Childhood fibrous dysplasia presenting as blindness: a skull base approach for resection and immediate reconstruction.

Fibrous dysplasia is an abnormal fibroosseous process of bone of unknown cause. The incidence of skull involvement varies, painless enlargement being the most common presenting symptom. Change in vision is a rare but recognized finding. We report a 3-year-old boy with extreme fibrous dysplasia involving the skull base, who presented with blindness. He underwent exposure osteotomies of the frontal bones and orbits to provide access for skull base tumor removal. The orbital roofs were reconstructed with microplate-fixed cranial grafts. One and one half years after tumor excision followed by imme

[Development of reconstruction techniques of the anterior skull base after tumor excision].

183 benign and malignant tumors of the anterior cranial base were operated in the department of neurosurgery at Ste Anne's hospital, from June 1982 to December 1993. The bone defect was repaired first with bone sheets taken from the inner table of the cranial flap. Then allografts of irradiated bone and madreporic coral blocks were used as reconstruction material. Since January 1992, no bone reconstruction was performed; the anterior base was just covered with a sheet of Vicryl Composit Mesh (VCM) which was secured bilateraly on the pterion, the temporalis pars squamosa and around the ethmoid

Transforniceal lateral deep bone decompression--a modified technique to prevent postoperative diplopia in patients with disfiguring exophthalmos due to dysthyroid orbitopathy.

BACKGROUND/PURPOSE: Postoperative diplopia remains a significant complication of orbital decompression in dysthyroid orbitopathy. This study evaluated the results of orbital decompression treatment using a transforniceal approach to sculpt the lateral orbital deep bone area. METHODS: The two areas of bone in the deep lateral orbit (the basin of the inferior orbital fissure and the sphenoid door jamb) were exposed using a transforniceal swinging eyelid approach. An electric drill was used to sculpt these deep bone areas of the lateral orbit, and approximately 1 mL intraconal fat was removed si

Coronoid-temporalis pedicled rotation flap for middle skull base reconstruction.

We report on a patient who underwent maxillectomy and subsequent coronoid-temporalis flap reconstruction of the surgical defect to treat a maxillary sinus carcinoma invading the middle skull base. The patient underwent successful reconstruction of the middle skull base and displayed acceptable functional result. Middle skull base defects can be successfully repaired using a local coronoid-temporalis pedicled flap. The coronoid process of the mandible is advantageous as a local flap because it is close to the skull base defects and can easily be dissected from adjacent tissues. The use of a cor

Diffuse Basisphenoid Enhancement: Possible Differentiating Feature for Granulomatous Hypophysitis.

BACKGROUND AND PURPOSE: Granulomatous hypophysitis is a rare inflammatory condition of the pituitary gland with an imaging appearance that can overlap with that of pituitary adenoma. Differentiating the two before surgical resection can have important treatment implications. The purpose of our study was to determine whether it was possible to differentiate between granulomatous hypophysitis and pituitary adenoma on the basis of diffuse enhancing infrasellar basisphenoid bone marrow. MATERIALS AND METHODS: We present 3 cases, initially thought to be pituitary adenomas, that were pathology-prov…

A Rare and Atypical Manifestation of Intraosseous Hemangioma in the Zygomatic Bone.

Intraosseous hemangiomas (IH) are rare intrabony lesions that represent less than 1% of intraosseous tumors. IH are mostly seen in the axial skeleton and skull. Most commonly, the frontal bone, zygomatic, sphenoid, maxilla, ethmoid, and lacrimal bone can manifest IH. Currently, IH is classified as a developmental condition of endothelial origin. According to WHO, the five histological types of IH are cavernous, capillary, epithelioid, histiocytoid, and sclerosing. IH of the zygoma is an extremely rare condition with female predominance. A systematic review recently estimated that there were 78

Study on measurements for margin of safety in lateral operation of a transnasoethmoid-sphenoid approach to decompress the optic canal.

OBJECTIVE: The surgical approach through nasal and ethmoid cells to sphenoid sinus is a common clinical method to do the decompression of the optic canal. During this surgery, we can follow landmarks such as ethmoidal arteries. However, when we do the surgery, it is possible to open excessively in lateral operation and damage orbital medial wall. To prevent this from happening, we do a lot of measurements to estimate the distance in lateral operation. We hope to give surgeons some help in clinical use. METHODS: We chose 120 brain imaging findings that showed a normal appearance in our measuri

Growth of the cranial base in craniosynostosis.

The configuration of the neurocranium has long been used as a diagnostic tool in assessing infants with abnormal head shape. In the case of craniosynostosis, a characteristic shape is caused by a constraint placed on growth of the neurocranium by prematurely closed sutures and secondary accommodation to that constraint. This investigation is a preliminary test of our hypotheses of growth of the cranial base under these constraints. Three dimensional landmark coordinate data were collected from pre-, peri-, and postoperative CT scans of eleven patients from The Cleft Palate and Craniofacial Def

[The rhinosurgical management of nasal liquorrhoea (author's transl)].

Rhinoliquorrhoea is the leading symptome of an osteodural defect of the base of the skull, the pneumatic system of the rhinobasis being affected. Surgical attention to these defects is largely the responsibility of the rhinosurgeon. Following a description of the under-lying causes of this condition in the patients treated by us, details are given of the procedures undertaken to establish diagnosis of liquorrhoea, its localization. The indications, timing and technique of rhinosurgical treatment are discussed. Finally, some characteristic clinical cases are reported.

Timing of Spheno-Occipital Synchondrosis Closure in Cleft Lip and Palate Patients in Iranian Population.

This study aimed to evaluate the chronological age range associated with each stage of spheno-occipital synchondrosis (SOS) fusion in patients with cleft lip and palate compared to the noncleft group, using cone-beam computed tomography (CBCT) images. In this study, the degree of SOS fusion was assessed using a 4-stage scoring system on CBCT images of 190 individuals (92 patients with cleft lip and palate, 98 noncleft individuals). A χ2 test was performed to assess the correlation between age and fusion stage. The independent sample t tests were used to evaluate the differences in the mean val

Optic atrophy in thalassemia intermedia.

A 21-year-old man with thalassemia intermedia presented with progressive decrease in vision and was found to have severe bilateral optic atrophy. Orbital and brain neuroimaging revealed massive diffuse expansion of the diploe of the craniofacial bones, narrowing of the optic canals, and expansion of hematopoietic tissue in the sellar region with compression of the optic chiasm. Although increased bone marrow is a well-known consequence of thalassemia, optic atrophy due to and such marked extramedullary expansion of hematopoietic tissue is a very rare phenomenon. Decompression of extramedullary
