Neoadjuvant chemotherapy in the setting of locally advanced olfactory neuroblastoma with intracranial extension.

Olfactory neuroblastoma (esthesioneuroblastoma) is a rare malignant tumor of neuroectodermal origin. With only about 1,000 cases reported, there are no clear guidelines regarding management of this disease. Intracranial extension and orbital involvement have been shown to be independent risk factors associated with poorer outcomes. We hereby report a case of a 46-year old male presented with an 8-month history of progressive nasal obstruction and intermittent right-sided epistaxis associated with anosmia and increased pressure sensation in and around the right eye. Further evaluation revealed

MRI features of craniofacial fibrous dysplasia.

A 13-year-old female with a 4-year history of monostotic fibrous dysplasia had noticed a progressive proptosis of the right eye and diplopia on upward gaze for 4 weeks. A few years previously an incisional biopsy of the skull had verified the presumed diagnosis of fibrous dysplasia with recurrent bleeding into pathologic cystic bony structures of the skull. The patient was known to have craniofacial fibrous dysplasia with involvement of the frontal and intermediate cranial base, the posterior ethmoidal labyrinth, and the sphenoidal and maxillary sinuses. Eye examination showed a reduced visual

Surgical management of nasopharyngeal angio-fibroma.

Seventy cases of nasopharyngeal angio-fibroma are reported. Most of the patients were young adolescents between 13-24 years of age. 91% were males. The main symptoms were nose blockage and nasal bleeding, the latter was present in all the patients but the amount of blood varied from a few milliliters a time to a few hundreds of milliliters. Although angio-fibroma is benign, it invades the surrounding structures, especially the sphenoidal sinus, maxillary sinus and the ethmoidal sinuses, by expansion or by erosion. In 1/7th of the cases he palate is pushed downwards or bulges. Surgery may be th

Indications and Limits of Endoscopic Endonasal Approaches in Pediatric Neurosurgery.

Endoscopic endonasal approaches (EEAs) are a safe and effective method for treating skull base lesions, including those in the sellar and parasellar areas. Pioneered initially for the treatment of skull base pathology in adults, these approaches have shown clear advantages including cost-effectiveness, limited blood loss, and improved visualization allowing for a better plane of dissection and clearing anatomical landmarks. However, in younger patients, this approach could be limited due to their developing anatomy, small anatomical corridors, or peculiarities of specific lesions which could n

Sinonasal Glomangiopericytoma: A Report of Two Cases.

Sinonasal glomangiopericytoma (GPC) is a rare mesenchymal tumor that is seen exclusively in the sinonasal region, which shows perivascular myoid differentiation. Here, we describe two cases of sinonasal glomangiopericytoma. The first case was a 62-year-old female patient with right-sided nasal obstruction and headache. Endoscopic examination revealed a polypoid mass in the right nostril. Surgical removal revealed spindle cell proliferation with staghorn vasculature surrounded by perivascular hyalinization beneath an intact ciliated respiratory epithelium. Tumor cells were reactive to smooth mu

Preoperative Embolization for Sinonasal Angiosarcoma Optimized Intraoperative and Postoperative Outcomes: A Case Report.

BACKGROUND: Angiosarcoma is a rare and aggressive malignancy, representing less than 1% of head and neck cancers. Primary sinonasal angiosarcoma is exceedingly uncommon, with fewer than 40 cases reported. It carries a poor prognosis due to early recurrence and metastatic potential, and no standard treatment exists. CASE REPORT: A 33-year-old male presented with a 2-year history of progressive nasal obstruction, later accompanied by epistaxis, facial pain, and headache. Imaging revealed a large vascular sinonasal mass extending into the nasopharynx, sphenoid, and ethmoid sinuses with bone dest

Radiotherapy of sinonasal cancers: 2025 update.

Sinonasal cancers, including tumours of the nasal cavity, ethmoid, maxillary, sphenoid, and frontal sinuses, account for 3 to 5 % of head and neck malignancies. While the majority are carcinomas, other histologic types such as mucosal melanomas, sarcomas, and lymphomas may also occur. These tumours often present with non-specific symptoms and exhibit a heterogeneous prognosis. The Réseau d'expertise français des cancers ORL rares (the French expert network for rare head and neck cancers), accredited by the Institut national du cancer (the French national cancer institute), is responsible for m

The use of balloon dilation in revision sinus surgery.

BACKGROUND: Recent studies have demonstrated a dramatic increase in the use of balloon sinus dilation (BSD) in the United States. However, the use of BSD specifically in revision sinus surgery has not been investigated. This study addresses the question of how BSD is utilized as a tool in revision sinus surgery. METHODS: Data from MarketScan (Truven Health) over a 5-year period (2012-2016) were analyzed. Patients who underwent a sinus procedure with a minimum of 2 years of follow-up were included. RESULTS: A total of 62,304 patients met inclusion criteria; 6847 (10.99%) underwent revision. A

One and a half syndrome following penetrating head injury: Case report.

