Expanding the reach of pre-exposure prophylaxis services in suburban and rural Ontario: an implementation science evaluation of a public health-led model.

BACKGROUND: Despite the proven efficacy of pre-exposure prophylaxis (PrEP) in preventing HIV, disparities among service delivery prevail in Canada, particularly among suburban and rural regions. This study evaluates the reach and effectiveness of a mixed-service delivery PrEP clinic embedded within a public health unit, located in Southeastern Ontario. METHODS: The PrEP clinic possesses an interdisciplinary team that delivers in-person and remote services. In consultation with PrEP clinic staff, we used the RE-AIM Framework to describe client characteristics (reach) and evaluate implementatio

Cultural adaptation, validity and reliability of the Marathi language version of the Patient Health Questionnaire-9 to screen depression in rural Maharashtra, India.

BACKGROUND: Screening is essential to bridge the significant care gap that exists for depression. PHQ-9 is a brief, valid, and reliable questionnaire, making it a valuable tool for detecting depression in both clinical and community settings. However, there is a lack of culturally and contextually appropriate local language versions of the PHQ-9, especially in India. This study aims to culturally adapt the existing Marathi version of the PHQ-9 for a rural Marathi-speaking population and evaluate its diagnostic accuracy and psychometric properties for detecting depression. METHODS: We followed

A study on the impact of digital infrastructure development on the health of low-income rural residents: based on panel data from 2010 to 2022.

The health status of low-income rural residents is intricately linked to social equity and justice and the realization of the goal of common prosperity. Based on the CFPS data from 2010 to 2020 and the list of "Broadband China" demonstration cities, this paper employs a multi-period Difference-in-Differences (DID) approach to empirically analyze the impact of digital infrastructure construction on the health of low-income rural residents and its mechanism. The results indicate that: (1) Digital infrastructure construction had a significant positive impact on the health of rural low-income resi

Insights Into Cancer Awareness and Health Practices in Rural Kenya: A Cross-Sectional Study of Esophageal, Breast, Prostate, Cervical, and Colorectal Cancers.

PURPOSE: In sub-Saharan Africa (SSA), there has been an epidemiologic transition from infectious diseases to noncommunicable diseases, including a projected doubling in incidence of cancer. Understanding the underlying factors driving this surge is crucial for designing effective strategies to mitigate the cancer burden. This study focuses on assessing knowledge, attitudes, screening practices, and health behaviors related to five most prevalent cancers in SSA: prostate, breast, cervical, esophageal, and colorectal-among a predominantly rural coastal population in Kenya. MATERIALS AND METHODS

Integration of a Tobacco Cessation Program into a Rural Community-Based Maternal and Child Health Program in India: A Stakeholders' Perspective on Task Shifting.

INTRODUCTION: According to the Global Adult Tobacco Survey-2, India, 7.5% of pregnant women are tobacco users with a high prevalence of smokeless tobacco use in rural India. Although pregnant women may receive advice to quit tobacco, lack of knowledge on providing cessation support among healthcare workers results in missed opportunities. Community Health Workers (CHWs) play a significant role in maternal and child health (MCH) programs. Thus, task-shifting for providing cessation support by CHWs was planned and the perceptions and attitudes of concerned stakeholders were explored. AIMS AND M

Meeting the Needs of Emerging Adults With Type 1 Diabetes Living in a Rural Area With Mobile Health Interventions: Focus Group Study.

BACKGROUND: Emerging adults (EAs; age 18-30 years) with type 1 diabetes (T1D) have more challenges with diabetes management and glycemic control than other age groups. Living in a rural community introduces additional unique diabetes care challenges due to limited access to specialty care and ancillary support services. Yet, few interventions have been developed to improve diabetes management in rural-dwelling EAs with T1D. OBJECTIVE: This study aimed to understand the diabetes management experiences of older adolescents and EAs (age 16-25 years) with T1D living in a rural area and to assess

Integrated Care Model by the Village Health Volunteers to Prevent and Slow down Progression of Chronic Kidney Disease in a Rural Community, Thailand.

