Dynamic properties of vestibular reflexes in the decerebrate cat.

Vestibulocollic (VCR) and vestibulo-ocular (VOR) reflexes were studied during angular rotation in the horizontal plane in precollicular decerebrate cats. Angular position was modulated by sinusoids or sums of sinusoids with frequencies ranging from 0.05 to 5 Hz. Reflex motor output was measured by recording electromyographic (EMG) activity of the lateral rectus and dorsal neck muscles and discharge of abducens motoneurons. Measured with respect to input angular acceleration VCR motor output displayed a second order lag at low frequencies, bringing mean EMG phase (-136 degrees) and gain slope (

[Diagnosis of brain death using trancranial Doppler ultrasonography in an experimental model of organ donation for transplantation].

OBJECTIVE: To create an experimental organ donor model and validate it by ultrasound evaluation of intracranial blood flow velocity. MATERIAL AND METHOD: Ten white New Zealand rabbits were assigned to either a control group or an experimental group. Brain death was induced in the experimental group by way of severe cranial hypertension. A diagnosis of brain death was based on physical examination and velocity of intracranial blood flow determined by transcranial doppler ultrasonography. Physical and ultrasound examinations were performed on all animals at baseline and at the time of the exper

Vestibular hair cell redundancy and the critical requirement of type I cells for balance maintenance.

Vestibular hair cells (HCs) are sensory mechanotransducers essential for balance and spatial orientation, yet the relative contributions of HC number and subtype to vestibular function remain unresolved. Here, we developed a dose-dependent injury model in adult Pou4f3DTR mice by administering diphtheria toxin (DT), which induced selective HC ablation across vestibular organs. Both type I and type II HCs were lost following DT treatment. Type II HCs exhibited higher susceptibility to DT at lower doses, although this difference diminished as DT dose increased and overall HC survival declined. Fu

Horizontal head-shaking test: pathophysiological mechanisms and clinical interpretation.

The horizontal head-shaking test (HST) is a simple, rapid, and non-invasive bedside maneuver that provides valuable insights into both peripheral and central vestibular function. Originally described in the late 19th century and standardized in the 1970s, the HST predominantly stimulates the lateral semicircular canals (LSCs) at high frequencies. Its diagnostic power lies in revealing dynamic asymmetries of the angular vestibulo-ocular reflex (aVOR) and uncovering central abnormalities involving the brainstem and, above all, cerebellum. The pathophysiological basis of post-head-shaking nystagm

Case report: A disconjugate pattern in video head impulse testing hints toward a central cause of acute vertigo.

When acute vertigo occurs, the challenge for the medical practitioner lies in the focused assessment to find the cause of its symptoms. Especially in the case of central pathology, a fast diagnosis is essential for therapy. The head impulse, nystagmus, test of skew (HINTS) protocol and the additional video head impulse test (VHIT) can distinguish between central and peripheral vestibular causes in the acute setting and thus help to set the right path for further evaluation and treatment. In this case, a patient with acute onset of vertigo presented with an unusual pattern in the VHIT. Binocula

Sensitized rotatory motion perception and increased susceptibility to motion sickness in vestibular migraine: A cross-sectional study.

BACKGROUND AND PURPOSE: Vestibular migraine (VM) patients are ictally and interictally hypersensitive for self-motion and visual perception. Increased cortical excitability of the vestibular system represented by lowered motion perception thresholds might play an important role in the pathophysiology of VM. We aimed to compare motion perception thresholds and the vegetative response to rotatory motion, as well as the vestibulo-ocular reflex (VOR) during rotation in VM patients compared to healthy controls (HC). METHODS: In this cross-sectional study, 28 female VM patients in the interictal st

The aVOR App Increases Medical Students' Competence in Treating Benign Paroxysmal Positional Vertigo (BPPV).

HYPOTHESIS: Implementation of the "aVOR app" into teaching courses at medical school enhances students' satisfaction with the course and increases their competence in treating benign paroxysmal positional vertigo (BPPV). BACKGROUND: BPPV is often underdiagnosed and left untreated. One problem in teaching the management of BPPV to health care professionals is the lack of simulation-based training tools. The aVOR app (aVOR = angular vestibulo-ocular reflex) works as a bionic labyrinth that simulates the activation of the semicircular canals by rotational acceleration and the resulting vestibula

Nuclear, internuclear, and supranuclear ocular motor disorders.

In the brainstem, lateral and vertical eye movements are controlled by separate structures, the former mainly in the pons and the latter in the midbrain. The abducens nucleus (VI) in the pons controls all ipsilateral eye movements, i.e., ipsilateral saccades as well as the horizontal vestibulo-ocular reflex (VOR). This nucleus contains the abduction motoneurons, but also the internuclear neurons involved in adduction, passing through the contralateral medial longitudinal fasciculus (MLF) before relaying in the third-nerve nucleus in the midbrain. Lesions affecting the abducens nucleus result i

Abnormal connection between lateral and posterior semicircular canal revealed by a new modeling process: origin and physiological consequences.

We developed a modeling procedure using CT scans or MRI data for exploring the bony and lymphatic canals of vestibular patients. We submitted 445 patients with instability and spatial de-orientation to this procedure. Out of the 445 patients, 95 had scoliosis, some of them, because malformations were suspected also had CT-scan modeling and functional tests. We focused on a never described, abnormal connection between the lymphatic lateral and posterior canal (LPCC) with a frequency of 67/445 (15%). In the scoliosis subgroup, the frequency was 52/95 (55%). Three scoliotic patients had CT scans.