The authors report a case of a 22-year-old otherwise healthy female who presented following a head injury during a bar altercation, with no associated loss of consciousness and an unknown mechanism of injury. Examination revealed an isolated 1cm laceration on the right upper eyelid, superior to her medial canthus. She experienced diplopia on right horizontal gaze due to a left internuclear ophthalmoplegia (INO) with an associated left conjugate horizontal gaze palsy, collectively described as a left one-and-a-half syndrome. CT and MRI demonstrated evidence of a deep penetrating injury above th

Rate of recurrence and malignant transformation in 88 cases with inverted papilloma between 1998-2008.

Conclusions The recurrence rate of IP in the current study is comparable to international and national studies. When the primary tumour was located in the frontal or the sphenoid sinus, the tumour seemed more likely to recur. Some recurrences were more than 2 years after primary surgery, suggesting continued need for long-term follow-up. The rate of malignant transformation in inverted papilloma cases has not changed in Denmark. Objectives Inverted papillomas are benign tumours located in the nasal cavity and paranasal sinuses. They are known for their tendency to recur after surgery and show

Chronic hypertrophic polypoid rhinosinusitis.

Chronic hypertrophic polypoid rhinosinusitis is a condition in which the mucous membrane lining the nose and paranasal sinuses becomes markedly hypertrophied, and nasal polyps become evident. Superimposed infection is common. Bony erosion, sometimes marked, may also result. Because of the alterations in bone, the condition may be mistaken for other more serious lesions.

Allergic Aspergillus sinusitis: concepts in diagnosis and treatment of a new clinical entity.

The clinical features in 15 young adult patients with allergic Aspergillus sinusitis includes a history of asthma, recurrent nasal polyps, and radiographic evidence of pansinusitis. Multiple surgical procedures were performed on 12 patients. The histologic diagnosis is made on the mucinous material which contains eosinophils, fungal hyphae (Aspergillus species), and Charcot-Leyden crystals. Mycetomas, direct mucosal and soft tissue invasion by fungi, were not observed. Immunologic findings include an immediate cutaneous reactivity to Aspergillus in 60% of patients, total serum IgE elevation in

Sinonasal Neoplasia - Clinicopathological Profile And Importance of Computed Tomography.

BACKGROUND: Nasal cavity and Paranasal sinus malignancies are very rare, in which maxillary sinus is the commonest, followed by ethmoid, frontal and sphenoid sinus. Computed Tomography (CT) & Magnetic Resonance Imaging (MRI) play a key role in diagnosis, staging and management of paranasal sinuses and nasal pathologies. Multiplanar imaging in CT helps better imaging of critical anatomical areas. Aim of our study was to study the incidence, clinical features, CT features and its importance in the management of sinonasal neoplasms. MATERIALS AND METHODS: This prospective study was carried out i

Quantitative and qualitative 5-aminolevulinic acid-induced protoporphyrin IX fluorescence in skull base meningiomas.

OBJECT: Complete resection of skull base meningiomas provides patients with the best chance for a cure; however, surgery is frequently difficult given the proximity of lesions to vital structures, such as cranial nerves, major vessels, and venous sinuses. Accurate discrimination between tumor and normal tissue is crucial for optimal tumor resection. Qualitative assessment of protoporphyrin IX (PpIX) fluorescence following the exogenous administration of 5-aminolevulinic acid (ALA) has demonstrated utility in malignant glioma resection but limited use in meningiomas. Here the authors demonstrat

Intravenous voriconazole after toxic oral administration.

In a male patient with rhinocerebral invasive aspergillosis, prolonged high-dosage oral administration of voriconazole led to hepatotoxicity combined with a severe cutaneous reaction while intravenous administration in the same patient did not. High concentrations in the portal blood precipitate liver enzyme abnormalities, and therefore, oral administration of voriconazole may have a hepatotoxicity profile different from that of intravenous (i.v.) administration. Intravenously administered voriconazole might still be an option after oral-voriconazole-induced toxicity has resolved.

[X-ray computed tomographic study of the ethmoid and middle meatus. II. Radio-anatomy (axial incidence) and morphological variations].

Chronic rhinosinusitis endoscopic surgery needs an accurate evaluation of diseases and paranasal sinus anatomic variations. High resolution CT by thin section (2 mm) allow this pre-operative assessment. An anatomical study of the ethmoid air cell system is always possible in axial plane. The bidimensional CT exploration (axial and coronal plane) displays the anatomic variations of ostiomeatal unit, that have been reported to predispose sinusitis. They are nasal septal deviation, pneumatization and paradoxical curvature of the middle turbinate, pneumatization of unciform process, Haller's cells
