High altitude retinopathy: An overview and new insights.

High altitude retinopathy (HAR) is a common ocular disorder that occurs on ascent to high altitude. There are many clinical symptoms, retinal vascular dilatation, retinal edema and hemorrhage are common. These usually do not or slightly affect vision; rarely, severe cases develop serious or permanent vision loss. At present, the research progress of HAR mainly focuses on hemodynamic changes, blood-retinal barrier damage, oxidative stress and inflammatory response. Although the related studies on HAR are limited, it shows that HAR still belongs to hypoxia, and hypobaric hypoxia plays an aggrava

Rate-dependent pacing failure after pacemaker implantation: Novel insights into the mechanism of using adenosine.

An 82-year-old woman received pacemaker implantation for sick sinus syndrome. Two days after the implantation, electrocardiography showed 2:1 atrial pacing failure, followed by a bradycardia-dependent increase in the atrial pacing threshold during a pacemaker examination. However, transient 1:1 atrial pacing capture recovered by adenosine triphosphate (ATP) administration, which was performed to evaluate the bradycardia-dependent pacing failure mechanism. We considered this phenomenon to be caused by Phase 4 depolarization and avoided replacing this atrial lead. Three weeks later, the atrial p

[The yellow fever epidemic in Saint-Nazaire in 1861].

In 1861, a brief epidemic of Yellow Fever made 26 dead among 44 sick persons. Because of the lack of knowledge in bacteriological origins it was impossible to link the illness to the role in the transmission of the virus by insects. Chiefly it was impossible to understand why the people on the shore were hit while some sailors leaving the boat were not? Why some people living to leeward of the ship were touched? Thus Melier championed the concept of transmission by an agent carried by the atmosphere. Without scientific epidemiological knowledge he was able to bring out some wise pre-emptive me

Holter monitoring in the diagnosis and management of cardiac rhythm disturbances.

Dynamic, long-term electrocardiography by the Holter technique has gained a definite and important role in cardiac diagnosis. Indications for its application include the diagnosis of ischemic heart disease, the assessment of antiarrhythmic therapy, the evaluation of function of artificial pacemakers, the investigation of physiologic correlates of known arrhythmias, the evaluation of the efficacy of antianginal therapy, and the assessment of the natural history of cardiac arrhythmias in various conditions. In our hands, the technique has proved most fruitful in the uncovering of arrhythmic corr

Betwixt and between: workplace perspectives on work reintegration in the eldercare sector in Denmark.

PURPOSE: Contemporary return-to-work (RTW) policies in Denmark and other welfare nations recommend employees on long-term sick leave, due to physical or mental health problems, to RTW gradually. Factors that influence the process of work reintegration (WR) is well documented, however, co-workers experiences of this process are a rather new research topic. Moreover, in the context of the present research, no studies have so far explored the workplace as an arena for social interaction. The aim of this study was to explore co-workers' experiences of the reintegration process and how these experi

Illusory self-location in virtual reality: The interplay of vestibular function, aging, and gaming experience.

The vestibular system plays a crucial role in body and self perception, as evidenced by the effects of artificial vestibular stimulation in healthy individuals and the depersonalization-like symptoms observed in patients with otoneurological disorders. However, its role in the bodily self-an immediate and pre-reflective form of self-consciousness-remains relatively unexplored. This study investigated the influence of bilateral vestibulopathy (BVP) on the bodily self using a full-body illusion (FBI) in virtual reality. We assessed illusory self-location and self-identification in 34 patients wi

Background sensitivity to chemotherapy-induced nausea and vomiting and response to antiemetics in paediatric patients: a genetic association study.

Chemotherapy-induced nausea and vomiting (CINV) remains a common adverse effect for children with cancer. In children, chemotherapy emetogenicity and patient factors such as susceptibility to motion sickness and age group determine a patient's risk of CINV. Besides known risk factors, genetic factors may play a role in interindividual variation in the occurrence of CINV. We investigated the influence of candidate gene polymorphisms on the efficacy of antiemetics and on the background sensitivity to CINV in children. This prospective study included 100 children with cancer (median age 6.4 years

Loss of Neuron Navigator 2 Impairs Brain and Cerebellar Development.

