Paraganglioma of the Lumbar Spine: A Case Report.

Paragangliomas are rare neuroendocrine tumors, with spinal involvement representing a small minority of cases. Lumbar spinal paragangliomas are especially uncommon and may present with nonspecific symptoms, often mimicking more common intradural tumors. We describe a 49-year-old female who presented with chronic low back and sacral pain, along with bilateral lower extremity symptoms. Lumbar MRI demonstrated an intrathecal, homogeneously enhancing mass at L3 causing canal stenosis. The patient underwent lumbar laminectomy with gross total resection of an intradural, extramedullary lesion. Histo

Multifocal Emphysematous Osteomyelitis of the Spine and Pelvis.

Teaching point: Detection of intraosseous gas with a bubbly or irregular pattern on imaging should raise a strong suspicion for emphysematous osteomyelitis and prompt urgent antimicrobial treatment, particularly in immunocompromised patients without a history of trauma or surgery.

Psychological Impact of Spine Sarcoma on Wellbeing.

BACKGROUND: Spine sarcomas are rare but debilitating cancers requiring extensive treatment with high morbidity. While physical outcomes are well studied, the psychological impact and support needs of patients remain poorly understood. The aim of this study was to qualitatively explore patient experiences of spine sarcoma on wellbeing and their perceived support needs. METHOD: Sixteen adult patients diagnosed or treated for spinal sarcoma at Oxford University Hospitals completed semi-structured qualitative interviews. Data were analysed using thematic analysis. RESULTS: Patients frequently ex…

Branchial cleft cyst of the cervicothoracic spine.

A branchial cleft cyst (BCC) is a neck mass, present from birth, caused by incomplete development of the branchial arches during fetal development. It commonly presents as a solitary, painless mass in the neck of a child or young adult. We present a case of a large intramedullary BCC in a patient with Klippel-Feil syndrome (KFS). A male in his 60s presented with long-lasting neck pain and fast-progressing paraparesis. MRI showed a large intramedullary cystic tumour in the cervicothoracic spine with accompanying cervical diastematomyelia and extensive cervical vertebrae fusion. The patient was …

Time is spine: critical updates for the intensivist.

PURPOSE OF REVIEW: The concept of 'time is spine' emphasizes early or ultra-early surgical decompression within 24 or 12 h, respectively, after spinal cord injury (SCI) to maximize recovery. This review updates the latest findings on the timing of surgical decompression and hemodynamic management in acute SCI, focusing on neurological outcomes and complications. RECENT FINDINGS: While early decompression may improve neurological outcomes, factors like injury severity, comorbidities, and system resources affect surgical timing. Recent studies question the benefits of ultra-early decompression,…
