Activity of pontine neurons during sleep and cataplexy in hypocretin knock-out mice.

Narcolepsy is a human sleep disorder resulting from the loss of neurons containing the neuropeptide orexin, also known as hypocretin. Cataplexy, which is a sudden loss of muscle tone during waking, is an important diagnostic symptom of narcolepsy. In humans and canines with narcolepsy, cataplexy is considered to be a separate and distinct behavioral state. However, in the mouse model of the disease this issue is not resolved. The present study monitored the activity of forty four neurons in the rostral pons in hypocretin knock-out mice. Majority of the neurons were active during wake and REM s

Small platform sleep deprivation selectively increases the average duration of rapid eye movement sleep episodes during sleep rebound.

The single platform-on-water (flower pot) method is extensively used for depriving rapid eye movement sleep (REMS). Detailed comparison of sleep-wake architecture, recorded during the rebound period after spending three days on either a small or large platform, could separate the effects of REMS deficit from other stress factors caused by the procedure. A further aim of the study was to find the most characteristic REMS parameter of the rebound originating from REMS deficit. Rats were kept on a small or large platform for 72 h. Their fronto-parietal electroencephalogram, electromyogram and mot

REM disruption and REM vagal activity predict extinction recall in trauma-exposed individuals.

BACKGROUND: Accumulating evidence suggests that rapid eye movement sleep (REM) supports the consolidation of extinction memory. REM is disrupted in posttraumatic stress disorder (PTSD), and REM abnormalities after traumatic events increase the risk of developing PTSD. Therefore, it was hypothesized that abnormal REM in trauma-exposed individuals may pave the way for PTSD by interfering with the processing of extinction memory. In addition, PTSD patients display reduced vagal activity. Vagal activity contributes to the strengthening of memories, including fear extinction memory, and recent stud

Genetic architecture of sleep in a genome wide association study of device measured sleep traits.

Sleep is essential for health and regulated by genetic and environmental factors. We perform genome-wide association studies of device-measured sleep duration, efficiency, and accelerometer-derived rapid eye movement (REM) and non-rapid eye movement (NREM) sleep in 80,013 UK Biobank participants. We identify 20 autosomal loci, 12 of which have not been previously reported, including genome-wide significant associations for REM and NREM sleep duration. MEIS1 shows strong opposing effects on REM and NREM durations and is intolerant to loss-of-function mutations, suggesting an essential role in t

The diagnostic value of power spectra analysis of the sleep electroencephalography in narcoleptic patients.

OBJECTIVE: Manifestations of narcolepsy with cataplexy (NC) include disturbed nocturnal sleep - hereunder sleep-wake instability, decreased latency to rapid eye movement (REM) sleep, and dissociated REM sleep events. In this study, we characterized the electroencephalography (EEG) of various sleep stages in NC versus controls. METHODS: EEG power spectral density (PSD) was computed in 136 NC patients and 510 sex- and age-matched controls. Features reflecting differences in PSD curves were computed. A Lasso-regularized regression model was used to find an optimal feature subset, which was valid

Isolated rapid eye movement sleep without atonia in amyotrophic lateral sclerosis.

OBJECTIVE: The aim of this study was to quantitatively analyze, with the most recent and advanced tools, the presence of periodic leg movements during sleep (PLMS) and/or rapid eye movement (REM) sleep without atonia (RSWA), in a group of patients with amyotrophic lateral sclerosis (ALS), and to assess their eventual correlation with the clinical severity of the disease. METHODS: Twenty-nine ALS patients were enrolled (mean age 63.6 years) along with 28 age-matched "normal" controls (mean age 63.8 years). Functional impairment due to ALS was evaluated using the ALS-Functional Rating Scale-Rev

The impact of antiseizure medications on polysomnographic parameters: a systematic review and meta-analysis.

BACKGROUND: Oral antiseizure medications (ASMs) are first-line treatments for patients with epilepsy. However, ASMs may alter sleep architecture, adversely affecting patient outcomes. The meta-analysis aimed to elucidate the effect of ASMs on sleep architecture. METHODS: PubMed, Embase, and Cochrane Central database (up to Febrary 2021) were searched for randomized control trials (RCT) with effects of ASMs on polysomnography parameters. A meta-analysis using a random-effects model was performed. We did not set limitation to the participants with underlying diagnosis of epilepsy. RESULTS: Eig

Association between obstructive sleep apnea and metabolic profiles across sleep stages in pediatrics.

