described isokinetic equipment modifications in literature that would allow a short below the knee amputee to effectively use isokinetic equipment. This article describes such a modification.

[Simultaneous Ruptures of All Four Extensor Mechanism Tendons in Both Knees: a Rare and Challenging Case in a Chronic Kidney Disease Patient].

We report a rare and remarkable case of simultaneous bilateral injuries to all four extensor mechanism tendons in a 39-year-old male with a history of diabetes and chronic kidney disease. Physical examination revealed pronounced swelling, palpable gaps above and below both patellae, and complete loss of active knee extension. Imaging confirmed complete ruptures of the right quadriceps and left patellar tendons, alongside partial ruptures of the left quadriceps and right patellar tendons. Surgical repair was performed using suture anchors to ensure secure tendon reattachment. Postoperatively, b

Does quadriceps contractile fatigue influence rehabilitation outcomes in COPD-chronic respiratory failure patients?

BACKGROUND: In patients with moderate COPD, response to pulmonary rehabilitation including exercise training varies according to the presence of peripheral muscle fatigue (pMF) of quadriceps. This study investigates the role of pMF in predicting pulmonary rehabilitation outcomes in more severe COPD patients who have already developed chronic respiratory failure (COPD-CRF). METHODS: A post hoc analysis of a prospective randomised controlled trial was performed at Istituti Clinici Scientifici Maugeri Lumezzane (Brescia, Italy), involving 30 COPD-CRF patients undergoing a pulmonary rehabilitatio

Trigonelline improves mitochondrial calcium homeostasis and reduces calpain activity after skeletal muscle ischemia-reperfusion injury in mice.

Skeletal muscle ischemia-reperfusion injury (IRI) causes persistent tissue damage and functional impairment, partly through mitochondrial calcium dysregulation and calpain-mediated proteolysis. This study investigated whether trigonelline (TG), a natural alkaloid, improves skeletal muscle recovery and mitochondrial homeostasis after IRI. Male C57BL/10 mice were randomly assigned to sham control, untreated IRI, or IRI plus TG groups. After 2 h of bilateral hindlimb ischemia, mice received a single intraperitoneal dose of TG (150 mg/kg) immediately before tourniquet release and were evaluated af

Abnormal gait pattern in downhill hiking is related to muscular deficits of the knee flexors and extensors in active patients with total knee arthroplasty.

BACKGROUND: To assess the in-field walking mechanics during downhill hiking of patients with total knee arthroplasty five to 14 months after surgery and an age-matched healthy control group and relate them to the knee flexor and extensor muscle strength. METHODS: Participants walked on a predetermined hiking trail at a self-selected, comfortable pace wearing an inertial sensor system for recording the whole-body 3D kinematics. Sagittal plane hip, knee, and ankle joint angles were evaluated over the gait cycle at level walking and two different negative slopes. The concentric and eccentric low

Anterior cruciate ligament reconstruction using quadriceps tendon autograft is a viable option for small-statured female patients.

PURPOSE: The choice of graft for anterior cruciate ligament (ACL) reconstruction remains controversial. The quadriceps tendon (QT) autograft is a good alternative for ACL reconstruction. However, concerns regarding its use in short-statured patients, related to donor site morbidity, anterior knee pain, or loss of muscle strength remain. This study aimed to compare muscle strength and morbidity between patients with short and normal statures following ACL reconstruction with a QT autograft. METHODS: A total of 73 female patients (mean age, 33.8 ± 11.5 years) who underwent primary ACL reconstru

Peripheral Nociception Is Associated with Voluntary Activation Deficits and Quadriceps Weakness Following Total Knee Arthroplasty.

BACKGROUND: Quadriceps weakness is a hallmark of total knee arthroplasty and is driven by reduced voluntary muscle activation following the surgical procedure. The mechanisms underlying postoperative activation deficits are not well established, although nociception has been implicated via both spinal reflex and supraspinal pathways. The purpose of this study was to assess the role of nociception in postoperative recovery of strength and activation. METHODS: A total of 53 participants were assessed prior to total knee arthroplasty and at 6 weeks postoperatively. Quadriceps strength was measur

[Streptococcus pyogenes pyomyositis of the piriformis muscle in a child].

Acute pyomyositis is a suppurative infection of the skeletal muscle. Staphylococcus aureus is the most frequent etiological agent. Cases produced by Streptococcus pyogenes are uncommon. The most usual locations are quadriceps, buttocks and iliopsoas. The involvement of the piriformis muscle is very exceptional. We present the case of a previously healthy 8-year-old male who was admitted to our hospital for acute pyomyositis caused by Streptococcus pyogenes located in the piriformis muscle. Early diagnosis and prompt initiation of antibiotic treatment allowed the conservative management of the
