Does spinal morphology influence the occurrence of low back pain? A retrospective clinical, anthropometric, and radiological study.

OBJECTIVE: Insurance companies are showing considerable interest for measurement devices claimed to be effective in evaluating risks associated with occupational or sporting activities. The aim of this study was to evaluate the potential influence of spinal morphology on the risk of low back pain. METHODS: 1500 male construction work apprentices aged 16 to 23 years underwent spinal measurements at the Caen Teaching Hospital from 1991 to 1996. The 384 subjects with abnormal measurements or a history of spinal pain underwent plain radiography and a second set of spinal measurements done by a di

Variations in the prevalence of scoliosis by age, sex, geographic region, and subtype among Chinese children: A systematic review and modelling study.

BACKGROUND: Scoliosis is a prevalent spinal condition typically detected in children, yet its prevalence in China has not been fully understood. This study aimed to estimate the prevalence of scoliosis among Chinese children and explore its variations by age, sex, geographic region, and subtype. METHODS: We conducted a comprehensive search of seven bibliographic databases to identify epidemiological studies that reported the prevalence of scoliosis among Chinese children published since 1990. Scoliosis was defined as a Cobb angle equal to or greater than 10°, as detected radiographically. We

Novel risk factors and personalized risk calculator for predicting proximal junctional kyphosis after adult spinal deformity surgery.

AIMS: Proximal junctional kyphosis (PJK) is a prevalent and detrimental complication associated with corrective surgery for adult spinal deformity (ASD). While existing predictive models have been able to predict PJK, they have lacked individualized risk factor assessment, and the inclusion of non-modifiable predictive variables has limited their clinical usefulness. Thus, this study aimed to identify novel risk factors and establish a personalized risk calculator for PJK. METHODS: We conducted a retrospective analysis of consecutive patients who underwent realignment surgery for ASD between

Postoperative Ileus: Old and New Observations on Prevention and Treatment in Adult Spinal Deformity Surgery.

BACKGROUND: The reported incidence of postoperative ileus (POI) after spine surgery depends on the surgical approach and definition used. It is therefore possible that the overall incidence is substantially higher than previously thought. POI has consequences for both the patient and hospital resources, and can significantly increase health care costs. METHODS: We retrospectively reviewed all patients aged 18 years or older who underwent elective complex spine surgery at our tertiary referral institution from 2011 through 2017. Preoperative comorbidities, operating time and approach, estimate

[Sagittal Balance of the Spine--Clinical Importance and Radiographic Assessment].

Sagittal deformities of the spine frequently result in back pain, as patients have to expend much energy in compensation. The sagittal alignment of the spine is defined by its curvatures (lordosis and kyphosis) relative to the position of the pelvis. Diagnostic assessment is based on full spine a. p. and lateral X-rays. The sagittal balance is primarily described by different angles that can be measured, e.g. lumbar lordosis, pelvic incidence, pelvic tilt and thoracic kyphosis. The quality of life can best be estimated by subtracting lumbar lordosis from the pelvic incidence. However, initial

Total vertebrectomy for stabilisation of chronic spinal lumbar luxation in a paraplegic dog without nociception.

An adult male crossbred dog was referred with a history of a road traffic accident that took place 1 month earlier. Neurological examination revealed paraplegia with absent nociception in the pelvic limbs. On epaxial palpation, significant curvature of the anatomical axis of the spine between the third and fourth lumbar vertebrae was observed, with the presence of a bone end almost piercing the dog's skin. Survey radiographs of the lumbar spine revealed severe dislocation between L3 and L4 vertebrae. During surgery, the spinal cord was not visible between the dislocated segments. Because of di

Relief of postural orthostatic tachycardia syndrome with chiropractic rehabilitation.

Postural orthostatic tachycardia syndrome (POTS) is a form of dysautonomia that's characterised by an abnormal heart rate response to a positional change. A 50-year-old female presented with lightheadedness, palpitation, and neck pain for 3 years. Lightheadedness and palpitation occurred when shifting from a prolonged seated or lying position. Standing radiographs showed spinal misalignment with cervical kyphosis and thoracic scoliosis. Continuous static stress on these minor variants could aggravate biomechanical and autonomic disorder like POTS. After ruling out cardiovascular, neurological,

Neuromuscular scoliosis.

