A shock toxin that produces disseminated intravascular coagulation and multiple organ failure.

OBJECTIVE: To introduce a new concept in the etiology and treatment of traumatic and septic shock. It describes 3 types of shock: (1) hypovolemic shock, (2) traumatic shock, and (3) septic shock. BACKGROUND: The mortality of septic shock in both total number and mortality rate has been increasing over the past 40 years despite major advances in diagnosis and treatment, including a number of "magic bullets." Trauma is the No. 1 cause of death in persons under the age of 44 and the No. 3 cause of all deaths. Traumatic shock has been assumed to be caused by hypovolemia; however, many traumatic s

Characteristics of out-of-hospital shock care.

OBJECTIVE: Regionalization of care is a potential strategy for the management of shock. There are no data describing the regional distribution of patients with out-of-hospital shock. We sought to describe the incidence, demographic, clinical, and regional characteristics of patients with traumatic and nontraumatic medical shock treated by out-of-hospital emergency medical services. DESIGN: Descriptive study using Pennsylvania statewide emergency medical services patient care data. SETTING: Commonwealth of Pennsylvania, 2006-2008. PATIENTS: Adult (age ≥ 18 yrs) noncardiac arrest patients wit

[The efficacy of hyperbaric oxygenation in experimental and hemorrhagic shock].

The efficacy of hyperbaric oxygenation (HBO, 2 atm, 1 h, altitude chamber "Irtysh-MT") has been studied in acute experiments on 56 adult dogs with hemorrhagic and traumatic shock. It has been established that HBO has a marked therapeutic effect in hemorrhagic shock, especially in its early period, when almost all the animals are saved from death. In the late period of hemorrhagic shock and in the early period of traumatic shock HBO effect was manifested only in insignificant increase in the animals' survival and lifespan. In the late period of traumatic shock toxic effect of hyperbaric O2 appl

Clinical characteristics and course of out-of-hospital shock in a national emergency medical services cohort.

BACKGROUND: Shock from medical and traumatic conditions can result in organ injury and death. Limited data describe out-of-hospital treatment of shock. We sought to characterize adult out-of-hospital shock care in a national emergency medical services (EMS) cohort. METHODS: This cross-sectional study used 2018 data from ESO, Inc. (Austin, TX), a national EMS electronic health record system, containing data from 1289 EMS agencies in the United States. We included adult (age ≥18 years) non-cardiac arrest patients with shock, defined as initial systolic blood pressure ≤80 mm Hg. We compared pati

The value of variables of disseminated intravascular coagulation in the diagnosis of adult respiratory distress syndrome.

UNLABELLED: In an investigation on 19 patients with traumatic (n = 11) and septic (n = 8) shock, at risk of developing adult respiratory distress syndrome (ARDS), various coagulation and fibrinolysis variables and also blood gases and chest x-ray were monitored. Eight patients developed ARDS - two after traumatic shock and six following septic shock. LABORATORY FINDINGS: the conventional disseminated intravascular coagulation variables (fibrinogen, platelet counts, activated partial thromboplastin time, ethanol gelation test, thrombotest, normotest and fibrin degradation products) could not d
