Voluntary sexual intercourse in an adult woman with normal vagina is not a known cause of vesicovaginal fistula (VVF), though sexual abuse or sexual intercourse with young girls before reaching physical maturity can result in VVF. Postcoital VVF in an adult woman without these predisposing factors is very infrequent. A nulliparous woman who had VVF after first intercourse is presented, as it is unusual by virtue of etiology.

The nipple and areola diameter of 498 girls aged 8-17 years were studied with the aim of finding measurable criteria for sexual maturation, including breast and pubic hair development during female puberty. The ratio of growth hormone response to clonidine and L-dopa challenge was compared in 74 boys: 15 with purported physical abuse, 7 with purported sexual abuse, 13 normal controls, and 39 psychiatric controls. Sexually abused boys demonstrate a statistically significant elevated ratio of growth hormone response to clonidine versus response to L-dopa. Physically abused boys demonstrate lower clonidine/L-dopa growth hormone response ratios compared with controls. These effects widen with increasing physical development.

A 20-year-old man was found to have rudimentary testes, microphallus and minimal evidence of sexual maturation, along with low serum testosterone and high serum gonadotropin levels, consistent with the syndrome of rudimentary testes and microphallus. A 18 year old patient presented with a female phenotype and no signs of sexual maturation. The leukocyte karyotype was XY. At laparotomy, there was no uterus and the gonadal streaks had a XY/XO mosaicism. There were rudiments of fallopian tubes and epididymis.