Cerebellar hypoplasia and dysplasia encompass a group of clinically and genetically heterogeneous disorders frequently associated with neurodevelopmental impairment. The Neuron Navigator 2 (NAV2) gene (MIM: 607,026) encodes a member of the Neuron Navigator protein family, widely expressed within the central nervous system (CNS), and particularly abundant in the developing cerebellum. Evidence across different species supports a pivotal function of NAV2 in cytoskeletal dynamics and neurite outgrowth. Specifically, deficiency of Nav2 in mice leads to cerebellar hypoplasia with abnormal foliation

Congestive Heart Failure and Thyroid Dysfunction: The Role of the Low T3 Syndrome and Therapeutic Aspects.

BACKGROUND: Both the morbidity and mortality rates from congestive heart failure (CHF) remain elevated despite the medical and non-medical management of the disease, thus suggesting the existence of residual risk factors such as thyroid dysfunction. Particularly, the 15-30% of patients with CHF, especially those with severe ventricular dysfunction, display the so-called low T3 syndrome (LT3S), which seems to negatively affect the cardiovascular prognosis. OBJECTIVE: Only a few clinical trials have been carried out to verify both the safety and the efficacy of thyroid replacement in the LT3S,

Patent Foramen Ovale: Pivotal Role of Transesophageal Echocardiography in the Indications for Closure, Assessment of Varying Anatomies and Post-procedure Follow-up.

Patent foramen ovale (PFO) is present in 15%-30% of the general population and has been associated with various pathologic states, including cryptogenic stroke, platypnea-orthodeoxia syndrome, decompression sickness and migraine with auras. Transesophageal echocardiography (TEE) has a major role in the diagnostic evaluation of PFO, as well as in the post-procedural assessment after transcatheter closure. The goals of this article were to synthesize the echocardiographic transesophageal techniques required for accurate PFO diagnosis and careful anatomic assessment of its anatomic variants, to f

Transmission Dynamics of Rhodesian Sleeping Sickness at the Interface of Wildlife and Livestock Areas.

Many wilderness areas of East and Southern Africa are foci for Rhodesian sleeping sickness, a fatal zoonotic disease caused by trypanosomes transmitted by tsetse flies. Although transmission in these foci is traditionally driven by wildlife reservoirs, rising human and livestock populations may increase the role of livestock in transmission cycles. Deciphering transmission dynamics at wildlife and livestock interface areas is key to developing appropriate control. Data are lacking for key parameters, including host distributions, tsetse density, and mortality rates, and the relative roles of l

[Better patient orientation through certified cancer centres in oncological care? Patient satisfaction in breast cancer patients in certified breast cancer centres and noncertified hospitals].

OBJECTIVES: We investigated whether patient satisfaction (including its subdimensions) among breast cancer patients depends on the place of the treatment (certified breast cancer centre vs. noncertified clinics). Furthermore, we examined the impact of patient- and disease-related factors on patient satisfaction. METHODS: 652 breast cancer patients (n = 480 from certified breast cancer centres and n = 172 from noncertified clinics) took part in the study after their hospital stay and completed a questionnaire. Data on patient satisfaction (Hamburger Fragebogen zum Krankenhausaufenthalt, HFK),

Parenting-related childhood learning history and panic vulnerability: a test using a laboratory-based biological challenge procedure.

A small but growing literature highlights specific parenting behaviors in increasing panic vulnerability among offspring. The current study examined the association between parenting-related instrumental and observational learning of sick-role behavior during childhood and reactivity to a panic-relevant biological challenge procedure that has evidenced predictive validity in terms of panic onset. Participants were 93 physically and psychologically healthy young adults (39 females; M(age)=23.41 years). As expected, results indicated that instrumental learning experiences involving (panic-releva

'Make me normal': the views and experiences of pupils on the autistic spectrum in mainstream secondary schools.

The following article reviews literature pertaining to the association between child maltreatment and self-injury and the ways it varies according to maltreatment type. Research supporting various mediators of the relations between different maltreatment types and self-injury is summarized. Informing mediator models, dominant theories of functionality, particularly affect regulation theories, are summarized and granted empirical support. Following from explanations of its functionality, three developmental pathways (regulatory, representational/interpersonal, and reactive/neurobiological) lead

The allocation of scarce medical resources across medical conditions.

This study aimed to determine if the criteria participants use to make decisions concerning scarce medical resources differ across medical conditions - from life-saving to life-enhancing surgery (heart transplant, in vitro fertilization treatment and cosmetic surgery). Participants completed three questionnaires requiring them to rank order 16 hypothetical patients in order of priority for each medical condition. Demographic information about the hypothetical patients varied on four dimensions: age, annual income, smoking behaviour and whether or not the patient had children. There were signif
