[Retroperitoneal fibrosis].

Retroperitoneal fibrosis is a rare clinical entity characterised by the presence of patologic collagen tissue in a retroperitoneal space. The fibrous mass covers abdominal organs causing their disfunctions. RPF was described at the begining of XX century but its etiology is not clear yet. Usually it causes an ureter obstuction and hydronephrosis, that is why most commonly is diagnosed by urologists and nephrologists. However, retroperitoneal fibrosis can be multifacial disease. In some patients localisation of fibrosis is atypical and manifestationns can be varied. Gastrological symptoms like …

Surgical Management of a Pancreaticopleural Fistula After Failed Endoscopic Therapy.

Inflammatory diseases of the pancreas or pancreatic trauma result in ductal cell disruption, which in turn may lead to leakage of pancreatic fluid, mostly in the retroperitoneal space. Pancreatopleural fistulas are uncommonly encountered following pancreatic injury; however, they often prove a difficult problem to manage. Herein, we present a rare case of a 68-year-old male suffering from a pancreaticopleural fistula (PF) between the pancreatic tail and the left pleural space one year following splenectomy for trauma. About three months after percutaneous drainage of a left pleural effusion an…

Signet-ring cell carcinoma of the urinary bladder mimicking retroperitoneal fibrosis.

We present the case of a 77-year-old white woman with a past medical history of transitional cell carcinoma of the urinary bladder that presented with symptoms of acute renal failure and duodenal obstruction and posed a diagnostic dilemma. Initially, she presented with bilateral ureteral strictures and eventually required bilateral nephrostomy tubes. Later, the patient developed intractable nausea and vomiting secondary to a duodenal stricture. The finding of a "stranding appearance" on computed tomography imaging of the retroperitoneal space raised the suspicion of retroperitoneal fibrosis. S…

[Surgical removal of retroperitoneal liposarcoma after transarterial embolization: a case report].

A case of retroperitoneal liposarcoma that was removed after transarterial embolization is reported. A 62-year-old man was admitted with body weight loss and general fatigue. Computed tomography revealed an extrarenal tumor, 27 x 17 x 11 cm in size, in the left retroperitoneal space. Arteriography revealed that the hypervascular tumor was fed from the left renal artery, the left adrenal artery and the left lumber arteries (L1-L4). At first the patient underwent transarterial embolization of the left renal artery and the left lumbar arteries (L1, L3, L4). Twenty-two days later he underwent surg…

[IgG4-related lesion of the retroperitoneum, ureter, and kidney, mimicking infiltrating endometriosis].

Urinary tract endometriosis is rare, and ureteral involvement is particularly insidious due to its asymptomatic nature. The absence of symptoms can lead to ureteral stricture, hydronephrosis, chronic kidney disease up to end-stage renal failure, and irreversible loss of kidney function. Diagnosis requires imaging methods to assess the degree of obstruction, while treatment involves surgical release of the ureter from endometriotic infiltrates to preserve organ function. Our observation illustrates the difficulty of establishing the etiology of a retroperitoneal tumor-like infiltrate leading to

Retroperitoneal-First Laparoscopic Approach (Retlap) for Bile Duct Resection in Pancreaticobiliary Maljunction (PBM): A Novel and Reasonable Approach to Intrapancreatic Bile Duct.

BACKGROUND: Pancreaticobiliary maljunction (PBM) is a congenital anomaly in which the pancreatic and bile ducts join outside the duodenal wall.1,2 Reflux of pancreatic juice induces chronic inflammation and epithelial regeneration, promoting carcinogenesis. Biliary dilation increases the risk of cholangiocarcinoma, and prophylactic resection of the extrahepatic bile duct and gallbladder is recommended even for asymptomatic patients.3,4 To prevent postoperative carcinogenesis, the intrapancreatic bile duct should be transected just above the pancreatobiliary junction. The authors describe a nov

Retroperitoneal ectopic pregnancy localized to the mesorectum: A case report and mini-review of literature.

Retroperitoneal ectopic pregnancy is an extremely rare form of ectopic pregnancy that can be life-threatening due to severe intra-abdominal hemorrhage. Retroperitoneal ectopic pregnancy is frequently misdiagnosed because of its atypical location and nonspecific symptoms. To the best of our knowledge, no previous cases of ectopic pregnancy localized to the mesorectum have been reported to date. Here, we present the case of a woman in her mid-30s with retroperitoneal ectopic pregnancy localized to the mesorectum. The condition was initially misdiagnosed as gastroenteritis due to atypical symptom

The Minimally Invasive Intercostal Subdiaphragmatic Access without Rib Resection for Lateral Lumbar Interbody Fusion at L1/2: Surgical Techniques and Cases Illustration.

