[Hibernoma and cervical rib: two rare diseases, the same manifestation].

The hibernoma is a rare benign tumor of soft tissue, derived from remnants of fetal brown adipose tissue. A cervical rib is a supernumerary … an infectious or lymphoproliferative disease. In cervical radiograph we observed bilateral cervical ribs. Cervical ultrasound revealed calcified nodule 0.8 cm, compatible with calcified adenopathy. Biopsy was performed and histology revealed a hibernoma, which was completely removed surgically. This case illustrates the association of two diagnoses, un …

Factors affecting pneumonia occurring to patients with multiple rib fractures.

BACKGROUND: Rib fractures are the most common type of thoracic trauma and cause other complications. We explored the risk factors for pneumonia in patients with multiple rib fractures. MATERIALS AND METHODS: Four hundred and eighteen patients who visited our hospital with multiple rib fractures between January 2002 and December 2008 were retrospectively reviewed. Chest X-rays and chest computed tomography were used to identify injury severity. Patients with only a single rib fracture or who were transferred to another hospital within 2 days were excluded. RESULTS: There were 327 male patient

[Diagnosis and treatment of rib fracture during spontaneous vaginal delivery].

INTRODUCTION: Progress of labor in multiparous women usually is not accompanied with risk of any kind of birth trauma. CASE OUTLINE: We report a very rare case of rib fracture in a neonate during vaginal delivery in the 39/40 week of gestation. The expulsion started spontaneously without any manipulation from the obstetrician. Live male newborn was delivered 4650 g. in weight, 55 cm long, with head circumference of 39 cm, Apgar score 9. The child was immediately examined by the neonatologist. Crepitations were palpable over the left hemithorax, and auscultatory on the left side inspiratory cr

Superior subluxation of an anterior end of the first rib in a trauma patient.

We present a case of a young man with a superior subluxation of an anterior end of the first rib following a road traffic accident. To the best of our knowledge, no similar case has so far been described in the literature. The patient presented with headache, backache and pain in the right anterior chest wall. After initial assessment, a chest radiograph was performed and it showed no abnormalities. A CT of the chest revealed an isolated hypermobile first rib at the anterior end owing to superior subluxation. The patient was treated conservatively.

Correction of enophthalmos with rib bone segment and diced cartilage grafts.

We introduce our experiences of correcting enophthalmos with a rib bone segment graft and diced cartilage graft simultaneously. The affected orbit was exposed through a subcilliary incision, a medial brow incision, and a lateral brow incision. The periorbita was carefully dissected from the bony orbit circumferentially. The affected area was examined, and the size and shape of the bone graft were determined. A 5-cm incision was made at the 7th rib-cartilage junction, and the 7th and 8th ribs and costal cartilage was harvested as needed. The donor site was closed. The harvested rib bone segment …

Surgical resection for isolated myeloid sarcoma arising from a rib.

We report on a rare manifestation of myeloid sarcoma with chronic myelogenous leukemia. The neoplasm arising from a rib showed osteolytic changes and infiltrated the surrounding muscles. We resected the lesion to relieve chest pain caused by pathologic rib fractures. The patient has continued receiving systemic therapy and is doing well as of 1 year postoperatively. Experience with a surgical approach for myeloid sarcoma is limited in thoracic surgery. The clinical course in our case suggests that surgical resection can be a valid choice of treatment.

Thoracic Epidural Anesthesia and Analgesia (TEA) in Patients with Rib Fractures.

BACKGROUND: We analyzed the results of surgical reduction and fixation of ribs under thoracic epidural anesthesia and analgesia (TEA) in patients who had no more than 3 consecutive rib fractures with severe displacement to examine the clinical usefulness of this method. MATERIALS AND METHODS: From May 2008 to March 2010, 35 patients underwent surgical reduction and fixation of ribs under TEA. We reviewed the indications for this technique, number of fixed ribs, combined surgical procedures for thoracic trauma, intraoperative cardiopulmonary events, postoperative complications, reestablishment…

Continuous intercostal nerve blockade for rib fractures: ready for primetime?

BACKGROUND: Providing analgesia for patients with rib fractures continues to be a management challenge. The objective of this study was to examine our experience with the use of a continuous intercostal nerve block (CINB). Although this technique is being used, little data have been published documenting its use and efficacy. We hypothesized that a CINB would provide excellent analgesia, improve pulmonary function, and decrease length of stay (LOS). METHODS: Consecutive adult blunt trauma patients with three or more unilateral rib fractures were prospectively studied over 24 months. The cathe

Paracoccidioides brasiliensis causing a rib lesion in an adult AIDS patient.