OBJECTIVE: To evaluate the association between metabolic profiles and obstructive sleep apnea (OSA) during rapid eye movement (REM) and non-REM (NREM) sleep in children. METHODS: A total of 514 children underwent overnight polysomnography (PSG) and biochemical testing. Obstructive apnea hypopnea indices (OAHI) and oxygen desaturation indices (ODI) were recorded for total sleep time (TST), REM, and NREM. A multivariate analysis of covariance model assessed the associations of OAHITST, OAHINREM, and OAHIREM with overall metabolic parameters, adjusting for body mass index (BMI) z-score and other

Propagation effects of abnormal beta oscillations on sleep rhythms in Parkinson's disease: A computational study.

Parkinson's disease (PD) is characterized by abnormal beta oscillations (13-30 Hz) within the basal ganglia, which contribute not only to motor symptoms but also to sleep disturbances. In this study, we developed a computational model of the basal ganglia-thalamocortical (BGTC) network that includes the pedunculopontine nucleus (PPN), to investigate the mechanism by which the abnormal oscillations disrupt sleep. The model incorporates key nuclei, neurotransmitter systems, and neural pathways to simulate sleep-related brain activity in Parkinsonian conditions. Our simulations show that abnormal

Relevance of sleep and associated structural changes in GBA1 mouse to human rapid eye movement behavior disorder.

Rapid eye movement (REM) sleep behaviour disorder (RBD) is a REM parasomnia that often predicts the later occurrence of alpha-synucleinopathies. Variants in the gene encoding for the lysosomal enzyme glucocerebrosidase, GBA, strongly increase the risk of RBD. In a GBA1-mouse model recently shown to mimic prodromal stages of α-synucleinopathy, we now demonstrate striking REM and NREM electroencephalographic sleep abnormalities accompanied by distinct structural changes in the more widespread sleep neurocircuitry.

Sleep in human narcolepsy revisited with special reference to prior wakefulness duration.

Sleep of 11 narcoleptic subjects was recorded on baseline and after 16 and 24 hours of prior wakefulness (16 and 24 hours sleep deprivation). Eleven sex- and age-matched control subjects were recorded for comparisons. All recordings in narcoleptic subjects were characterized by frequent sleep onset rapid eye movement (REM) episodes, increased amounts of wake time after sleep onset and low sleep efficiencies. Mean total sleep time (TST) was significantly decreased in narcoleptic subjects after sleep deprivation (SD). Recovery sleep after 24 hours SD showed reduced nonREM (NREM) sleep stage 2 pe

The electroencephalographic sleep pattern in schizophrenic patients treated with clozapine or classical antipsychotic drugs.

The effects of antipsychotic agents on sleep were tested by examining the nocturnal electroencephalographic recordings of 14 drug-naïve schizophrenic patients and comparing these with recordings from 12 patients treated with clozapine and 10 treated with classical neuroleptics (haloperidol: n = 7; flupentixol: n = 3). In both of the treated groups, sleep continuity measures and rapid eye movement (REM) density were significantly higher than in the drug-naïve group. Clozapine-treated patients showed significantly more stage two sleep, more stable non-REM sleep (stages two, three and four) and l

REM-OSA vs NREM-OSA in Children: Separate Clinical Phenotypes or Markers of OSA Severity.

INTRODUCTION: Obstructive sleep apnea (OSA) occurs predominantly during REM sleep (REM-OSA) compared to NREM sleep (NREM-OSA) in both children and adults. However, it is not clear whether REM-OSA and NREM-OSA are separate clinical phenotypes of OSA or represent 2 ends of OSA severity spectrum. OBJECTIVE: We aimed to compare clinical and polysomnography (PSG) characteristics of REM-OSA versus NREM-OSA in children, and to evaluate the effect of these characteristics on REM-related over NREM-related obstructive AHI ratio (REM-OAHI/NREM-OAHI). METHODS: Clinical and PSG data of all children diagn

Sleep respiratory concomitants of comorbid panic and nightmare complaint in post-traumatic stress disorder.