OBJECTIVE: The minimally invasive (MI) lateral approach to the thoracolumbar junction for treating various spinal pathologies is quite challenging for spine surgeons due to this region's unfamiliar and complex anatomical structures. In addition, controversy still exists regarding approach selection, the need for rib resection, and diaphragm manipulation. METHODS: We present the senior author (N. A.) technique of an intercostal subdiaphragmatic retroperitoneal approach without rib resection for the lateral lumbar interbody fusion (LLIF) procedure at L1/2 in patients who underwent multilevel LL

Renal preservation in high-risk retroperitoneal neuroblastoma: Impact on survival and local progression.

BACKGROUND: Retroperitoneal neuroblastomas predominantly encroach upon critical structures, complicating surgical intervention and yielding elevated rates of surgery-associated complications. The kidney and renal vasculature represent the organs most susceptible to retroperitoneal neuroblastoma infiltration. Prior investigations have revealed high nephrectomy incidence and a paucity of renal-preserving surgical approaches. METHODS: A retrospective analysis was undertaken, examining patients with retroperitoneal neuroblastoma who underwent surgical procedures from January 2018 to December 2019

The anatomical positioning change of retroperitoneal organs in prone and lateral position: an assessment for single-prone position lateral lumbar surgery.

PURPOSE: There are reports that performing lateral lumbar interbody fusion (LLIF) in a prone, single position (single-prone LLIF) can be done safely in the prone position because the retroperitoneal organs reflect anteriorly with gravity. However, only a few study has investigated the safety of single-prone LLIF and retroperitoneal organ positioning in the prone position. We aimed to investigate the positioning of retroperitoneal organs in the prone position and evaluate the safety of single-prone LLIF surgery. METHODS: A total of 94 patients were retrospectively reviewed. The anatomical posi

Predicting surgical outcome in posterior retroperitoneoscopic adrenalectomy with the aid of a preoperative nomogram.

BACKGROUND: Posterior retroperitoneoscopic adrenalectomy (PRA) has several advantages over transperitoneal laparoscopic adrenalectomy (TLA) regarding operative time, blood loss, postoperative pain, and recovery. However, it can be a technically challenging procedure. To improve patient selection for PRA, we developed a preoperative nomogram to predict operative time. METHODS: All consecutive patients with tumors of ≤ 7 cm and a body mass index (BMI) of < 35 kg/m2 undergoing unilateral PRA between February 2011 and March 2020 were included in the study. The primary outcome was operative time a

Indocyanine Green-Guided Intraoperative Imaging to Facilitate Video-Assisted Retroperitoneal Debridement for Treating Acute Necrotizing Pancreatitis.

Video-assisted retroperitoneal debridement (VARD) is a feasible, minimally invasive necrosectomy method for treating severe acute necrotizing pancreatitis, if it does not resolve or is accompanied with infected necrosis in the retroperitoneum. As there are rarely any visually clear separating surface in white light image between necrotic debris and adjacent inflammatory normal tissues due to extensive retroperitoneal adhesions, VARD is accompanied with the risk of vascular injury, external pancreatico-cutaneous or enterocutaneous fistulae. In view of the above disadvantages, we apply real-time

A large retroperitoneal Mullerian cyst: case report and review of the literature.

BACKGROUND: Mullerian duct cysts are considered to be part of the urogenital cysts. Mullerian cysts, anatomically distant to the genitourinary organs or the pelvis, are extremely rare entities. MATERIAL AND METHODS: Case report and review of the literature using key word "Mullerian cyst", "retroperitoneal" and "mediastinal". RESULTS: A 48-year-old woman with abdominal discomfort and nausea was referred to our hospital. Further work-up with CT and MRI showed a large retrogastric, retropancreatic cyst, herniating through the diaphragm in the mediastinum. As etiology was unsure, a laparoscopic

Successful resection of cellular angiofibroma in the retroperitoneum by using laparoscopic approach.

Retroperitoneal cellular angiofibroma (RCA) is very rare, and the optimal treatment for RCA has not been established. We report the case of RCA in a 58-year-old man who underwent curative laparoscopy-assisted resection. Preoperative computed tomography showed heterogeneous enhancement of the 7 cm diameter tumor in the pelvis. A smaller (2.3 cm) mass was also detected in the small intestine. The preoperative diagnosis was peritoneal metastasis of the gastrointestinal tumor of the small intestine. The pelvic tumor was laparoscopically mobilized from the rectum, the left ureter, and the left inte

First case report of retroperitoneal metastasis of fascioliasis after surgery.

RATIONALE: Fascioliasis is a rare cause of liver abscesses, and its clinical course consists of hepatic phase and biliary phase. PATIENT CONCERNS: We describe a 58-year-old female patient who presented with a 2-month history of intermittent fever and abdominal pain. An abdominal computed tomography (CT) revealed confluent low-density lesions in the liver. Complete surgical resection of these abscesses was performed, and postoperative pathological examination and serological tests confirmed a diagnosis of fascioliasis. However, 4 months after the surgery, follow-up CT revealed a lesion in the
