Absence of both stapedius tendon and muscle.

During surgery for otosclerosis, it is common for the surgeon to cut the stapedius tendon. The absence of the stapedius muscle with its tendon is uncommon. In this study, we present a case of the absence of the unilateral stapedius tendon and muscle. During dissections of adult temporal bones, the absence of the stapedius tendon and muscle was found in one case. The tympanic cavity was explored with the help of a surgical microscope. The pyramidal process was not developed. A possible ontogenetic explanation was provided. In the presented case, the cause of the anomaly may be failure of the em

Imaging the infrapatellar tendon in the elite athlete.

Extensor mechanism injuries constitute a major cause of anterior knee pain in the elite athlete. Sonography and magnetic resonance imaging (MRI) are the imaging methods of choice when assessing the infrapatellar tendon. A comprehensive imaging review of infrapatellar tendon normal anatomy, tendinopathy, and partial/full-thickness tendon tears is provided. The value of imaging the infrapatellar tendon in clinical practice, including whether sonography can predict symptoms in asymptomatic athletes, is discussed. Acute avulsion fractures, including periosteal sleeve avulsion, and chronic avulsion

Tendon injuries across the world: treatment.

This article outlines current status of primary and secondary surgical treatment of flexor and extensor tendon injuries in the hand and rehabilitation regimens worldwide. Unsolved problems associated with tendon repairs in the hand are tendon adhesions, rupture of the repairs, finger stiffness, power of hand motion, and surgical skills. Future development may include improvement of tendon healing through biological approaches, repair techniques, and, in particular, establishment of adequate rehabilitation systems and training of surgeons in some regions of the world.

The magic-angle effect of the supraspinatus tendon.

CONTEXT: Rotator cuff injuries are common and result in either a tear or tendinopathy within the distal portion of the supraspinatus tendon. Magnetic resonance (MR) imaging shows the injury as a high signal region within the distal tendon. Similarly, a magic-angle effect also appears within healthy patients in this same region on T1- or proton density (PD)-weighted MR sequences. OBJECTIVE: This study was conducted to evaluate the prevalence of the magic-angle effect found with MR imaging within the supraspinatus tendon. METHODS: In this study, 300 consecutive patients were selected from thos

Tendon avulsion injuries of the distal phalanx.

UNLABELLED: Mallet injuries are the result of incompetence of the terminal tendon. Most acute mallet fingers can be treated by continuous splinting of the distal interphalangeal (DIP) joint in extension for 5-6 weeks. Fracture dislocations require open reduction and internal fixation. Treatment of chronic mallet injuries must be individualized. If there is a flexible swan neck deformity, spiral oblique ligament reconstruction is indicated. For a fixed contracture, DIP joint arthrodesis is preferred. Profundus avulsion injuries, or jersey finger, seen within 10 days of injury require operative

Current concepts in the management of tendon disorders.

Primary disorders of tendons are common and constitute a high proportion of referrals to rheumatologists. Certain tendons are particularly vulnerable to degenerative pathology; these include the Achilles, patella, elements of the rotator cuff, forearm extensors, biceps brachi and tibialis posterior tendons. Disorders of these tendons are often chronic and can be difficult to manage successfully in the long term. Significant advances have been made in understanding the pathophysiology of these conditions. Histopathological evidence, together with advances in imaging techniques, has made us more…

Achilles tendon disorders: etiology and epidemiology.

The Achilles tendon is the strongest tendon in the human body. Because most Achilles tendon injuries take place in sports and there has been … is known to be multi-factorial. Although histopathologic studies have shown that ruptured Achilles tendons have clear degenerative changes before the rupture, many Achilles tendon ruptures take place suddenly without any preceding signs or symptoms.

Tendon and ligament: development, repair and disease.

Tendons and ligaments (T/L) are very similar fibrous tissues that respectively connect muscle to bone and bone to bone. They are comprised of fibroblasts that produce large amounts of extra-cellular matrix, resulting in a dense and hypocellular structure. The complex molecular organization of T/L, together with high water content, are responsible for their viscoelastic properties, hence insuring their mechanical function. We will first review recent work on tendon embryology and discuss ligament formation, which has been less documented. We will next summarize our current knowledge of T/L mole…

Disorders of the distal biceps brachii tendon.

Pathologic conditions of the distal biceps brachii tendon are of clinical interest, with partial and complete tears being the most common. However, the anatomy of the distal biceps brachii tendon makes imaging of the distal tendon somewhat difficult. An innovation in patient positioning for magnetic resonance (MR) imaging of the distal biceps tendon was recently described in which the patient lies prone with the arm overhead, the elbow flexed to 90 degrees , and the forearm supinated, so that the thumb points superiorly. The acronym FABS (f lexed elbow, abducted shoulder, forearm supinated) ha

[Quadriceps tendon ruptures: evaluation and treatment].

OBJECTIVES: We evaluated the patients who underwent surgical repair for rupture of the quadriceps tendon. METHODS: Five patients (4 men, 1 woman; mean age 56 years; range 33 to 77 years) had rupture of the quadriceps tendon at the osteotendinous junction. One patient had bilateral rupture. Etiology was a traffic accident in one patient, and fall from height in two patients. Two patients who were on dialysis treatment developed simultaneous bilateral rupture. Ruptures were repaired with heavy, nonabsorbable sutures placed in transosseous tunnels in the patella. The mean follow-up was two years

Ankle pain and peroneal tendon pathology.

Chronic ankle pain can be due to multiple causes. A thorough review of the patient's history with a physical examination concentrating on anatomic structures surrounding the ankle is imperative. The most common of causes have been presented. The addition of provocative testing and radiographic examinations can aid in elucidating the pathology. After treatment of the injury, attention to training technique, shoe and insert usage as well as individual gait abnormalities are integrated into global patient education to decrease the incidence of injury recurrence.

Outcomes following quadriceps tendon ruptures.

Complete rupture of the quadriceps femoris tendon is a well-described injury. There is a scarcity of literature relating to the outcome of patients with this injury after surgery. We undertook a retrospective analysis of patients who had surgical repair of their ruptured quadriceps tendon at our institution over a seven year period-totalling 27 patients. Males were more commonly affected with a M:F ratio of 8:1. The peak incidence was in the sixth decade of life. The non-dominant limb was twice as likely to be affected. Assessment consisted of completion of a functional knee questionnaire, cli

The blood supply of the posterior tibial tendon.

We studied the vascular pattern of human posterior tibial tendons by injection techniques and immunohistochemically using antibodies against laminin. The intravascular volume of the posterior tibial tendon was determined using a new method of injection of a solution of 99mTc and gelatin ink into the lower legs of cadavers. Three segments of 1 cm length from different regions of the human posterior tibial tendon were measured using a gamma well counter. The main blood supply arises from the posterior tibial artery. Blood vessels enter the paratenon of the posterior tibial tendon via a mesotenon…

Ocular fixation with quadriceps tendon allograft.

Management of difficult strabismus, such as strabismus fixus and paralytic strabismus, in order to maintain the alignment is complicated. There are many surgical approaches described in the current literature, together with notes on the materials used to stabilize these deformities. We present a new surgical approach using quadriceps tendon allograft for the correction of difficult strabismus such as strabismus fixus and paralytic strabismus. Our idea for using deep frozen quadriceps tendons developed from the fact that this is the most stable and strongest tendon in the whole body. Six patien…
