Perceived Changes in Communication as an Effect of STN Surgery in Parkinson's Disease: A Qualitative Interview Study.

The aim of the present study was to explore four individuals' perspective of the way their speech and communication changed as a result of subthalamic nucleus deep brain stimulation treatment for Parkinson's disease. Interviews of two men and two women were analyzed using qualitative content analysis. Three themes emerged as a result of the analysis. The first theme included sub-themes describing both increased and unexpected communication difficulties such as a more vulnerable speech function, re-emerging stuttering and cognitive difficulties affecting communication. The second theme comprise

Clinical neurophysiology of language: the MEG approach.

Clinical evaluation of language function and basic neuroscience research into the neurophysiology of language are tied together. Whole-head MEG systems readily facilitate detailed spatiotemporal characterization of language processes. A fair amount of information is available about the cortical sequence of word perception and comprehension in the auditory and visual domain, which can be applied for clinical use. Language production remains, at present, somewhat less well charted. In clinical practice, the most obvious needs are noninvasive evaluation of the language-dominant hemisphere and map

New therapeutic indications for botulinum toxins.

The efficacy of botulinum toxin (BTX) without systemic effects has led to the rapid development of applications in neuromuscular disorders, hyperactivity of sudomotor cholinergic-mediated glandular function, and pain syndromes. The successful use of BTX in conditions with muscle overactivity, such as dystonia and spasticity, has been established and new areas in the field of movement disorders such as tics, tremor, myoclonic jerks, and stuttering has been explored with satisfactory results. Strategies to temporarily inactivate muscle function after orthopaedic or neurosurgery have also been de

Audit of outcomes from pre-school stammering intervention in a defined catchment population in the west of Ireland.

We have carried out an audit of outcomes from a pre-school stammering intervention offered to all 64 children, aged under 5 years referred from a defined catchment population to a single speech and language therapist over an 11 year period (1993 -2003 inclusive). Therapy was based on a client centred eclectic approach which combines elements of direct therapy focused on the child's speech and a number of indirect approaches to treatment which focus on changing the child's environment. Follow-up was conducted by means of a short questionnaire to the parents. Non responders were assessed informa

Methodological variables in choral reading.

This preliminary study explored changes in prosodic variability during choral reading and investigated whether these changes are affected by the method of eliciting choral reading. Ten typical adult talkers recorded three reading materials (poetry, fiction and textbook) in three reading conditions: solo (reading aloud alone), track (reading aloud with a recording), and choral (reading aloud with another live talker). Measurements of fundamental frequency, amplitude, and vowel duration variability were performed. Compared to solo reading, choral reading featured decreased variability of fundame

Ambiguity in speech.

Ambiguity in speech during analysis is one manifestation of transference. Four types of ambiguity are illustrated. Each ambiguity communicates an unconscious wish or intention and the defense against this wish or intention. In each type of ambiguity, transference manifestations are expressed in the form of speech and not alone by its content, as we are accustomed to identifying the transference. Each type of ambiguity within the analysis expresses fear to responsibility and recrimination for libidinal or aggressive intentions. In some patients, the combinative ambiguity or malapropism expresse

Neural Changes in Children With Residual Speech Sound Disorder After Ultrasound Biofeedback Speech Therapy.

PURPOSE: Children with residual speech sound disorders (RSSD) have shown differences in neural function for speech production, as compared to their typical peers; however, information about how these differences may change over time and relative to speech therapy is needed. To address this gap, we used functional magnetic resonance imaging (fMRI) to examine functional activation and connectivity on adaptations of the syllable repetition task (SRT-Early Sounds and SRT-Late Sounds) in children with RSSD before and after a speech therapy program. METHOD: Sixteen children with RSSD completed an f

Viral introductions and return to baseline sexual behaviors maintain low-level mpox incidence in Los Angeles.