INTRODUCTION: Chronic kidney disease (CKD) is a major health problem in Thailand and health behaviors are central to its risk and progression. Because of the shortage of healthcare personnel, village health volunteers (VHVs) have been collaborating in the primary health care system. However, the contribution of VHVs to CKD reduction has not been evaluated yet. This study aimed to evaluate the efficacy of the VHV-integrated model in preventing and slowing down CKD and its risk factors. METHODS: The population-based cohort study was conducted in a rural community of Thailand between 2017 and 20

Distribution, sources, and potential health risks of fluoride, total iodine, and nitrate in rural drinking water sources of North and East China.

Small-scale water sources serving villages and towns are the main source of drinking water in rural areas. Compared to centralized water sources, rural water sources are less frequently monitored for water quality and have poor post-treatment facilities, making them vulnerable to drinking health risks. To reveal the hydrochemical characteristics, contaminant sources, and health risks in rural water sources, 189 water samples were collected from lakes and reservoirs, rivers, and groundwater in North and East China for major ions, nutrient salts, microelements, and stable isotope analysis. A qualitative study to inform adaptations to a brain health intervention for older adults with type 2 diabetes living in rural regions of Ireland.

AIMS: Type 2 diabetes is a risk factor for late-life dementia, but dementia prevention strategies have yet to be comprehensively evaluated in people with diabetes. The Finnish Geriatric Intervention Study to Prevent Cognitive Impairment and Disability (FINGER) demonstrated cognitive benefits of a 2-year multidomain lifestyle intervention. However, given the intensive nature of FINGER, there is uncertainty about whether it can be implemented in other high-risk populations. Our aim was to explore attitudes towards dementia risk, and barriers to an intervention based on the FINGER model in older

Decentralised hepatitis C testing and treatment in rural Cambodia: evaluation of a simplified service model integrated in an existing public health system.

BACKGROUND: Direct-acting antiviral treatment for hepatitis C virus (HCV) has provided the opportunity for simplified models of care delivered in decentralised settings by non-specialist clinical personnel. However, in low-income and middle-income countries, increasing overall access to HCV care remains an ongoing issue, particularly for populations outside of urban centres. We therefore aimed to implement a simplified model of HCV care via decentralised health services within a rural health operational district in Battambang province, Cambodia. METHODS: The study cohort included adult reside…

The impact of HIV status on the distance traveled to health facilities and adherence to care. A record-linkage study from rural South Africa.

BACKGROUND: For people living with HIV (PLWH), the burden of travelling to a clinic outside of one's home community in order to reduce the level of stigma experienced, may impact adherence to treatment and accelerate disease progression. METHODS: This study is set in the Agincourt Health and Demographic Surveillance System (HDSS) in South Africa. Probabilistic and interactive methods were used to individually link HDSS data with medical records. A regression analysis was used to assess whether travel distance was correlated with the condition for which individuals were seeking care (primarily

Moving beyond living arrangements: the role of family and friendship ties in promoting mental health for urban and rural older adults in China.

Objective: This study examines the interplay among living arrangements, social networks, and depressive symptoms among Chinese older adults. Methods: Data are derived from the 2014 baseline survey of the China Longitudinal Aging Social Survey (CLASS), which provides a sample of older Chinese who had been married and had children (N = 7,662). This study examines the association between living arrangements and depressive symptoms (measured as CES-D scale, 0-18) of older adults, and addresses the moderating role of social networks (measured as family ties and friendship ties, 0-15) on this perceiv

Understanding the public health consequences of suspending a rural syringe services program: a qualitative study of the experiences of people who inject drugs.

BACKGROUND: Syringe services programs (SSPs) are evidence-based interventions that are associated with decreases in prevalence and incidence rates of HIV and viral hepatitis among people who inject drugs (PWID). SSPs are also effective conduits to deliver overdose prevention resources among PWID. In December 2015, the Kanawha-Charleston Health Department (KCHD) in West Virginia implemented a SSP; however, the program was indefinitely suspended in early 2018 following policy changes that would have forced the program to operate in ways that conflicted with established best practices.