Posttraumatic stress disorder (PTSD) patients with comorbid panic disorder (PD) may express additive symptoms of central fear system disturbance. They endorse elevated levels of sleep and nightmare disturbance [Leskin GA, et al., J Psychiatr Res 2002;36:449-452], and demonstrate movement suppression during laboratory sleep [Woodward SH, et al., Sleep 2002;25:681-688]. We estimated respiratory rate and rate variability separately for rapid-eye movement (REM) and non-rapid-eye movement (NREM) sleep. Subjects were 49 Vietnam combat-related PTSD inpatients (11 with comorbid PD and 38 without) and

Rapid eye movements, muscle twitches and sawtooth waves in the sleep of narcoleptic patients and controls.

Seventeen unmedicated patients with narcolepsy-cataplexy and 17 age- and sex-matched controls were recorded polygraphically for 3 consecutive nights. Rapid eye movements (REMs), m. mentalis twitches and sawtooth waves in the EEG were visually scored. REM and twitch densities during REM sleep were significantly higher in the patients than in the controls. The distribution pattern of REMs and twitches was altered in the patients: twitch density peaked in the first REM period and density of REMs showed an even distribution across all the REM periods of the night. In the controls both REM and twit

Emotional Processing in a Nap Paradigm: The Role of Sleep Stages in Memory and Reactivity.

It is widely known that sleep has a role in modulating emotional processing. However, while sleep facilitates the consolidation of emotional memories, its influence on emotional reactivity remains unclear. Most prior work has not fully addressed emotional reactivity, often relying solely on subjective or physiological measures, nor has it disentangled the contribution of different sleep stages. Here, using a between-subjects design, we asked participants to complete two picture-based tasks assessing emotional memory and emotional reactivity before and after a manipulation condition (nap, N = 4

Hypersomnia in bipolar depression: a comparison with narcolepsy using the multiple sleep latency test.

OBJECTIVE: This study characterized objectively the hypersomnia frequently seen in the depressed phase of bipolar affective disorder. On the basis of previous work in sleep and affective disorders, it has been hypothesized that the hypersomnia is related to greater REM sleep. This hypothesis was tested by using a multiple sleep latency test to compare bipolar affective disorder with narcolepsy, a well-defined primary sleep disorder associated with known REM sleep dysfunction. METHOD: Twenty-five bipolar depressed patients were selected on the basis of complaints of hypersomnia. They underwent

Forty-eight-hour polysomnographic evaluation of narcolepsy.

Eleven narcoleptic and 11 control subjects completed five multiple sleep latency test (MSLT) procedures with instructions to "fall asleep" while supine and five maintenance of wakefulness test (MWT) procedures with instructions to "remain awake" while comfortably sitting. Narcoleptic subjects had a shorter sleep latency and a higher frequency of sleep onset REM periods (SOREMPs) on both daytime tests than controls. Each group had a longer sleep latency on the MWT than MSLT. For patients with narcolepsy, the differences between the two daytime procedures suggest the tests measure distinct aspec

Association between body mass index and sleep duration assessed by objective methods in a representative sample of the adult population.

INTRODUCTION: Sleep duration has been associated with overweight individuals in many epidemiological studies; however, few studies have assessed sleep using objective methods. Our study was designed to evaluate the association between body mass index (BMI) and sleep duration measured by actigraphy (Acti), polysomnography (PSG) and the Pittsburgh sleep quality index questionnaire (PSQIO). Furthermore, we evaluated other biochemical and polysomnographic parameters. METHODS: A representative sample of 1042 individuals from Sao Paulo, Brazil, including both genders (20-80 yrs), participated in ou

Influence of Surgical-Level Propofol Anesthesia on Non-Rapid Eye Movement and Rapid Eye Movement Sleep in Aged Mice.

BACKGROUND: Clinical studies indicate that sedation with propofol is associated with loss of rapid eye movement (REM) sleep, whereas animal experiments suggest that propofol anesthesia may be functionally equivalent to naturally occurring sleep. However, most preclinical studies of propofol's effects on sleep have used sedative doses in young animals, despite the frequent use of high doses in the operating room and the increased susceptibility of older individuals to sleep disturbances. Here, we investigated how surgical levels of propofol anesthesia affect sleep in aged mice. We hypothesized
