Sleep disorders and the eye.

During the past decade, associations between sleep disorders and certain ophthalmologic disorders have been increasingly recognized. To review the literature on these important associations, we conducted a PubMed search using combinations of the following terms: sleep disorders, sleep apnea, circadian rhythm disorder, continuous positive airway pressure, eye disease, floppy eyelid syndrome, glaucoma, ischemic optic neuropathy, papilledema, nocturnal lagophthalmos, and vision loss. We limited our search to articles published in English that involved human participants. All available dates were …

Sleep and trauma: an overview.

Sleep disturbance is common after traumatic events of various types, such as combat, physical trauma, and sexual abuse, and closely intertwined with Posttraumatic Stress Disorder (PTSD), a common outcome of severe and prolonged trauma. This paper reviews the current literature on the significance and characteristics of sleep disturbance occurring in the context of trauma, examines the relationship between sleep disturbance and PTSD, identifies gaps in knowledge relative to the role of sleep disturbance in trauma and PTSD, and discusses the implications of this body of knowledge for clinical pr…

Sleep apnea in the elderly.

Obstructive sleep apnea (OSA) is a common clinical condition associated with obesity. A high prevalence of sleep apnea exists in the elderly, presumably because of changes in oropharyngeal collapsibility. Elderly patients may be less likely to seek medical attention for this condition, because they are less likely to be symptomatic. Excessive daytime somnolence is a known consequence of untreated OSA, but adverse cardiovascular consequences, such as hypertension, arrhythmias, and congestive heart failure, are more serious in older patients. Continuous positive airway pressure therapy is the mo…

Effects of exercise on sleep.

Historically, perhaps no daytime behavior has been more closely associated with better sleep than exercise. The assumption that exercise promotes sleep has also been central to various hypotheses about the functions of sleep. Hypotheses that sleep serves an energy conservation function, a body tissue restitution function, or a temperature down-regulation function all have predicted a uniquely potent effect of exercise on sleep because no other stimulus elicits greater depletion of energy stores, tissue breakdown, or elevation of body temperature, respectively. Exercise offers a potentially att…

[Sleeping and eating disorders].

Over the last three decades there has been a dramatic increase in the prevalence of eating disorders (ED) in western society. The main syndromes are anorexia nervosa (AN), bulimia nervosa (BN) and non-specified eating disorders (ED-NOS). These disorders are with high morbidity and life threatening complications. Sleep disturbances are predominant symptom in these disorders. Researches have examined sleep disorders among people suffering from eating disorders, using different methods: sleep polysomnography, actigraph and self report questionnaires. The article presents the diagnostic criteria o…

Impact of sleep in COPD.

Sleep has well-recognized effects on breathing, including changes in central respiratory control, airways resistance, and muscular contractility, which do not have an adverse effect in healthy individuals but may cause problems in patients with COPD. Sleep-related hypoxemia and hypercapnia are well recognized in COPD and are most pronounced in rapid eye movement sleep. However, sleep studies are usually only indicated in patients with COPD when there is a possibility of sleep apnea or when cor pulmonale and/or polycythemia are not explained by the awake PaO(2) level. Management options for pat…

The sleep needs of adolescents.

The amount of sleep affects the way adolescents perform, feel, think, learn, and remember. Significant physiological and social changes have profound effects on adolescents' sleeping patterns. Lack of sleep increases the possibility of increased daytime sleepiness, which may result in a tragic automobile or work accident. Other consequences of sleep deprivation include poor school performance, heightened risk of drug and alcohol use, increased irritability, and aggressive behavior, all of which can interfere with relationships with classmates, parents, and teachers. The purpose of this paper i…

The development and disorders of sleep.

Sleep-related problems are extremely frequent sources of parental concern and question. Many childhood sleep difficulties are simply minor behavioral problems, whereas other sleep changes represent serious disorders. Increased knowledge about the development and disorders of sleep has enormous value in making accurate diagnoses and effective interventions. Overviews are presented on common and important sleep disorders in infancy, childhood, and adolescence, including night walking, partial arousal, sleep-disordered breathing, narcolepsy, and sleep loss secondary to late and erratic schedules.…

Effects of hormones on sleep.

Administration of hormones to humans and animals results in specific effects on the sleep electroencephalogram (EEG) and nocturnal hormone secretion. Studies with pulsatile administration of various neuropeptides in young and old normal controls and in patients with depression suggest they play a key role in sleep-endocrine regulation. Growth hormone (GH)-releasing hormone (GHRH) stimulates GH and slow wave sleep (SWS) and inhibits cortisol, whereas corticotropin-releasing hormone (CRH) exerts opposite effects. Changes in the GHRH:CRH ratio contribute to sleep-endocrine aberrations during norm…

[Sleep disorders in the elderly].

Insomnia dominates the sleep disorders of the elderly. Sleep apnea, the restless legs syndrome and nocturnal agitation represent other facets. After eliminating the possibility of painful or degenerative organic disease, iatrogenic cause and mode of life, other symptoms should be sought, namely mental and personality-related. Investigation requires polysomnogram recording, supported by careful history-taking, including objective and subjective factors. Treatment includes notions of diet, biological rhythm and desynchronisation. It should take into consideration the deleterious effects of prolo…

Sleep in the elderly patient.

Sleep problems in the elderly are extraordinarily common. The authors discuss normal sleep, changes in sleep with normal aging, and sleep disorders in the elderly, focusing on sleep-disordered breathing. In addition, nocturnal myoclonus and rapid-eye movement behavior disorder are reviewed.

Sleep and respiration: a postscript.

The recent advances in our understanding of breathing in sleep include an evolution in the selection and surgical therapy of patients with the sleep apnea syndrome. Recent work suggests that shorter polysomnographic studies may be adequate for diagnostic purposes in many sleep apnea patients. It is now clear that central apnea may occur, paradoxically, in patients with either very blunted chemical drives to breathe or increased drives to breathe.

Sleep disorders in the elderly.

Sleep disorders are difficult to distinguish from changes in sleep typically occurring after age 60. Sleep laboratory evaluation is needed to differentiate normal sleep alterations from the similar but more severe symptoms of depression or organic brain syndrome, and to detect sleep apnea, the most common sleep disorder found in geriatric patients. This disorder may be fatal, especially if a hypnotic drug is prescribed. Since biologic rhythms are easily disturbed in the elderly, education about sleep habits may be the most helpful therapy.