Paracoccidioidomycosis is a systemic mycosis with a geographic distribution that is limited to Central and South America; Brazil has the highest number of cases. Severe disseminated disease caused by paracoccidioidomycosis was observed in acquired immunodeficiency syndrome patients who live or have resided in endemic paracoccidioidomycosis areas. Here we describe a male patient admitted to a large public hospital with diffuse nodular infiltrates observed in chest radiographs and with erosion at the second rib near the sternum. Blood tests showed anti-human immunodeficiency virus antibodies, a

Pseudarthrosis of cervical rib: an unusual cause of thoracic outlet syndrome.

Thoracic outlet syndrome is uncommon in adolescence. Cervical rib fracture is an extremely rare cause of thoracic outlet syndrome in this age group. We report an unusual case of thoracic outlet syndrome in a 14-year-old girl caused by pseudarthrosis of the cervical rib. A magnetic resonance imaging scan showed significant compression of the brachial plexus by the pseudarthrosis mass. Excision of the cervical rib through a supraclavicular approach gave excellent results in this case.

Sacralization is not associated with elongated cervical costal process and cervical rib.

Cervical rib/elongated costal process of the seventh cervical vertebra and sacralization of a lumbar vertebra are associated with clinical problems-neurological, vascular, and obstetrical. A previous study reported an association between these morphologies, and suggested that clinicians use presence of one trait to predict presence of the other. This study tested three hypotheses from this association: costal process length among individuals with sacralization differs from that among individuals without sacralization for: (1) only the seventh cervical vertebra, (2) only transitional presacral

Rib head protrusion into the central canal in type 1 neurofibromatosis.

BACKGROUND: Intraspinal rib head dislocation is an important but under-recognized consequence of dystrophic scoliosis in patients with neurofibromatosis 1 (NF1). OBJECTIVE: To present clinical and imaging findings of intraspinal rib head dislocation in NF1. MATERIALS AND METHODS: We retrospectively reviewed clinical presentation, imaging, operative reports and post-operative courses in four NF1 patients with intraspinal rib head dislocation and dystrophic scoliosis. We also reviewed 17 cases from the English literature. RESULTS: In each of our four cases of intraspinal rib head dislocation,

Rib fracture repair: indications, technical issues, and future directions.

Rib fracture repair has been performed at selected centers around the world for more than 50 years; however, the operative indications have not been established and are considered controversial. The outcome of a strictly nonoperative approach may not be optimal. Potential indications for rib fracture repair include flail chest, painful, movable rib fractures refractory to conventional pain management, chest wall deformity/defect, rib fracture nonunion, and during thoracotomy for other traumatic indication. Rib fracture repair is technically challenging secondary to the human rib's relatively t

Spontaneous rib fractures in a black woman with familial hypocalciuric hypercalcemia.

BACKGROUND: Familial hypocalciuric hypercalcemia (FHH) is a rare but important consideration in the differential diagnosis of hypercalcemia. FHH results from an autosomal dominantly inherited inactivating mutation of the calcium sensing receptor (CaSR) gene and is typically associated with a benign clinical course and normal bone mineral density. CASE REPORT: We describe the unusual case of a 57-year old African American woman with spontaneous rib fractures who was found to have FHH due to a novel set of polymorphisms of the CaSR gene. She also had hypertension, esophageal reflux disease trea

Hydatid disease of the first rib causing thoracic outlet syndrome.

OBJECTIVE: To present a case of hydatid disease of the first rib as a rare cause of thoracic outlet syndrome. CLINICAL PRESENTATION: A 57-year-old female patient who had suffered from pain on the left shoulder radiating to the arm and numbness and weakness on the left arm for 3 months was admitted to our hospital. She had undergone an operation due to a mass lesion of the first rib compressing the thoracic outlet which was detected in the preoperative examinations. During the intraoperative examination it was decided that the lesion was a hydatid cyst and the first rib was totally resected.

Active pulmonary tuberculosis with vertebra and rib involvement: case report.

Multifocal skeletal tuberculosis is defined as osteoarticular lesions that occur simultaneously at two or more locations and is generally associated with disseminated disease. Although involvement of bones accounts for 1 to 5% of all tuberculosis cases, multifocal involvement of the skeleton is extremely rare. We present a case of active pulmonary tuberculosis (TB) with vertebral and rib involvement and multiple hypodense lytic lesions accompanied by a paravertebral mass lesion. In the differential diagnosis, metastases, lymphoma, multiple myeloma, chordoma sarcoidosis and rare spinal infectio

Klippel-Feil syndrome associated with pneumatocyst of the right cervical rib.

We describe the case of a 50-year-old male with a history of asthma and seizure disorder who presented with a 5-month history of dyspnea. The patient had been treated with multiple courses of antibiotics for presumed community-acquired pneumonia before being determined to have allergic bronchopulmonary aspergillosis (ABPA) by serologic and radiographic criteria. Inflammation resulting from this disease had potentiated a postobstructive pneumonia caused by Nocardia asteroides and Stenotrophomonas maltophilia.