In 2022, mpox clade IIb disseminated around the world, causing outbreaks in more than 117 countries. Despite the decay of the 2022 epidemic and the increased immunity within sexual networks, mpox continues to persist in North America without extinction, raising concerns of future outbreaks. We combined phylodyamic inference and microsimulation modeling to understand the heterogeneous dynamics governing local mpox persistence in Los Angeles County (LAC) from 2023 to 2024. Our Bayesian phylodynamic analysis revealed a time-varying pattern of viral importations into the county, which seeded mpox

Manganese exposure during early larval stages of C. elegans causes learning disability in the adult stage.

Manganese (Mn), even though an essential trace element, causes neurotoxicity in excess. In adults, over-exposure to Mn causes clinical manifestations, including dystonia, progressive bradykinesia, disturbance of gait, slurring, and stuttering of speech. These symptoms are mainly because of Mn-associated oxidative stress and degeneration of dopamine neurons in the central nervous system. Children with excessive Mn exposure often show learning disabilities but rarely show symptoms associated with dopaminergic neuron dysfunction. It is unclear why Mn exposure shows distinctive clinical outcomes i

Deep brain stimulation fine-tuning in Parkinson's disease: Short pulse width effect on speech.

BACKGROUND: subthalamic nucleus deep brain stimulation (STN-DBS) may have a detrimental effect on speech in Parkinson's disease (PD) patients and new stimulation technologies may help in addressing this issue. OBJECTIVE: to evaluate the STN-DBS acute effect of 30 μs pulse width (30PW) versus conventional 60 μs PW (60PW) on speech and identify the core features of voice modified by 30PW. METHODS: seven STN-DBS treated PD patients participated into a pilot cross-sectional study. Motor and speech performances were tested by means of both automatic analysis and blinded clinical evaluations in fo

Effects of tDCS on Sound Duration in Patients with Apraxia of Speech in Primary Progressive Aphasia.

Transcranial direct current stimulation (tDCS) over the left inferior frontal gyrus (IFG) was found to improve oral and written naming in post-stroke and primary progressive aphasia (PPA), speech fluency in stuttering, a developmental speech-motor disorder, and apraxia of speech (AOS) symptoms in post-stroke aphasia. This paper addressed the question of whether tDCS over the left IFG coupled with speech therapy may improve sound duration in patients with apraxia of speech (AOS) symptoms in non-fluent PPA (nfvPPA/AOS) more than sham. Eight patients with non-fluent PPA/AOS received either active

A new family with spastic paraplegia type 51 and novel mutations in AP4E1.

BACKGROUND: Autosomal recessive mutations in the AP-4 (adaptor protein complex 4) complex subunit ϵ - 1 (AP-4E1) gene on chromosome 15q21.2 are known to cause spastic paraplegia 51 (SPG51). The exact phenotype of SPG51 remains poorly characterized, because only a few families have been reported as carriers of the mutation. In addition, a previous study identified an autosomal dominant mutation in the AP4E1 gene as being associated with persistent stuttering. The aim of the current study was to characterize the phenotype of a paediatric patient with an identified novel AP4E1 mutation presenting

Heritability of non-speech auditory processing skills.

Recent insight into the genetic bases for autism spectrum disorder, dyslexia, stuttering, and language disorders suggest that neurogenetic approaches may also reveal at least one etiology of auditory processing disorder (APD). A person with an APD typically has difficulty understanding speech in background noise despite having normal pure-tone hearing sensitivity. The estimated prevalence of APD may be as high as 10% in the pediatric population, yet the causes are unknown and have not been explored by molecular or genetic approaches. The aim of our study was to determine the heritability of fr

TIDE Analysis of Cryptosporidium Infections by gp60 Typing Reveals Obscured Mixed Infections.

BACKGROUND: Cryptosporidiosis is a parasitic disease associated with potentially fatal diarrhea. The most used method in Cryptosporidium subtyping is based on the glycoprotein gene gp60. Each infection can represent a parasite population, and it is important to investigate the influence on transmission and virulence, as well as any impact on public health investigations. However, an easy-to-use method for detection is lacking. METHODS: Here we report on the use of the bioinformatic program TIDE for deconvolution of gp60 chromatograms. A combination of single oocyst analysis and cloning succes