Scoliosis is a common deformity in many types of neuromuscular disease. Severe spinal curvature can cause difficulty in sitting. Conservative and surgical treatment of neuromuscular scoliosis differs from idiopathic scoliosis, being more complex and with a higher complications rate. Non-surgical measures rarely fully control progressive scoliosis, but aim to prevent spinal deformities secondary to muscular hypotonia or contracture. Twenty-four hour bracing should be adjusted throughout growth, and may induce functional impairment and loss of independence. Corrective surgery requires multidisci

Bracing for neuromuscular scoliosis: orthosis construction to improve the patient's function.

PURPOSE: To present technical particularities of construction and application of trunk orthosis in non-ambulatory patients with severe neuromuscular spinal deformity. Unlike in the idiopathic scoliosis, the aim of bracing was not morphological (decrease of Cobb angle) but functional. METHODS: Specially designed orthosis, resting against the seat, not leaned against the pelvic, partially suspending the trunk, was applied to 45 children aged 2.0 - 15.0 years. The construction of the orthosis, based on the achievements of the Society of Scoliosis Orthopaedic and Rehabilitation Treatment SOSORT,

[Sacral bone tuberculosis in a 6 year-old child].

BACKGROUND: Spinal tuberculosis is now rare in developed countries; its appearance in sacral vertebrae is exceptional. CASE REPORT: A 6 year-old boy was admitted because he had suffered from lumbar pain and rigidity for 24 hours, limiting anterior flexion. X-rays of the spine was normal, but a CT-scan showed lytic lesions of the sacrum. Bone scintigraphy showed no inflammatory lesions. Ewing sarcoma was diagnosed: surgical biopsy showed a cold abscess containing many bacilli that where resistant to decoloration with acid-alcohol. They were cultured for 12 days and identified as Mycobacterium

Unplanned reoperations due to implant-related complications in older versus non-older patients following adult spinal deformity surgery: A retrospective cohort study using a multi-center database.

BACKGROUND: Adult spinal deformity (ASD) surgery involves instrumentation, which can lead to implant-related complications (IRCs) and unplanned returns to the operating room (UPROR). Older patients often have risk factors like osteoporosis and multiple comorbidities, but research on how age-related factors affect IRCs and UPROR across age groups is limited. The aim of this study was to evaluate the characteristics of UPROR due to IRCs between older and non-older patients following ASD surgery. METHODS: A retrospective analysis of patients (≥20 years) who underwent spinal fusion with instrumen

Camptocormia as a Phenotypic Variant of FSHD in the Elderly: Clinical, Genetic, and Imaging Features.

BACKGROUND AND OBJECTIVES: Camptocormia, a pathological forward flexion of the spine, is a relatively common but often unexplained postural abnormality. Facioscapulohumeral muscular dystrophy (FSHD), one of the most prevalent adult myopathies, is caused by a contraction of D4Z4 repeats on chromosome 4 and typically presents with facial, scapular, and lower limb weakness. However, atypical phenotypes are increasingly recognized. We investigated camptocormia as a presenting feature of FSHD in a large neuromuscular cohort. METHODS: This cross-sectional study assessed clinical, genetic, and muscl

Spinal Realignment for Adult Deformity: Three-column Osteotomies Alter Total Hip Acetabular Component Positioning.

INTRODUCTION: A goal of adult spinal deformity surgery is correction of sagittal imbalance by increasing lumbar lordosis (LL), allowing a previously retroverted pelvis to normalize as evidenced by decreases in pelvic tilt (PT). Realignment of pelvic orientation may alter the position of preexisting total hip arthroplasties (THAs). METHODS: Twenty-seven patients with unilateral THA who underwent thoracolumbar fusions for adult spinal deformity from the pelvis to L1 or above were retrospectively reviewed (levels fused, 10.3 [range, 6 to 17]; age, 70 ± 9 years). Comparisons of preoperative and p