A head autorotation test for patients with Menière's disease.

OBJECTIVE: We compared the vestibulo-ocular reflex (VOR) of healthy volunteers and of two groups (group A, conservatively treated; and group B, with gentamicin or surgically treated) of Menière's disease (MD) patients to determine whether the head autorotation test (HART) can reveal abnormality of the VOR and whether the HART can be applied in evaluation of MD. METHODS: The gain, phase, asymmetry, and the highest frequency band reached was evaluated with the HART in the frequency range of 1-5 Hz. The caloric responses and posturography results were also determined. RESULTS: The mean gain was

Torsional optokinetic nystagmus after unilateral vestibular loss: asymmetry and compensation.

The aim of this study was to analyse torsional optokinetic nystagmus (tOKN) in 17 patients with Menière's disease before and after (1 week, 1 month and 3 months) a curative unilateral vestibular neurotomy (UVN). The tOKN was investigated during optokinetic stimulations around the line of sight directed towards either the lesioned or the healthy side, at various constant angular velocities. Dynamic properties of tOKN and static ocular cyclotorsion were analysed using videonystagmography. Patients' performances were compared with those of 10 healthy subjects. The results indicate that, in the ac

High-frequency pseudo-hypofunction of the superior semicircular canal dehiscence syndrome: a vHIT - oVEMP dissociation explained by hydrodynamic energy loss.

BACKGROUND: Superior semicircular canal dehiscence syndrome (SCDS) produces a wide spectrum of vestibular and auditory abnormalities due to the presence of a "third mobile window." While enhanced otolith-mediated responses are well recognized, the behavior of high-frequency semicircular canal function remains debated. Many patients show reduced superior canal vestibulo-oculomotor reflex (VOR) gains despite preserved or exaggerated utricular responses. OBJECTIVE: To investigate the relationship between dynamic semicircular canal responses assessed by video Head Impulse Test (vHIT) and otolith

Cross-coupling vestibular stimulation: motion sickness and the vestibulo-sympathetic reflex.

Motion sickness is associated with a variety of autonomic symptoms, presumably due to proximity or functional interconnectivity between the autonomic centers in the brainstem and the vestibular system. A direct influence of the vestibular system on cardiovascular variables, defined as the vestibulo-sympathetic reflex, has been reported previously. Our aim was to investigate the sudomotor components of the autonomic responses associated with motion sickness during passive cross-coupling stimulation ("roll while rotating"). Healthy subjects (n = 17) were rotated at 40°/s around an earth-vertical

Vestibular implantation and longitudinal electrical stimulation of the semicircular canal afferents in human subjects.

Animal experiments and limited data in humans suggest that electrical stimulation of the vestibular end organs could be used to treat loss of vestibular function. In this paper we demonstrate that canal-specific two-dimensionally (2D) measured eye velocities are elicited from intermittent brief 2 s biphasic pulse electrical stimulation in four human subjects implanted with a vestibular prosthesis. The 2D measured direction of the slow phase eye movements changed with the canal stimulated. Increasing pulse current over a 0-400 μA range typically produced a monotonic increase in slow phase eye v

Imaging cortical activity after vestibular lesions.

In this article we will discuss our current knowledge of multisensory vestibular structures and their functions in the human cortex. Most of it derives from brain activation studies with PET and fMRI in humans conducted over the last decade. They have confirmed the existence of several separate and distinct cortical areas that were identified earlier by tracer and electrophysiological studies in animals, especially in monkeys. The patterns of activations and deactivations during vestibular stimulations in healthy subjects have been compared with those in patients with acute and chronic periphe

Evidence of vestibular and balance dysfunction in children with profound sensorineural hearing loss using cochlear implants.

OBJECTIVES/HYPOTHESIS: Similarities between the peripheral auditory and vestibular systems suggest that children with sensorineural hearing loss (SNHL) may demonstrate vestibular and balance impairments. This hypothesis was studied in 40 children with severe to profound SNHL and unilateral cochlear implants. STUDY DESIGN: Prospective cross-sectional study with repeated measures. METHODS: Vestibular function was assessed with caloric, rotational, and vestibular evoked myogenic potential testing; balance was assessed using the balance subset of the Bruininks-Oseretsky Test of Motor Proficiency

"Dumping" of activity within the VOR is possible after unilateral vestibular neurectomy.

The effect of head tilt on postrotatory nystagmus was studied in patients at long time follow-up of complete unilateral vestibular lesions. Ten patients were tested 2-4 years after unilateral vestibular neurectomy. They were all found to have short VOR time constants. However, at postrotatory head tilt the VOR time constant was further shortened. This suggests that the velocity storage mechanisms may function even in cases with complete unilateral vestibular lesions.

A quantitative analysis of head-shaking nystagmus of peripheral vestibular origin.

A quantitative analysis of horizontal head-shaking nystagmus (HSN) was made on 48 patients with unilateral peripheral vestibular lesions in conjunction with stimulus intensity. Each patient underwent three head-shaking tests with 10, 30 and 50 horizontal head-excursions at a frequency of approximately 2 Hz, and HSN was recorded on ENG with eyes open in total darkness. i) HSN appeared in a biphasic or monophasic pattern. ii) The maximal slow-phase eye velocity (MSV) of the 1st phase (PI) of biphasic HSN increased significantly in proportion to stimulus intensity, and was significantly greater t
